A Doctor's Release clause requires an individual, typically an employee, to obtain written confirmation from a licensed physician stating they are fit to return to work or resume certain activities after an illness or injury. In practice, this clause is often invoked when someone has been absent due to medical reasons, and the employer needs assurance that the person can safely perform their duties without risk to themselves or others. The core function of this clause is to protect both the individual and the organization by ensuring that health and safety standards are met before resuming normal responsibilities.

Doctor’s Release. 6 Unit members returning to work from sick leave after surgery or 7 serious illness, upon the request of the District, must provide a 8 medical doctor’s release certifying medical permission to return to 9 work listing any work related restrictions, the duration of the 10 restrictions and the medical basis for such restrictions, prior to 11 returning to work. Upon being released to return to work, a unit 12 member may be required to have a fit for duty exam before returning 13 to work.
Doctor’s Release. When an employee returns to duty after an absence equal to or longer than three (3) days chargeable to sick leave, due to his or her illness, or that of a member of their immediate family, the department head or designee may require a signed statement from a doctor or dentist that the employee was incapacitated and unable to perform his or her duties throughout the entire period of sick leave. If such a statement is not provided, the employee is not entitled to be paid for sick leave unless the department head and/or designee grants a waiver. An employee who is absent from duty due to an injury or illness arising out of and occurring in the course of County employment shall be requested to provide a doctor's release for work prior to returning to work. In the event an employee uses more than 96 hours of sick leave, or exceeds their available sick leave accrual, or if a pattern of sick leave abuse is suspected, an employee may be requested to file a Physician's Statement for each illness regardless of duration for a period not to exceed six
Doctor’s Release. A doctor’s release or medical clearance may be required by the Fire Chief to return to work following any absence from duty exceeding twenty four (24) hours due to illness or injury. The Fire Chief reserves the right to select the appropriate physician. The cost for such release shall be paid at the District’s expense. Such release must state the Employee’s ability to perform his regular Fire District duties.
Doctor’s Release. After a verbal counseling an employee may be required, at his/her Supervisor's discretion, to bring a doctor's release upon returning to work when he/she is absent because of illness or injury. Before returning to work from sick leave or a leave of absence due to illness or injury, an employee will be required to submit a doctor's release if he/she has been hospitalized for any reason or if he/she is absent for fifteen (15) successive working days. This release shall indicate that the employee is released to return to work and whether there are any medical restrictions that prevent the employee from performing all of his/her essential job duties. Failure to present this doctor's release when requested may result in the employee being withheld from service without pay, sick pay, or other compensation until an acceptable release is presented. If an employee becomes ill for more than three (3) days during his/her scheduled vacation time he/she may submit a doctor's verification of his/her illness in order to utilize his/her sick leave benefits instead of vacation time. The vacation time missed will then be rescheduled at a later time. An employee may take a leave of absence due to illness as provided for in Article 11 of this Agreement. To care for an ill spouse, dependent child, or parent, a unit member may be authorized to use up to one half (½) of the annual accrual of sick leave. Such leave may be included in the twelve (12) weeks of Leave of Absence authorized for Family Care and/or Medical Leave. An employee may take a leave of absence due to illness as provided for in Article 11.2.

On the basis of authoritative medical advice, the department head or designee shall determine whether an employee is physically incapacitated for the duties of his or her position and may take the action he or she considers appropriate.

Doctor’s Release. The Library may require a doctor’s release before the employee returns to work. If the employee has been released by the doctor to return to work after the weekly schedule has been posted and the Library has deemed that there is no place for the employee to work for that week, the employee shall be allowed to take up to a maximum of five (5) days of unpaid time off provided that at the time of return to work the employee has five (5) days or less of vacation remaining. The employee may be put on the extra hours list and given priority within the scheduling pattern for the week in question.

  • Release and Waiver I, the Volunteer, acknowledge and understand that participation in the Activities may involve certain risks, including, but not limited to, personal injury(ies), bodily injury, illness, permanent disability, property damage, loss and/or death (“Risks”). These Risks include, but are not limited to, exposure to and/or infection with COVID-19 and/or other viruses and/or bacterial infection even in ideal conditions, and despite any and all reasonable efforts made to mitigate such Risks. I further acknowledge and agree that, due to the nature of the Activities, social distancing of six feet per person will not always be possible and that my participation in the Activities may result in an elevated risk of contracting COVID- 19 and/or other viruses and/or bacterial infection. I, the Volunteer, further confirm that prior to engaging in the Activities, I may be required to complete a COVID-19 health screening questionnaire provided by one or more of the Released Parties. I agree that I will answer all questions on the questionnaire truthfully. I agree to not participate in any Activities if, at such time and to the best of my knowledge, I am a carrier of COVID-19 or infected with COVID-19. I further agree to follow all safety precautions outlined by any Released Party while volunteering. In consideration of and in order to be allowed to participate in the Activities, I do hereby release and forever discharge and hold harmless the Released Parties and their successors and assigns from any and all liability, claims, demands, costs and damages of any kind, whether arising from tort, contract or otherwise, which I or my heirs, assigns, next of kin or legal representatives may have or which may hereinafter accrue, arise from, or are in any way related to my Activities with any of the Released Parties, including but not limited to Risks, whether caused wholly or in part by the simple negligence, fault or other misconduct of any of the Released Parties or of other volunteers, other than their intentional or grossly negligent conduct. In addition, the Released Parties shall have the benefit of any future liability protection for businesses as relating to the COVID-19 pandemic passed by any governmental entity to which the Released Parties are subject. I understand and acknowledge that by signing this Release I knowingly assume the Risks associated with the Activities. I also understand that the Released Parties do not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance in the event of injury, illness, death or property damage. Regarding any illness or virus, including COVID-19, I, the Volunteer, understand that even if I follow all guidelines for the prevention and handling of any illness or virus, including COVID-19, there is still a risk that Volunteer could contract such virus or illness.
