MEDICAL / PHARMACY / VISION / DENTAL INSURANCE. 13.1 The District agrees to make available medical, vision, dental benefits to each employee. District contribution, shall be limited to the following: The District shall contribute one thousand, four hundred twenty seven ($1427) for 2020-21, unless any other employee group receives a higher monthly District contribution, in which case the OSEA employees will receive the same amount. Employees working less than an eight (8) hour work day shall receive medical, vision, dental benefits on a prorated basis (employees working six (6) hours would receive 75% of benefits, etc.). In order to be eligible for Medical / Pharmacy / Vision / Dental insurance, an employee must work an average of three (3) hours per day or a total of fifteen (15) hours per week. 13.1.1 The District will also make available a cancer insurance plan, long term disability insurance plan, and accidental death coverage to all employees, although purchase of these benefits will be optional. Employees who are currently diverting dollars not used as specified in Article 13.1, as of October 1, 2013, may divert any dollars not used as specified in Article 13.1 or may use personal funds to pay for cancer insurance, long term disability, and accidental death coverage in accordance with the conditions listed below: 1. The individual district contribution for optional coverage costs may continue at the same benefit level for the same optional coverage (i.e. Life Insurance amount, disability level, etc.). 2. Employees who opt to take the high deductible health care plan are not eligible to divert district contributions to optional insurances. Employees who have not been diverting dollars not used as specified in Article 13.1 as of October 1, 2013, or new employees hired after October 1, 2013, will not have this option of diverting dollars not used as specified in Article 13.1 available to them. However, they may use personal funds to pay for cancer insurance, long-term disability, and accidental death coverage. Eligible employees may select a high deductible health plan and may divert 50% of unused individual monthly District contribution into a Health Savings Account (HSA). 13.1.2 The Union will review and select the Medical / Pharmacy / Vision / Dental benefits for the bargaining unit. District contribution, shall be limited to the following: The District shall contribute nine hundred thirty dollars ($930) per member per month for 2008-09 to cover the purchase of family medical, pharmacy, vision and dental insurance. For the 2009-10 school year, the District shall contribute one thousand thirty dollars ($1030) per month. For the 2010-11 school year, the District shall contribute one thousand one hundred thirty dollars ($1130) per month. The Association will notify the District of its selection of plans by May 15 of each contract year to allow notification time to the insurance carrier(s). 13.2 Insurance will be provided twelve (12) month/year according to the employees’ established daily hours of work. 13.2.1 Part-time employees who work full-time (40 hour work week for complete month) in any month shall receive the full District contribution for the following month. 13.2.2 Employees’ entitlement to insurance and other fringe benefits, including leaves: Any employee who is consistently working additional hours that he/she would qualify for a higher insurance reimbursement than currently received may ask for a review by the Director of Human Resources.   • Group Dental Insurance Not available to part-time Station Attendants. Group insurance coverage for temporary full-time employees will be in accordance with XXX #1. Such benefits, once established, are retained even if an employee's status reverts back to part-time, providing that employment has been continuous.

  • Dental Insurance The State agrees to pay one hundred percent (100%) of the employee premium of a dental insurance program for full-time employees. The benefit levels of this program shall provide one hundred percent (100%) coverage for preventive care and eighty percent (80%) coverage for general service care. The State agrees to provide payroll deduction for dental insurance, provided such arrangements are agreed to by the insurance carrier. Dependent coverage will be available provided there is sufficient employee participation in the dental insurance program. Dependent coverage will be at the employees' expense.

  • Medical, Dental and Vision Insurance a. Effective July 1, 2002, medical benefits shall be offered through CalPERS Health Plans. b. The Employer shall pay up to eight percent (8%) of future premium increases for medical, dental, and vision plans. In the event that a medical plan has a premium decrease (<0%), the Employer will apply ninety percent (90%) of the premium decrease towards Employer contribution and ten percent (10%) towards employee plan premiums. c.   • Vision Care Insurance The District agrees to provide vision care insurance for 39 eligible employees. The Medical Eye Services plan provides one (1) comprehensive 40 examination every twelve (12) consecutive months; two (2) pairs of lenses in any 41 twenty-four (24) consecutive months. Employee is responsible for paying a ten 42 dollar ($10) deductible per calendar year. Prior enrollment in the plan is required. 43

  • Basic Medical Insurance All regular Employees may choose to be covered by the medical plan for which the British Columbia Medical Plan is the licensed carrier. Benefits and premiums shall be in accordance with the existing policy of the plan. The Employer will pay one hundred percent (100%) of the regular premium.

  • Health and Dental Insurance ☐ Husband ☐ Wife shall maintain coverage for each minor child under the medical and dental insurance provided through his/her employment. To facilitate the use of such coverage for the child(ren), the Couple shall cooperate fully and in a timely manner, including, but not limited to, obtaining and providing all necessary insurance cards and claim forms, completing and submitting all necessary documents, and delivering all insurance payments. For purposes of duration and modification, this provision shall be deemed part of the child support orders made by the local court in the Couples’ dissolution action.

  • Retiree Medical Insurance Retiree insurance coverage is included within each medical plan for all retirees under the age of 65 years, through self-payment. The Employer shall make available an appropriate medical plan for all eligible retirees ages 65 years or older.

  • Health Care Insurance While a faculty member is on an approved leave of this type, the faculty member will be advised regarding the right to continue health care benefits in accordance with COBRA during the period of unpaid absence.
