Eligibility and Requirements. You are eligible for and may receive the Services only if You (a) purchase the System independently or directly from CT and, unless CT is responsible for installing the System under a separate written and signed agreement with You, correctly install or have installed the System in good working order at the Premises, (b) should a digital communicator be used, furnish and maintain a suitable telephone line at Your own cost and pay for any device required and any monthly charge or other change from the telephone provider, (c) if and as requested by CT, register Your System on-line with the Platform (which may require computer with internet connection) and/or provide a list of the names and phone numbers of persons that You wish CT to have contacted upon receipt of Signals from the System (such list, the “Emergency Contact List”, and each such person, an “Emergency Contact”), and (d) for certain Services, have a wireless device with internet access (for remote access to the System.)
Eligibility and Requirements. Membership in IFTA, Inc. shall be open to any state of the United States of America, the District of Columbia, any province or territory of Canada or a state of the United Mexican States. Any such entity desiring membership must submit an application to IFTA, Inc. The application must be in accordance with Article XIV of the International Fuel Tax Agreement (hereinafter referred to as the “Agreement”). All members must pay the annual membership fee adopted at an annual meeting of IFTA, Inc. and required by the Agreement. Continued membership in IFTA, Inc. is contingent upon compliance with all terms of the Agreement.
Eligibility and Requirements a. To qualify for on-campus housing, the student should be currently enrolled as full-time status. If the student is classified at part-time status, approval is needed for the student to live and/or remain on campus as long as the student is not in violation of any university policies and/or regulations.
b. If a student has any family members (spouse, and children only) residing with them, the student signing the contract is responsible for educating them on all university policies and is held responsible for any and all of their actions. Family members can be asked to leave, and/or evicted at any time due to university policy violations.
Eligibility and Requirements.
1. The recipient employee, recipient employee's family, or other person designated in writing by the recipient employee must submit a request to the HRS Department.
2. The recipient employee is not eligible so long as the employee has paid leaves available, however, the request may be initiated prior to the anticipated date leave balances will be exhausted.
3. A confidential medical verification including diagnosis, prognosis, and estimated date of return to work must be provided by the recipient employee.
4. A recipient employee is eligible to receive one hundred eighty (180) working days of donated time per employment.
5. Donations shall be made in full-day increments of eight (8) hours, and in increments of four (4) hours for less than full-time employees. Employees may donate unlimited amounts of time. All donations are irrevocable. In addition, employees with vacation balances that exceed the amount that can be paid off, may donate unlimited amounts of vacation to a Department catastrophic sick leave pool.
6. The donor employee may donate vacation, compensatory time or in lieu holiday time which shall be converted to recipient employee's sick leave balance and all sick leave provisions will apply. Time donated in any pay period may be used in the following pay periods. No retroactive donations will be permitted.
7. The donor's hourly value will be converted to the recipient's hourly value and then added to the recipient's sick leave balance on a dollar-for-dollar basis.
8. The recipient employee's entitlement to personal disability leave will be reduced by the number of hours added to the recipient's sick leave balance.
9. The determination of the employee's eligibility for Catastrophic Sick Leave donation shall be at the County's sole discretion and shall be final and non-grievable.
10. Recipient employees who are able to work but are working less than their regular schedule will integrate Catastrophic Sick Leave donations with time worked and their own paid leaves, which must be used first, not to exceed 100% of the employee’s gross salary.

Third party fees may apply for data and Internet access. Contact your smart phone device carrier for additional information. You MUST have the Glatco Credit Union mobile application installed on your smart phone device. The Glatco Credit Union mobile application can be downloaded from your device’s application store. The operating system version must be compatible with the latest version of the application as determined by your device’s application store. You MUST be enrolled in FLEXteller in order to download the app. You MUST be enrolled in Remote Deposit Capture to be able to deposit checks electronically from any location.
Eligibility and Requirements. A. All first year unmarried students living further than twenty miles from the Colorado School of Mines campus are required to reside on campus throughout the students first academic year. The Associate Director of Housing shall only authorize approvals for first year Residency housing waivers.
B. In order to be eligible to reside in the Residence Halls, students must be enrolled in a minimum of 12 hours per semester. Exceptions to this requirement may be made only by the Associate Director of Housing.
