Therapeutic Services. 1) CONTRACTOR shall provide structured day and evening services seven (7) calendar days a week which will include individual, group therapy, and community meetings amongst the Clients and crisis residential staff. 2) CONTRACTOR shall provide group counseling sessions at least four (4) times daily to assist Clients in developing skills that enable them to progress towards self- sufficiency and to reside in less intensive levels of care. Topics may include, but not be limited to: self-advocacy, personal identity, goal setting, developing hope, coping alternatives, processing feelings, conflict resolution, relationship management, proper nutrition, personal hygiene and grooming, household management, personal safety, symptom monitoring, etc. These groups will be clearly documented in the individual’s chart. All therapeutic process groups will be facilitated by a licensed clinician or clinically supervised registered/waivered clinicians. 3) CONTRACTOR shall provide individual therapeutic sessions provided by an MD/DO/NP, licensed clinician, or clinically supervised registered/waivered staff at least one time a day to each Client and these sessions will be clearly documented in the chart. 4) CONTRACTOR shall support a culture of “recovery” which focuses on personal responsibility for a Client’s behavioral health management and independence, and fosters Client empowerment, hope, and an expectation of recovery from mental illness. Activities and chores shall be encouraged and assigned to each Client on a daily basis to xxxxxx responsibility and learning of independent living skills. These chores will be followed up on by residential staff, in the spirit of learning, who will also assist the Client in learning the new skills and completing the chores as needed. 5) CONTRACTOR’s program will be designed to enhance Client motivation to actively participate in the program, provide Clients with intensive assistance in accessing community resources, and assist Clients developing strategies to maintain independent living in the community and improve their overall quality of life. Therapeutic outings (to local museums, art galleries, nature centers, parks, coffee shops) will be provided for all Clients in support of these goals. 6) CONTRACTOR shall assist the Client in developing and working on a WRAP throughout their stay at the program and will promote Client recovery on a daily basis via individual and/or group sessions. Therapeutic Services. 2.1(1) Contractor will provide a minimum of 2 therapeutic sessions per week in the home with the family for a minimum of 30 minutes face-to-face contact per session. Therapeutic services shall be provided by an LPC, LCSW, LMFT. (Provisional Licensed Clinicians cannot provide therapeutic services) 2.1(2) Counseling services shall be provided on an individual/family basis. The treatment plan and service delivery shall include the involvement of the Juvenile Probation Department, extended family, school, peers, health services, community groups, and/or other involved agencies to effectively address the needs of the family.

Therapeutic Services. Effective July 1, 2010: a. The ITP will be based on the comprehensive assessment and will drive the individual level therapeutic intervention by making the distinction as to therapy or other therapeutic supports needed to accomplish the child’s treatment goals. The Agency shall provide or agreement, as specified in KRS 199.640(5)(a)2, for therapeutic services individualized for the child. Any child identified as needing services from a facility licensed for treatment services must receive individualized counseling dealing with the underlying causes of the child’s behavioral issues a minimum of weekly, as well as weekly group counseling. The individual counseling shall be conducted by a social worker or other treatment professional; and the group counseling shall be conducted by a trained social worker or other treatment professional as determined appropriate by the treatment team and under the supervision of the treatment director. b. The therapeutic interventions will address issues identified in the assessment, such as loss and grief, attachment, child sexual abuse, self-efficacy, and behavioral self-control. c. Therapy shall be provided by a licensed/certified individual with the appropriate qualifications. Those “other therapeutic supports geared toward accomplishing treatment goals” will be conducted by an individual whose education and experience are appropriate to the level of service needed, which would include Bachelor’s level individuals or experienced paraprofessionals. d. The person responsible for conducting the assessment will conduct formal staffing with the individual(s) responsible for implementation of the child’s ITP at a minimum of once per month. e. The Agency shall maintain control of the therapy by either providing the therapy in house or by establishing a written agreement with the outside provider with expectations clearly defined and a well-established plan for communication between the Agency and the therapist to assist in treatment planning and the continuum of care for the youth. The agreement will specify the mechanism for payment for those services. The expectation is that all therapeutic xxxxxx care providers are to provide or agreement for the preponderance of clinical services. These services are currently built into the per diem rate.
Therapeutic Services. Therapy is a relationship between people that works in part because of clearly defined rights and responsibilities held by each person. As a client in therapy, you have certain rights and responsibilities that are important for you to understand. There are also legal limitations to those rights that you should be aware of. I, as your therapist, have corresponding responsibilities to you. These rights and responsibilities are described in the following sections. Therapy has both benefits and risks. Risks may include experiencing uncomfortable feelings, such as sadness, guilt, anxiety, anger, frustration, loneliness and helplessness, because the process of therapy often requires discussing the unpleasant aspects of your life. However, therapy has been shown to have benefits for individuals who undertake it. Therapy often leads to a significant reduction in feelings of distress, increased satisfaction in interpersonal relationships, greater personal awareness and insight, increased skills for managing stress and resolutions to specific problems. But, there are no guarantees about what will happen. Therapy requires a very active effort on your part. In order to be most successful, you will have to work on things we discuss outside of sessions. The first 1-2 sessions will involve a comprehensive evaluation of your needs. By the end of the evaluation, I will be able to offer you some initial impressions of what our work might include. At that point, we will discuss your treatment goals and create an initial treatment plan. You should evaluate this information and make your own assessment about whether you feel comfortable working with me. If you have questions about my procedures, we should discuss them whenever they arise. If your doubts persist, I will be happy to help you set up a meeting with another mental health professional for a second opinion. APPOINTMENTS Appointments will ordinarily be 45-60 minutes in duration, once per week at a time we agree on, although some sessions may be more or less frequent as needed. The time scheduled for your appointment is assigned to you and you alone. If you need to cancel or reschedule a session, I ask that you provide me with at least 24-hour notice. If you miss a session without canceling, my policy is to charge you the full rate ($100) for the hour. If you cancel with less than 24-hour notice, my policy is to collect a $35 late cancellation fee [unless we both agree that you were unable to attend due to circumstance...
