HEALTH FUND. 1. The Employer shall make contributions to a health trust fund, known as the “Building Service 32BJ Health Fund,” to cover employees covered by this Agreement who work more than two (2) days per week, with such health benefits as may be determined by the Trustees of the Fund. The Employer may, unless rejected by the Trustees, upon execution of a participation agreement in the form acceptable to the Trustees, cover such other of its employees as it may elect, provided such coverage is in compliance with law and the Trust Agreement. Employees who are on workers’ compensation or who are receiving statutory short term disability benefits, Building Service 32BJ long term disability benefits, or a Building Service 32BJ disability pension, shall be covered by the Health Fund without employer contributions until they may be covered by Medicare or thirty (30) months from the date of disability, whichever is earlier. In no event shall any employee who was previously covered for health benefits lose such coverage as a result of a change or elimination of the Health Fund provision extending coverage for disability. In the event the provision extending coverage for disability is discontinued for any reason, the Employer shall be obligated to make contributions for the duration of the period that would have otherwise been available.
2. Effective January 1, 2020, the rate of contribution to the Health Fund shall be $20,496.00 per year for each covered employee, payable when and how the Trustees determine.
3. Effective January 1, 2021, the rate of contribution to the Fund shall be $21,240.00 per year for each covered employee.
4. Effective January 1, 2022, the rate of contribution to the Fund shall be $22,188.00 per year for each covered employee.
5. Effective January 1, 2023, the rate of contribution to the Fund shall be $23,196.00 per year for each covered employee.
6. The parties agree that if there is governmental health care reform mandating payment, in full or part, by a contributing Employer for some or all of the benefits already provided for in the Health Fund to participants, the parties shall meet to discuss what ameliorative steps, if any, might be appropriate to minimize any adverse impact on the Funds, its participants and Employers. HEALTH FUND. Section 8.1 Effective June 1, 2020, each Employer covered by this Agreement shall pay the amount as stated on Schedule B per hour for each hour worked by all Employees (whether Union members or not) covered by this Agreement to the Trustees of The Wisconsin Laborers’ Health Fund. These payments shall be made not later than the 15th day of each month following the month for which payment is being made. Payments to such Health Fund are to be made out to: WI Laborers’ Fringe Benefit Funds and mailed to WI Laborers’ Fringe Benefit Funds, Lockbox 978844, X.X. Xxx 0000, Xxxxx Xxxxxx, XX 00000-0000. All of the hourly contributions paid by Employers to the Trustees of the Wisconsin Laborers’ Health Fund shall become part of the trust fund of such Health Fund and shall be used for health and welfare benefits, administrative costs and as may otherwise be prescribed in the Trust Agreement governing such Fund.
Section 8.2 The parties to this Agreement, and all Employers covered thereby, agree to be bound by all of the terms of the Trust Agreement governing the establishment, administration and operation of the Wisconsin Laborers’ Health Fund, and of the Plan established there under, amended from time to time, and, further, agree to be bound by all of the actions, rules and regulations heretofore and hereafter adopted by the Trustees in accordance with the Trust Agreement and Health Fund.
Section 8.3 The parties to this Agreement, and all Employers covered thereby, hereby accept as trustees, the Trustees appointed under and in accordance with such Trust Agreement, and all succeeding Trustees as shall have been or will be appointed under and in accordance with such Trust Agreement. The Employers and the union hereby ratify all of the actions already taken or to be taken by such Trustees within the scope of their authority.
Section 8.4 The Trustees are hereby authorized to establish a schedule of liquidated damages to be assessed against, and to be paid by, the Employer who fails to make timely payments to the Wisconsin Laborers’ Health Fund in accordance with Section 8.1 of this Article.
Section 8.5 The Trustees or Administrator of the Health Fund, to which Fund payments are required to be made by the Employers under this Agreement, are authorized by the parties hereto to seek any appropriate legal, equitable, or administrative relief to enforce the terms of this Agreement for the purposes of collecting any payments required to be made to such Funds, including damages a...
HEALTH FUND. (a) Each Employer covered by this Agreement shall pay monthly, effective June 1, 2019, to the Wisconsin Laborer’s Fringe Benefit Funds, Lockbox 978844, XX Xxx 0000, Xxxxx Xxxxxx, XX 00000-0000 the amount listed in Exhibit “A”.
(b) All of the hourly contributions paid by Employers to the Trustees of the Wisconsin Laborer’s Fringe Benefit Funds pursuant to this Article shall become part of the trust fund of such Fund and shall be used for health and welfare benefits, administrative costs and as may otherwise be prescribed in the Trust Agreement governing such Fund.
HEALTH FUND. Subject to the applicable provisions of Chapter 87, HRS, as amended, the Employer shall pay the following monthly contributions to the Hawaii Public Employees Health Fund effective July 1, 1995:
A. Sixty percent (60%) of the monthly premium for the Health Fund's medical plan for each employee-beneficiary or for each employee-beneficiary with a dependent-beneficiary enrolled in a health benefits plan covering medical, hospital and surgical benefits only.
B. Sixty percent (60%) of the monthly premium for the Health Fund's adult dental plan for each employee-beneficiary or for each employee-beneficiary with a spouse enrolled in the adult dental plan.
C. Sixty percent (60%) of the monthly premium for the Health Fund's prescription drug plan for each employee-beneficiary or for each employee-beneficiary with a dependent- beneficiary enrolled in the prescription drug plan.
D. Sixty percent (60%) of the monthly premium for the Health Fund's vision care plan for each employee-beneficiary or for each employee-beneficiary with a dependent- beneficiary enrolled in the vision care plan.
E. One hundred percent (100%) of the monthly premium for the Health Fund's children's dental plan for each child of an employee-beneficiary who has not attained the age of nineteen (19) and who is enrolled in the children's dental plan.
F. One hundred percent (100%) of the monthly premium for the Health Fund's group life insurance plan for each employee-beneficiary enrolled in the group life insurance plan. For purposes of this Article only, the following definitions and formulas shall apply:
A. Monthly premium" - the total amount of insurance premium for an active Employee by type of enrollment payable on a monthly basis to the insurance carrier under a contract with the Hawaii Public Employees Health Fund.
B. Health Fund's medical plan or adult dental plan or prescription drug plan or vision care plan or children's dental plan or group life insurance plan" - shall be that Employee benefit plan as determined by the Health Fund's Board of Trustees with the largest number of active Employee enrollments as of December 31 of the previous fiscal year.
C. Whenever the Employer's monthly contribution to the Hawaii Public Employees Health Fund is less than one hundred percent (100%) of the monthly premium amount, such monthly contribution shall be rounded to the nearest cent as provided below:

During the life of this Agreement each Employer covered by this Agreement shall pay the designated amount for each hour worked by all employees covered by this Agreement to the Wisconsin Laborers’ Fringe Benefit Funds, Lockbox 978844, X.X. Xxx 0000, Xxxxx Xxxxxx, XX 00000-0000 The Association and the Union, and all Employees covered by this Agreement, agree to be bound by all the terms of the Trust Agreements creating the Wisconsin Laborers' Health Fund, and by all the actions and rules of the Trustees administering such Health Funds in accordance with the Trust Agreements and regulations of the Trustees, provided that such Trust Agreements, actions, regulations and rules shall not be inconsistent with this Agreement. Each Employer covered by this Agreement hereby accepts all succeeding Trustees as will be appointed under and in accordance with the Trust Agreements. HEALTH FUND. (WGA Proposal No. 13)
a. The parties agree that the Trustees of the Health Fund shall implement plan design changes which shall reduce costs in an amount not less than $7 million, as estimated by the consultants to the Health Fund, during calendar year 2018. The Management Trustees shall not unreasonably object to the Guild Trustees’ proposed plan design changes.

HEALTH FUND. A. Effective as of July 1, 2019 the annual per active employee contribution to the Health Fund is $19,348.

HEALTH FUND. “The health fund, established and known as the ‘Writers Guild-Industry Health Fund,’ is funded and administered as follows:
1. Company agrees to contribute to the Health Fund amounts equal in the aggregate to eight and one-half percent (8.5%)11 ten and one-half 11 “ Effective May 2, 2015, the Trustees of the Writers Guild – Industry Health Fund exercised their authority to divert one-half percent (0.5%) from the salary increases for the period May 2, 2015 through May 1, 2016, and to increase the Health Fund contribution rate by one-half percent (0.5%), resulting in a Health Fund contribution rate of nine percent (9.0%) for the same period. “Effective May 2, 2016, the Trustees of the Writers Guild – Industry Health Fund exercised their authority to divert one-half percent (0.5%) from the salary increases for the period May 2, 2016 through May 1, 2017, and to increase the Health Fund contribution rate by one-half percent (10.5%) (eleven percent (11%) effective May 2, 2018 and eleven and one-half percent (11.5%) effective May 2, 2019) of all ‘gross compensation’ earned and paid or due to writers for services covered by and subject to this Agreement performed on or after the effective date hereof, in an employment capacity (to which employment the provisions of this Basic Agreement apply). “In connection with the purchase of literary material from a professional writer, if the Company also employs the writer under this Basic Agreement for at least one rewrite or polish (as required in Articles 16.A.3.c. and 16.B.3.h., or otherwise), the Company shall contribute to the Health Fund for such rewrite or polish an amount equal to eight and one-half percent (8.5%) ten and one-half percent (10.5%), eleven percent (11%) effective May 2, 2018, and eleven and one-half percent (11.5%) effective May 2, 2019) of the sum of the purchase price, up to the [Internal Revenue Code] Section 401(a)(17) limit, plus the amount paid for such rewrite or polish, but in no event shall the Company be required to make such contributions on sums in excess of the ceiling set forth in Article 17.B.1.e. or this Article 17.C.1. “Such amounts shall be contributed as and when the compensation is paid to the writer. HEALTH FUND. 31-1 A health fund shall be established for all unit members of a .5 FTE or more. The contribution for each full-time unit member shall be $980 in each year of the agreement. Administrative costs shall be deducted from the contributions. Part-time staff members of .5 FTE or more shall receive a prorated contribution. Staff members as of October 1 of each year shall be eligible for this benefit.
