Type of Application and Award. ‌ Type(s) of applications sought: New, Competing Continuation. Funding will be provided in the form of a cooperative agreement. A cooperative agreement, as opposed to a grant, is an award instrument of financial assistance where substantial involvement is anticipated between HRSA and the recipient during performance of the contemplated project. As a cooperative agreement, HRSA Program involvement will include: • Providing input on dissemination strategies and selection of publications/products for distribution including target audiences for dissemination; • Assuring the timely cooperation of the RHRCs in responding to information requests by the awardee; • Providing input into the decisions on content, presentation approach, and selection of products/publications; • Reviewing and providing comments on draft products and publications, such as research alerts, prior to dissemination to diverse audiences; • Providing input and knowledge on the current and future policy issues that will be informed by past and future RHRC studies; • Providing input on metrics that will be used to evaluate the awardee; and • Assisting the awardee in selecting appropriate professional meetings, including the total number of meetings, at which to exhibit each year. The cooperative agreement recipient’s responsibilities will include: • Adherence to HRSA guidelines pertaining to acknowledgement and disclaimer on all products produced by HRSA award funds; • Coordinating with RHRC grantees to summarize the key research questions for ongoing research and summarize findings for completed publications/products; • Updating the Rural Health Research website and Listserv based on information from the RHRC grantees about new, on-going, and recently completed research; • Assessing the market to understand how to best target audiences who will benefit from products published by the RHRCs; • Executing and monitoring dissemination strategies to increase RHRC publication and Rural Health Research (Rural Health Research Dissemination awardee) website awareness; • Providing input to RHRCs prior to product publication regarding how to effectively communicate the research findings to diverse target audiences; • Identifying appropriate professional meetings at which to exhibit each year; • Developing brief (1-2 pages) research summaries using findings from RHRC research products on key topic areas (e.g. quality, workforce, post-acute care); and • Establishing evaluation metrics and tracking related d...

A cooperative agreement is a financial assistance mechanism where substantial involvement is anticipated between HRSA and the recipient during performance of the contemplated project. HRSA Program involvement may include: • Participating in the planning and development of the annual program evaluation portfolio, to include advising awardee on: topic, scope, and the number of projects to be completed each year. • Reviewing and approving the products prior to public dissemination. • Facilitating the collaboration on work with the Flex program, TASC, and RQITA. • Providing the information related to state Flex programs and CAH-level performance activities. The cooperative agreement recipient’s responsibilities will include: • Adherence to HRSA guidelines pertaining to acknowledgement and disclaimer on all products produced by HRSA award funds, per Section 2.2 of the HRSA’s SF-424 Application Guide (Acknowledgement of Federal Funding). • Provide data and analysis to inform CAH improvement efforts. • Conduct Flex evaluation research activities to inform state Flex programs and CAHs of effective investments. • Share pertinent findings and evidence-based best practices regarding state Flex program-level and CAH-level performance activities to key stakeholders. • Identifying expert workgroups that ensure representation across appropriate expertise areas and organizations to broaden the value to data analysis for CAHs. • Collaborate with Flex program partners such as TASC and RQITA to link research to targeted technical assistance. • Advise HRSA in crafting Flex measures to inform internal and external progress as awardees use data to improve the efficiency of their programs. • Conduct statewide and national monitoring and evaluation research projects that can inform overall Flex program impact. • Establish or maintain a national CAH Database reflective of the most recent measure reporting. • Conduct short turnaround data requests. • Conduct trend data analysis and long-term national evaluative studies.

The cooperative agreement recipient’s responsibilities shall include: • Collecting, analyzing, synthesizing, and disseminating promising practices in rural health and key findings of leading rural researchers. • Responding to ORHP requests, comments, and questions on a timely basis. • Responding to a wide range of information requests on rural issues through operation of a toll-free phone service and via email. Examples of topics covered in such requests could include recruitment and retention of health care personnel, funding of rural health clinics, the effects of health care reform on rural areas, and use of non-physician providers in rural settings. • Systematically seeking out resources on a range of issues relevant to rural health and providing access to them via the web, email, social media, and other channels appropriate to rural stakeholders. • Development and maintenance of a Section 508-co...

As a cooperative agreement, HRSA Program involvement will include:  Participating in the planning and development of each HWRC’s annual project portfolio and in the selection of each HWRC’s annual projects.  Reviewing, and, if necessary, commenting on each HWRC’s project plans and methodologies presented in their project proposals.  Reviewing each HWRC’s work products, including each project’s methodology, analysis, results, policy implications, format, and tone, prior to public dissemination.  Hosting an annual meeting of the funded HWRCs.  Participating, as appropriate, in any additional meetings (beyond the annual HWRC meetings), training activities, or workgroups conducted during the project period.  Hosting quarterly conference calls with the HWRC Directors.  Providing consultation, as appropriate, to each HWRC to design strategies for disseminating HWRC work in order to target multiple audiences interested in health workforce issues. Dissemination products may include policy briefs; peer- reviewed journal articles; invited presentations; responses to inquiries from health policy programs and health policy researchers; webinars; presentations at national, regional, state, and local conferences; and other appropriate vehicle(s) for sharing rigorous, policy-oriented research. The cooperative agreement recipient’s responsibilities will include:  Adherence to HRSA requirements pertaining to acknowledgement and disclaimer on all products produced by HRSA award funds (Section VI.3.5 of this FOA).  Submitting project proposals with relevant timelines and milestones to the NCHWA Project Officer for review.  Participating in the planning and development of the HWRC’s annual project portfolio and in the final selection of projects.  Conducting health workforce projects, and synthesizing the results into reports or other work products developed for both technical and non-technical audiences.  Preparing and submitting a 250-word abstract and a 2-page brief for each completed project in addition to the project’s primary work product(s) (e.g., full policy brief, report, journal manuscript, monograph, etc.).  Submitting p...

  • Loss Mitigation and Consideration of Alternatives (i) For each Single Family Shared-Loss Loan in default or for which a default is reasonably foreseeable, the Assuming Institution shall undertake reasonable and customary loss mitigation efforts, in accordance with any of the following programs selected by Assuming Institution in its sole discretion, Exhibit 5 (FDIC Mortgage Loan Modification Program), the United States Treasury's Home Affordable Modification Program Guidelines or any other modification program approved by the United States Treasury Department, the Corporation, the Board of Governors of the Federal Reserve System or any other governmental agency (it being understood that the Assuming Institution can select different programs for the various Single Family Shared-Loss Loans) (such program chosen, the “Modification Guidelines”). After selecting the applicable Modification Guideline for each such Single Family Shared-Loss Loan, the Assuming Institution shall document its consideration of foreclosure, loan restructuring under the applicable Modification Guideline chosen, and short-sale (if short-sale is a viable option) alternatives and shall select the alternative the Assuming Institution believes, based on its estimated calculations, will result in the least Loss. If unemployment or underemployment is the primary cause for default or for which a default is reasonably foreseeable, the Assuming Institution may consider the borrower for a temporary forbearance plan which reduces the loan payment to an affordable level for at least six (6) months.

  • Method of Application Applicants for the SORACOM Air Global Service (each such applicant, an “Applicant”) shall apply for the SORACOM Air Global Service via the on-line sign-up in accordance with the procedure specified by SORACOM after the Applicant agrees to the General Terms (such application, the “Application”).

  • Acceptance and Application of Partial Payments Lender may accept and either apply or hold in suspense Partial Payments in its sole discretion in accordance with this Section 2. Lender is not obligated to accept any Partial Payments or to apply any Partial Payments at the time such payments are accepted, and also is not obligated to pay interest on such unapplied funds.
