The Student undertakes: • to comply with the most recent Study and Examination Regulations for the Study Program available on the University’s website (xxxxx://xxx.xxxxx.xx.xx/student-life/academic-services/rules-and- regulations), in particular, to attend lectures, classes, courses, tutorials, examinations and any other activities that form part of the Study Program, except in the case of absence for genuine medical reasons, previously agreed upon absences or other exceptional circumstances, and to follow the instructions of the lecturers and other persons responsible for the lessons, in particular to submit course work and other assignments for the Study Programs by the published deadlines and in the prescribed manner; • to follow the University’s regulations and House Rules (published on the University website, under xxxxx://xxx.xxxxx.xx.xx/student-life/academic-services/rules-and-regulations), as well as the house rules of any other institution in which courses may be held, and especially, to conduct themselves in an orderly manner, both in the academic and extracurricular activities, while in attendance at the University or engaged in any University activity; • to keep their personal data up to date, in particular their study and home address for delivery of urgent and important University documents, as well as their phone number(s) in case of emergency situations; • to notify University authorities immediately in the case of an interruption or discontinuation of studies. When a student becomes aware of a medical/personal/physical condition that disrupts their academic performance, the Xxxx must be informed as soon as possible, preferably before the start of a new semester. Agents or any other third parties involved are not official representatives of the University; • to notify the University of any changes to the information submitted on the Student’s application or registration, in particular regarding the Student’s personal data; • to agree to the University’s non-exclusive, locally and temporarily unlimited right to use all works, which have been created in the context of the Study Program, in all forms of exploitation, including the right to use such works on the Internet, networks and online learning platforms. Obligations of the Student. The student is committed to:
a) meet their obligations arising from this Contract, the Study Rules of Procedure at UPJŠ LF, Study Rules of Procedure at UPJŠ and other internal regulations at UPJŠ and UPJŠ LF, as well as the obligations arising from the laws of the Slovak Republic;
Obligations of the Student. The Student will attend in full the course of training with proper diligence and will undergo such examinations and tests as may be prescribed in or required by the training syllabus with a view to qualifying as a Public Health Nurse. The Student will be required to provide the Geographical Area with a copy of his/her examination results at the end of the academic year. If the sponsored candidate ceases employment or does not complete the programme he/she will be required to repay both the tuition fee and the portion of the salary received during the theory element of the programme. Such repayments shall be made to the HSE. Successful applicants for sponsorship will be required to give a written undertaking that, following successful completion of the programme, he/she will immediately register as a Public Health Nurse with the Nursing & Midwifery Board of Ireland (NMBI) and accept an initial assignment in their sponsoring Geographical Area in full time or part time/pro rata capacity, as a Public Health Nurse for a period of not less than 18 months from the date of registration with the Nursing & Midwifery Board of Ireland (NMBI). The Student acknowledges that failure to complete no less than 18 months continuous employment as a full-time- or pro-rata/part time Public Health Nurse in the sponsoring geographical area, on successful completion of the course, will result in the Student re-imbursing the HSE with the cost of the course fee and the value of the salary received by the student during the theory element of the programme. If monies are not received debt recovery proceedings may be initiated against the Student. A Student failing to obtain the Public Health Nursing qualification on completion of the course will, at the discretion of Health Service Executive, be retained on probation to afford him/her one further opportunity of securing the qualification at repeat examination but in any event no later than the end of the following academic year. During this repeat period the student will be remunerated as a student Public Health Nurse. No funding will be provided for repeat tuition or examination fees; such costs must be borne by the student concerned. Obligations of the Student. The Student, having been accepted to the UPCM and covered by/under the RSA Policy, shall:
1. Faithfully adhere to the Vision-Mission of the UPCM, specifically the ideals and requirements of the Program;
2. Abide by the prescribed course of instruction;
3. Obtain a Philippine Medical License within 2 years from the date of her/his UPCM graduation
4. Render health care services in the Philippines for a minimum of three (3) years within five (5) years from the date of her/his UPCM graduation; Submit on or before June 30 and December 31 of every year until completion of the Return Service Obligation, beginning the year he/she graduated from UPCM, a report of her/his health care activities and services for the preceding 6 months providing the necessary details as required by the UPCM.
Obligations of the Student. 2.1. The Student agrees to:
2.1.1. attend the CEED student induction program and other training as may be arranged in relation to the CEED Project;
2.1.2. work on the CEED Project at the location to be advised by the Industry Partner for a minimum period of [0-8] weeks during UWA vacation periods, or for an equivalent period as agreed between UWA, the Industry Partner and the Student;
2.1.3. undertake the undergraduate, Honours or postgraduate course required by the CEED Project if invited to do so by UWA;
2.1.4. present a written report detailing the progress of the CEED Project each month (for all except PhD projects) or quarterly (for PhD projects) to an appropriate representative of the Industry Partner (mentor), the Student’s supervisor at UWA and the CEED Office;
2.1.5. meet any other reporting requirements reasonably requested by the Industry Partner;
2.1.6. present the Industry Partner with the deliverables within four (4) weeks of the completion of the CEED Project or such other time as may be agreed between the parties. Such deliverables are to be in a form and contain information as required by the Industry Partner, which may be more extensive than the report required for academic assessment, and will be agreed on in the project brief; and
2.1.7. be responsible for the Student’s personal accommodation and travel expenses when required to work at sites for the CEED Project that are within a 50 kilometre radius of UWA.
2.2. The Student acknowledges that he/she:
2.2.1. will [subject to satisfactorily performance of the CEED Project receive a studentship of $<proposed studentship total>, the sum of which may be varied during the first two months of the Project depending upon which IP ownership provisions are selected by the Industry Partner and which is to be paid to the Student, by UWA, in installments over the duration of the CEED Project] OR [not receive a studentship for this Pro Xxxx CEED Project]; and
2.2.2. will not receive any other payment in relation to the CEED Project.
2.2.3. may forfeit the studentship described in clause 2.2.1 if the Student’s performance of any part of the CEED project, including administrative matters, is unsatisfactory and the Student does not take the required remedial action within 30 days after being so notified in writing by the CEED Office. Obligations of the Student. (1) The Student shall actively participate in the lectures and seminars designed for the Baccalaureate Degree Programme as set forth in the relevant curriculum, and shall abide by the relevant University Statutes and Regulations.
(2) The Student is obliged to keep all matters pertaining to his/her studies and work performed in placement organisations and at the University’s outpatient clinic confidential, in accordance with the regulations set forth by the respective organisation and pertinent data protection legislation.
(3) The University shall have free and unrestricted access to any work performed and research outcome obtained by the Student as well as to any research results gained by the Student as part of a research project and within the scope of his/her studies.
