Site Responsibilities a. Site will provide cooperating teachers who have a minimum of three years teaching experience and are properly certified in the area which they are teaching.
b. Site will ensure that cooperating teachers will provide assistance with planning, teaching, and evaluating student teachers in the area of preparation.
c. Site will ensure that cooperating teachers will submit, to the University Supervisor, university approved and supplied final evaluations on the student teacher in a professional manner by the last day of the assignment.
d. Site will confer with University prior to removing any student teacher from the classroom.
Site Responsibilities. The Participants must ensure that, in consultation with Main Roads' Representative, the Participants, relevant Main Roads' organisational units and all other relevant contractors and service providers engaged by Main Roads or a State Entity for the management of the existing roads and structures located within, or connected to, the Site, agree between them the roles and responsibilities of each entity with respect to:
(a) general maintenance;
(b) traffic management;
(c) network operations; and
(d) incident management, issues occurring within, or connected to, the Site and document any agreement reached in the PMP.
Site Responsibilities. The Xxxxx & Xxxxx Community Counseling Center (Counseling Center) will offer sufficient structure and stability to allow students to complete their practicum requirements at the site within a 4-month period (one Xxxxx & Xxxxx semester). This includes providing ongoing supervision and appropriate training to ensure client welfare and enhance student learning. Access to a client caseload to meet program requirements for breadth and depth of services, including a minimum of 40 direct client contact hours with an average of about 4 direct client contact hours per week throughout the contract time. A qualified supervisor on site during all times when a student is meeting with clients. If the primary supervisor in not available, a back-up supervisor will be available on site. Support to xxxxxx a learning environment. This includes time for appropriate supervision as outlined in the section on supervision expectations. Although this requires a minimum of one hour per week of individual supervision, it is expected that supervisors may need additional time to track client caseload and welfare and support the student’s skill development. Orientation to the site to ensure student understanding of agency policies and procedures including: Agency administrative and supervisory organization Client assignment and scheduling Clerical support available Paperwork and documentation expectations Protocols for reporting abuse Crisis response process for client danger to self or others (including suicide assessment supervisory support) Confidentiality procedures, release of information, and consent for treatment Acquainting students with culture and norms of setting Opportunities for the student to develop professional relationships with peers, experienced counselors, supervisors, and related agency personnel through in-service training, case staffings, and other agency activities. Ability to videotape client sessions for review during on-campus faculty supervision. An adequate workspace in which students can schedule time to see clients on a regular basis.
Site Responsibilities. Sites accepting graduate student counseling interns from Xxxxx & Xxxxx offer sufficient structure and stability to allow interns to complete their internship requirements at the site within a 9-month period. This includes providing appropriate training and supervision to ensure client welfare and enhance student learning. Access to a client caseload to meet program requirements for breadth and depth of services, including a minimum of 240 direct client contact hours with an average of about 8 client contact hours per week throughout the contract time (unless the site is designated as a secondary site only). A qualified supervisor on site during all times when an intern is meeting with clients. If the primary supervisor in not available, a back-up supervisor must be available at the location where the intern is meeting with clients. Exceptions must be approved by Xxxxx & Xxxxx prior to the internship. Support from the site administration to xxxxxx a learning environment. This includes support for site supervisors to provide adequate time for appropriate supervision as outlined in the section on supervision expectations. Although this requires a minimum of one hour per week of individual supervision, it is expected that supervisors may need additional time to track client caseload and welfare. Site Responsibilities. 1.1 Provides necessary HSE training to all the staff under Contractor and its subContractors before they start work in Site. Training covers but not limited to:

1.1.2 Work Permit and other specific permitting programs
1.1.3 Personal Protective Equipment (PPE)
Site Responsibilities. The Customer shall provide, at no expense to the Company, a 24-Foot-wide path of crushed gravel along the access drive to the facilities. The Customer is responsible for obtaining all necessary governmental approvals and permits.
Site Responsibilities. (Internship) host site will assign a site supervisor to oversee the student’s (internship) experience. Site Supervisor will review the student’s learning outcomes and their related objectives, and modify those objectives as necessary, in order to ensure a productive learning experience. Site Supervisor, working with the student and (faculty) and (coordinator), shall provide an (internship) experience that will enable the student to work toward achieving the learning objectives. Site Supervisor will meet weekly with the student to discuss the (internship) experience. Site Supervisor shall consult as necessary with the (faculty) regarding the student’s progress and provide a midterm and final evaluation of the student’s job performance.
Site Responsibilities. A. The Site controls the Site’s administrative and professional operations.
B. The Site shall provide students with safety training in all areas applicable to Site’s facility, and practice standard precautions and prudent safety practices when demonstrating procedures, and to teach and require the Student to follow these practices.
C. The Site shall provide supervision for assigned students in the internship area. The Site agrees that students will be functioning as part of the Site's workforce pursuant to 45 C.F.R. §160.103 and will be subject to the Site’s HIPAA policies and procedures. Further, the Site shall be responsible for enforcement of its HIPAA policies and procedures and compliance by students.
D. The School shall be informed regarding changes in Site’s facilities, which may affect the internship experience of the School's students.
E. The Site shall facilitate and guide the internship experiences of the student and allow for the student to observe, participate, and complete a preceptor-approved project under the guidance of the preceptor. Site will work with students to develop and achieve specific goals and objectives during the internship experience.
F. The Site shall ensure emergency care is provided to students for any accident, injury or illness. The student's health insurance shall be billed for any emergency service, and the balance billed to the student. Responsibility for follow-up care remains the responsibility of the student.
G. The Site shall provide information regarding each student's performance in the internship setting if so requested by the School.
H. When in the opinion of the Site, a student's conduct or performance adversely affects patient care, disrupts the operations of the Site, or violates the policies and procedures of the Site, the Site shall notify the School and the School and the Site shall together determine whether the Student should be removed from the internship rotation at the Site.
I. The Site shall provide access at reasonable times and with reasonable advance notice to representatives of the School to perform an on-site visit of the Site’s facility during the student’s internship experience. J. The Site acknowledges that information embodied in student education records shall be protected from disclosure pursuant to the Federal Family Education and Privacy Rights Act (FERPA), 20 U.S.C. § 1232(g) and...
Site Responsibilities. (1) The Site shall designate a Program administrator (the "Program Administrator") who shall be responsible for the conduct and administration of this Contract. The Site's Program Administrator shall be: Name: First and Last Name Title: Title Telephone: (XXX) XXX-XXXX If the Program Administrator reduces their role, or relinquishes or expects to relinquish their position, the Site shall immediately notify Primary Care Office in writing and provide Primary Care Office with the name and title of the new Program Administrator.
(2) The Site acknowledges and shall comply with the provisions of this Contract. The Site shall also comply with the Public Health Service Act, its implementing federal regulations, the program policies governing this Program, and with all other applicable Federal and State laws as they currently exist and may hereafter be amended, which are incorporated herein by this reference in the provisions of this Contract. Failure to comply with applicable federal regulations and program policies, including retention of Provider for clinical services as stated in this section, may result in future disqualification from Program.
(3) The Site shall immediately notify Primary Care Office, in writing, if the employment status of Provider changes at any time during Period of Service or if it is in default of any other term of this Contract.
(4) The Site employs Provider as an employee at-will. However, Site intends on employing Provider as employee for the next three years based on available funding and Provider’s performance.
(5) The Site shall maintain its status as a public or non-profit, private health care entity and shall immediately notify Primary Care Office, in writing, if the status of the Site changes at any time during the Period of Service.
(6) The Site shall assure that adequate malpractice insurance coverage for Provider is in place at all times.
(7) Site acknowledges that salaries for health professionals participating in this Program should be based on prevailing rates in the area and that this Contract and the loan repayment shall not be used as a salary offset.
Site Responsibilities. The Company will backfill and place excavated earth over any area of construction; the Customer is responsible for the final restoration of the construction area.
