2nd Post-Observation Conference. A. The post-observation conference between the evaluator and teacher will be held no later than five (5) days after the formal observation date.
B. The purpose of the post-observation conference is to review the evaluator’s and teacher’s evidence related to the scoring criteria during the observation, and to discuss the teacher’s performance. The teacher shall be provided an additional opportunity to submit evidence of the teacher’s professional performance that the teacher deemed was not observed in the classroom. The evidence provided by the teacher shall be incorporated on the negotiated form and will be used to determine the final evaluation score.
C. If there is an area of concern, the evaluator will identify, in writing, specific concerns for the applicable criteria and provide specific observable solutions with specific district support and resources to remedy the concern.
D. The teacher may attach written comments to the observation report.
E. The teacher may request additional formal observations.

The post-observation conference between the evaluator and teacher will be held no later than five (5) days after the formal observation unless the teacher agrees to waive the Post- Observation Conference because of the proximity in timing of the Post-Observation Conference to the Final Summative Conference.
2. The purpose of the post-observation conference is to review the evaluator’s and the teacher’s evidence related to the scoring criteria during the observation, and to discuss the teacher’s performance.
3. The teacher must be given the opportunity to attach written comments to the negotiated form.
4. If there is an area of concern, employees shall be notified of serious deficiencies that could affect their continued employment. Such notice shall include discussion of areas of concern, the applicable criteria and specific components, and the evaluator’s suggested solutions to remedy the concern. Concerns shall be placed in writing. The intent of such notice is to provide ample time for the employee to improve his/her performance.

3. The evaluator’s handwritten and/or electronic notes from this conference shall also be used as artifacts and evidence for the evaluation. If there is an area of concern, the evaluator will identify, in writing, specific concerns for the applicable criteria and provide specific recommendations for improvement with specific district support and resources to remedy the concern.
4. The final observation report shall be provided to the teacher within a maximum of two (2) days of the conclusion of the post-observation conference, which both the teacher and evaluator will sign. The teacher’s signature does not signify agreement with the observation report. The evaluator may or may not assign numerical ratings to individual components or criteria, based on the evidence collected by the time the report is prepared. The teacher will receive a copy of the completed, signed Observation Report Form (Appendix B-10). The teacher may attach written comments to the observation report.
5. The teacher may request additional formal observations. The evaluator may conduct additional formal observations, up to a total of four (4) observations. All formal observations must be a minimum of thirty (30) minutes in length, must be recorded on the negotiated form (Appendix B- 10), and a post-observation conference must be held in accordance with this subsection.

The evaluator shall complete the observation report form (▇▇▇▇▇▇▇▇ ▇-▇, Classroom Teacher Observation/Evaluation Form) during or at the conclusion of the conference and a printed copy shall be provided to the employee. The evaluator may or may not assign numerical ratings to individual components or criteria, based on the evidence collected by the time the report is prepared.

Relevant evidence provided by the teacher shall considered by the evaluator in determining the final evaluation score. Upon request, the administrator will initial and date all additional artifacts provided by the teacher.
3. If there is an area of concern observed, the evaluator will identify it in writing, linked to the applicable criteria, and provide suggestion(s) for improvement.
4. The teacher may attach written comments to the observation report.

The teacher shall be entitled to submit additional evidence to aid in the assessment of the teacher’s professional performance against the instructional framework rubric, especially for those criteria not observed in the classroom. Relevant evidence provided by the teacher shall considered by the evaluator in determining the final evaluation score. Upon request, the administrator will initial and date all additional artifacts provided by the teacher.
6. The evaluator will complete the observation report form (Appendix 2) during or within two (2) day after the conclusion of the conference, and a printed copy shall be provided to the employee. The evaluator may or may not assign numerical ratings to individual components or criteria, based on the evidence collected by the time the report is prepared. For third year Provisional employees there will be an additional Pre-observation, formal Observation, and Post-Observation cycle as delineated in subsections D, E and F, above.
