OBLIGATIONS OF THE INSTITUTION. Investigator after Termination Závazky zdravotnického zařízení / zkoušejícího po ukončení klinického hodnocení 8.5.1 Immediately upon receipt of a notice of termination, the Investigator shall stop entering potential patients into the Study and shall cease conducting procedures, to the extent medically and ethically permissible, on patients already entered into the Study. Po obdržení oznámení o ukončení klinického hodnocení zkoušející neprodleně ukončí zařazování potenciálních pacientů do klinického hodnocení a přestane u pacientů, kteří již do klinického hodnocení byli zařazeni, provádět úkony v míře lékařsky a eticky přípustné. 8.5.2 In the event of early termination of this Agreement by the Sponsor or ICON and subject to an obligation on the Institution/Investigator to mitigate any loss, ICON shall procure that the Sponsor shall pay all third party costs incurred and falling due for payment up to the date of termination, and also all non-cancellable third party expenditure falling due for payment after the date of termination which arises from commitments reasonably and necessarily incurred by the Institution/Investigator for the performance of the Study prior to the date of notice of termination, and agreed with the Sponsor. No further compensation shall be payable to Institution or Investigator. Na žádnou další kompenzaci nemá zdravotnické zařízení ani zkoušející nárok.

OBLIGATIONS OF THE INSTITUTION. (a) The Institution agrees: (i) to work collaboratively with the Health Authority to ensure the effective operation and administration of the Programs, including scheduling the practice education experience and instruction for Students and attendance by Institution Staff; (ii) to ensure all Students are duly registered at the Institution, are in good standing and satisfy all of the Institution’s admission and performance standards necessary for participation in the Programs; (iii) to work with the Health Authority to designate HA Staff and/or Institution Staff who will be engaged in planning of the practice education experience and instructing Students in the course of the Program; (iv) to consider, and where appropriate, recognize significant involvement of members of HA Staff in the Program through academic appointments to the Institution, subject to the Institution’s policies and procedures; (v) to take reasonable steps to ensure that the Students and Institution Staff are aware of and comply with this Agreement, conduct themselves professionally and courteously, and that they comply with the Health and Safety Standards, any applicable intellectual property policy, and all other rules, regulations, and policies of the Health Authority that apply to the Program; (vi) to acknowledge that the Health Authority may refuse to permit a Student or member of Institution Staff to attend at any of the Facilities if a student refuses to comply with the Health and Safety Standards, including any testing or screening requirements of the Health Authority; and (vii) that the Institution is solely responsible for the operation of the Programs, and the form of instruction, design and delivery of educational services to Students participating in a Program or Programs. (b) The Institution agrees that it is an independent body, and not the agent, partner or joint venturer of the Health Authority and the Institution will not hold itself out to the public as such or make representations to Students or others that the Health Authority has approved the Programs. (c) The Institution will take all reasonable steps to ensure that all Students and Institution Staff are aware and understand standards of work place behaviour, including but not limited to, harassment, discrimination, sexual misconduct, abuse, and appropriate professional and respectful work place behaviour, confidentiality, all consistent with Health Authority practice education policies and guidelines and other rules...

OBLIGATIONS OF THE INSTITUTION. 1. The Institution shall not disclose any confidential or unpublished information obtained, or made known to it by FAO in connection with the internship assignments, and will make reasonable endeavours to ensure that the interns do not publish or otherwise disclose such information, unless they receive prior written approval from FAO (see Article VII below). 2. Use of the FAO name and logo requires prior written approval from FAO. The Institution recognizes that the FAO name and logo may not be associated with any political or sectarian cause or otherwise used in a manner inconsistent with the status, reputation and neutrality of FAO.
OBLIGATIONS OF THE INSTITUTION. 6.1 The institution ensures sufficient supervision by the BPV coordinator. The student knows at the start of the BPV who their coordinator is. Contact details can be found in the BPV folder, the practical agreement (POK), or the student file, depending on the location's practices. The school organizes and participates in the agreed contact moments between the student, the school, and the training company. 6.2 The BPV coordinator from the institution monitors the BPV's progress by maintaining regular contact with the student and the training company's supervisor. The coordinator ensures the alignment of the student's learning objectives with the learning opportunities at the company. 6.3 The institution provides the schedule in a timely manner so that both the student and the training company can plan accordingly. 6.4 The institution has the final responsibility for assessing whether the student has achieved the qualification components pursued during the BPV. The assessment procedures and methods are described in the program's educational and examination regulations (OER). 6.5 The institution considers the training company's evaluation of the student as part of the overall assessment.

OBLIGATIONS OF THE INSTITUTION. 2.1 The Institution shall: 2.1.1 retain ultimate responsibility for the education of the Student; 2.1.2 use its best efforts to prepare the Students for effective participation in the Placement; 2.1.3 appoint an academic supervisor to oversee the Student’s performance in the Placement or, if applicable, appoint an academic supervisor who shall be present during the Placement; 2.1.4 in consultation with Placement Site, determine the times of attendance, and the nature and objectives of the Placement required for the Students according to its curriculum plan; 2.1.5 provide written goals and objectives to the Preceptor prior to the commencement of the Placement; 2.1.6 select the Students, subject to Placement Site approval as to their number, for the Placement, while giving due consideration to Hospital’s objectives for delivery of quality of care; 2.1.7 provide an up-to-date list of the Students to the Placement Site; 2.1.8 ensure its Students complete and provide evidence to the Placement Site prior to the start of a Placement, the following: (a) a criminal record check, (b) current immunization record including tuberculosis skin testing, and (c) a respiratory N95 mask fit test conducted within the last two (2) years of the Placement; 2.1.9 advise the Placement Site of restrictions imposed on Students, that prohibit the Students from performing certain duties without supervision during the Placement; 2.1.10 in cooperation and with the assistance of Placement Site, ensure that the Students are oriented to and are familiar with Placement Site’ mission, vision, values, protocols and procedures; and 2.1.11 In cooperation with Placement Site, periodically, and as needed, review the experience with the Students, in order to evaluate whether the goals and objectives of the Placement are being attained. 2.2 The Institution shall require Students to strictly observe the patient’s rights to privacy and confidentiality with respect to his or her personal health information, and act in accordance with Placement Site responsibilities under PHIPA as amended from time to time.
OBLIGATIONS OF THE INSTITUTION. 4.1 Institution shall provide for a suitable location and facilities to store the Equipment in order to protect it from loss or theft, destruction, damage or impairment. 4.2 Upon request, Institution will provide Sponsor with access to the Equipment in order to perform regular service inspections and to check whether the Equipment is used in the way agreed herein.
OBLIGATIONS OF THE INSTITUTION. 5.1 The Institution warrants and undertakes that: 5.1.1 the Research Project and Grant awarded has been reviewed and approved by the Headteacher/Chief Executive of the Institution and where appropriate Chair of the School/Academy Governors; 5.1.2 the Grant shall be applied exclusively in support of the Research Project which shall be carried out by employees of the Institution and collaborative partners and, when part of the Project, volunteers including parents and carers; 5.1.3 the Research Project shall be managed and carried out: (a) with reasonable care and skill in accordance with the Institution's normal policies and procedures, all customary and applicable ethical, legal, regulatory and other standards and generally recognised good/best practice in collaborative educational research; (b) in accordance with all appropriate child protection, health and safety standards; (c) in accordance with the Research Principles; (d) without breaching the Intellectual Property Rights or Know-How of any third party; (e) with appropriate insurance put in place by the Institution for children, staff, volunteers and equipment/materials against all usual risks and liabilities; and (f) on the condition that all persons who work on the Research Project shall be subject to contractual terms that provide that all Intellectual Property Rights created by them whilst working on the Research Project shall vest in the Trust.
