INSURANCE AND ANNUITY PLAN. A. The Committee will pay the maximum percentage permitted by law of the cost of the following types of insurance coverage:
1. A $5,000 term life insurance plan and a $5,000 accidental death and dismemberment policy. Upon retirement $5,000 life insurance without the accidental and dismemberment policy; The City of Fall River and the Fall River Public Employees Committee, of which the Association is a member, have entered into a separate agreement regarding health insurance (see Appendix D).
B. Teachers will be eligible to participate in a "tax sheltered" Annuity Plan established pursuant to the United States Public Law No. 87-370.
INSURANCE AND ANNUITY PLAN. A. In the event of any change in Town policy, the COMMITTEE will request the Selectmen to pay the maximum premium permitted by the Town policy of the cost of the following types of insurance coverage:
1. A life insurance plan of the type presently available to supervisors.
2. Individual or family coverage, whichever applies in the particular case, for the current Town approved health plan presently available to supervisors. S Effective September 1, 2012 the premium split for individual plans will change from 79% Employer and 21% Employee to 75% employer and 25% Employee; the premium split for the family plan will change from 77% employer and 23% Employee to 75% Employer and 25% Employee and from 73% Employer and 27% Employee to 75% employer and 25% Employee.
B. In the event a supervisor is on leave of absence in accordance with Article VIII, Sections G and H for health reasons or leave of absence under Article X, the insurance benefits provided above shall continue for the duration of that leave, payments to be apportioned according to State statute.
1. Payments of insurance premiums shall be made on the policies provided for in paragraph A above to provide insurance coverage for the full 12 month period commencing September 1 and ending August 31.
C. Supervisors will be eligible to participate in a "Tax Sheltered Annuity Plan" established pursuant to United States Public Law, No. 87-370 and pursuant to General Laws, Chapter 71-37B.
INSURANCE AND ANNUITY PLAN. A. 1. The employer will pay the following percentages of the cost of the type of insurance coverage listed below:
a. Eighty percent (80%) term life insurance plan offered by the Town of Plymouth. Additional, optional life insurance is available within the group plan at the employee's expense.
b. For individual or family coverage of group health insurance provided by the Town of Plymouth for eligible employees, in accordance with the Public Employee Committee Agreement between the Town of Plymouth Public Employee Committee and the Town of Plymouth contribution rates shall be made as follows: At the onset of the Agreement, the Committee shall continue to contribute 80% (eighty percent) of the premium cost, and the subscriber shall contribute 20% (twenty percent). Effective July 1, 2016, the Committee shall contribute 77.5% (seventy-seven and a half percent) of the premium cost, and the subscriber shall contribute 22.5% (twenty-two and a half percent). Effective June 30, 2018, the Committee shall contribute 75% (seventy-five percent) of the premium cost, and the subscriber shall contribute 25% (twenty-five percent). The Committee agrees that the level of benefits shall be substantially equal to the level of benefits provided as of July 1, 1997.
c. Except to the extent required by law, the employer shall not be required to pay the premiums outlined above for those unit members who are on a non- paid leave of absence.
INSURANCE AND ANNUITY PLAN. A. 1. The Revere Public Schools will pay the maximum percentage adopted by the City of Revere of the cost of a $5000 term life insurance plan and of single or family coverage (whichever applies in the individual case) for the most comprehensive Blue Cross/Blue Shield Insurance Plans available to employees of the Revere Public Schools under the provisions of Chapter 32B of the General Laws.
INSURANCE AND ANNUITY PLAN. The provisions of Article XVI are applicable to Special Needs Instructors who are employed twenty (20) hours or more per week. Special Needs Instructors shall be entitled to maternity leave in accordance with Massachusetts General Laws, Chapter 149, Section 105d. Further, the COMMITTEE may grant a maternity leave in excess of eight weeks for a period not to exceed the remainder of the one-year appointment.
INSURANCE AND ANNUITY PLAN. 1. The Committee will pay the maximum percentage permitted by town meeting enactment of the cost of the following types of insurance coverage:
a. A five thousand dollars ($5,000) term life insurance plan of the type presently available to teachers.
b. Payroll deductions shall be available to bargaining unit members for the Accidental Death and Dismemberment insurance available through the Massachusetts Teachers Association.
2. The Stoneham Teachers Association has ratified their health insurance agreement through the Public Employee Committee with the Town of Stoneham to enroll in health insurance through the Massachusetts Group Insurance Commission from July 1, 2015 to June 30, 2018.
3. Teachers will be eligible to participate in a "Tax Sheltered" Annuity Plan established pursuant to United States Public Law No. 87-370 and the "cafeteria plan" established pursuant to Chapter 125 of the Internal Revenue Code.
INSURANCE AND ANNUITY PLAN. 1. Prior to the start of the new school year, each employee shall receive, in addition to his/her contract, a statement of such employee’s benefits as appropriate, including but not limited to health, life and disability insurance.
2. The District will pay the percentage hereinafter provided for the cost of one of the following types of insurance selected by an employee:
a. The cost of a Point of Service plan (POS) with such benefits and through such organizations as the District may from time to time elect.
b. The cost of a Preferred Provider Plan (PPO) with such benefits and through such organizations as the District may from time to time elect.
c. The cost of a Health Maintenance Organization plan (HMO) with such benefits and through such organizations as the District may from time to time elect.
3. 4. The above premium percentages will be in effect for the three (3) years of the contract provided that the Berkshire Health Group (BHG) makes no changes in the individual plans offered. Should the BHG force a change in any particular plan type, the District and the Association agree to reopen negotiations solely for the purpose of discussing Article XIX, Section 3 regarding the premium co-payment rates. Such reopening will apply only to Article XIX, Section 3 and will not impact any other Articles or items in the successor contract.
6. Employees will be eligible to participate in a group life insurance plan. A $10,000 life insurance policy is available, 89% of the premium cost which is borne by the District, unless modified in accordance with Massachusetts General Laws, Chapter 32B.
7. Employees who work more than thirty (30) hours per week, will be eligible to participate in the long-term disability plan offered by the District providing each participating employee authorizes payment of the full premium through payroll deduction.

1. Term life insurance to the maximum amount authorized by the Town.
2. Individual or family coverage, whichever applies in the particular case, for Blue Cross and Blue Shield, or the type presently available to staff members.
3. Other health insurance plans presented by the Association and approved by the Committee, which they may legally provide for staff members.

b. Individual or family coverage for Blue Cross/Blue Shield, of the type presently available to Employees.
c. If the Town of Xxxxxx, through the process prescribed in Chapter 32B of the M.G.L., adopts a group dental insurance plan; the Committee and the Association will meet to negotiate any changes required in the Collective Bargaining Agreement.
