Reporting and Documentation. 4.17.1. Suppliers must ensure that appropriate policies and documentation are in place to ensure effective ethical management practices.
4.17.2. The supplier must take all necessary steps to ensure that:
a) it is able to make relevant personal information it holds available to Coles and Certified Auditors; and
b) Coles and Coles’ appointees may collect, use, and disclose that personal information, for the purposes of the Policy and Program Requirements, as well as any applicable agreements, laws or regulations, including providing employee personal information for compliance audits. This may include providing notices and obtaining consents where required by law for Coles and Coles’ appointees to do those things.
4.17.3. The supplier must provide all reasonable assistance to Coles so that Coles can ensure the supplier’s ongoing compliance with this Policy and any applicable agreements, laws, or regulations. This may include a site inspection or audit and/or all relevant documentation arising out of or in connection with the goods or services provided by the supplier.
Reporting and Documentation a. During the term of this Agreement, County shall provide OYA with the necessary service information to track treatment and accountability services in JJIS, as defined by JJIS policy, Exhibit F “Service Tracking in JJIS” as it may be from time to time amended, or by service extracts, for progress in achieving the high level outcomes. This information provision requirement also applies to providing information on funded services not tracked in JJIS.
b. In addition to the other reporting requirement of this Agreement, during the term of this Agreement, the County shall ensure that all OYA required data fields are entered into JJIS, unless a different process is approved by OYA.
c. If the County fails to meet any of the reporting requirements, OYA may conduct a performance review of the County’s efforts under the Service Plan in order to identify ways in which the Service Plan may be improved. If, upon review, OYA determines that there are reasonable grounds to believe that County is not in substantial compliance with the Service Plan or this Agreement, OYA may notify the County regarding the alleged noncompliance and offer technical assistance, which may include peer review or other assistance, to reach such compliance. Nothing in this Section shall be construed to limit or restrict any OYA right arising out of County’s default, as described in Exhibit B.
Reporting and Documentation. 5.1 The medical institution agrees to duly complete and submit all requested data via the electronic data capture system, and in compliance with the Electronic Access Terms and Conditions.
5.2 The medical institution shall produce the documentation in a manner that makes it pertinent and useable for the project. In case of any ambiguity in respect to the manner of documentation, the medical institution shall immediately notify UNEV to clarify the issue. The medical institution acknowledges that the documentation as defined supra may be updated and amended from time to time by UNEV.
5.3 The medical institution shall correct incorrect data via the electronic data capture system as soon as the medical institution becomes aware of incorrect data. All corrections will be documented by the “audit trail” of the electronic data capture system.
5.4 The medical institution shall notify UNEV when a patient quits the project. The medical institution shall include in this notification the exact time of termination. In this case, UNEV shall immediately delete the patient data concerned.
5.5 The medical institution will take care that the source records are maintained and stored in a secure manner (in accordance with all applicable laws and regulations).
5.6 UNEV will take care that the project data entered in the electronic data capture system is maintained and stored in a secure manner (in accordance with all applicable laws and regulations).
5.7 UNEV shall ensure that the processing of results and data is consistent with provisions of data protection laws.
5.8 The medical institution hereby agrees to the processing of the investigator’s personal data provided to electronic data capture system by the investigator or obtained from other participating CARE centers. The Investigator has the right to have access to and correct his/her personal data.
5.9 The Investigator has the right to have access to and correct his/her entered data.
5.10 Adverse events and/or laboratory abnormalities identified in the entered data are not followed up by UNEV. The medical institution is in charge to take care of reporting these events in accordance with all applicable laws and regulations, if applicable.
Reporting and Documentation. The Contractor shall provide and submit to the Monitor and the County, all reports and documents as may be necessary to adequately document its performance of this Contract, to include all requirements for documentation requested by FEMA and/or State government for reimbursement of costs. These prices shall remain in effect for a period of 36 months, no exception. No work outside the scope of work contained in the RFP will be performed without the advanced written consent of the County Manager or Project Manager.
Reporting and Documentation. 11.10.1 The Researcher and YTF will report to the Company and Meytav on a continuous basis (but not less than monthly) according to the instructions of Meytav, including written reports regarding the performance of the Approved Project and all difficulties encountered and proposed solutions and will timely provide Meytav and the Company all information and documents required by them to comply with their reporting obligations to the OCS.
11.10.2 The Researcher will document all research and development results and procedures undertaken by him and the Company in accordance with acceptable scientific practices and in accordance with the instructions of Meytav from time to time.

I will develop and maintain documentation of the technical services I provide, including invoices, in accordance with USDA manuals, handbooks, and technical guidance and furnish this documentation to the local USDA Service Center office and the program participant when the particular technical service is completed.
