Insurance and Benefit Plans. All insurance policies relating to the Business, including policies relating to the property, liability, business interruption, health and worker’s compensation and the lives of the employees of Seller Group; and
Insurance and Benefit Plans. 17.01 It is agreed that the application of the Company’s benefit plans relating to:
a) Supplemental Health Care;
b) The basic and supplemental Life Insurance and Accidental Death;
c) Weekly Disability Benefit; and
d) Dental Plan shall continue in respect to all eligible full time employees for the duration of this Agreement, in conformity with, and to the extent, of their general application throughout the Company for non- management employees. It is agreed and understood that the benefit plans themselves are not part of this Agreement and are not themselves subject to the grievance procedure or arbitration.
17.02 Eligibility for the benefit plans in Article 17.01 a) to d) shall be as set out in those plans, all coverages are limited to the terms and conditions set out by the Insurance Carrier in its Policies with the Company. Benefits will be made available to retirement.
17.03 The Insurance Company or carrier chosen by the Company to provide the benefits described above will be at the sole discretion of the Company. WORKPLACE SAFETY and INSURANCE BOARD
17.04 a) In the event that a Workplace Safety and Insurance Board claim is filed on behalf of a full time or part time employee, the Company agrees to advance the equivalent of sixty percent (60%) of their normal earnings after normal payroll deductions for each week of an employee’s absence until such time as the Board adjudicates the claim. The employee will be required to sign a promissory note providing for the repayment of all monies advanced on the earlier of the receipt of the Workplace Safety and Insurance benefit or the employee’s return to work. The parties agree that this promissory note is authorization under the provisions of the Ontario Employment Standards Act, 2000 for the employer to deduct the amount owing.

The Bath Community Schools Board of Education will implement the “hard cap” PA52 compliance method for the medical benefit plan coverage year, 9/1/18 to 8/31/2019 and will switch to the Simply Blue HSA PPO Plan ($1,350/$2,700 deductible), to include a prescription plan ($10/$40/$80 RX). Bath Community Schools will commit to the following premium payments for the medical benefit plan coverage year (includes HSA deductible of $1,350 for Single and $2,700 Family): Single ($7,017.72), Double/Single + One ($16,302.48), and Family ($19,703.16). Employees will be responsible for all other costs associated with the Simply Blue HSA PPO Plan and prescription plan. A. Each administrative assistant choosing Plan B shall also have a contribution made on his/her behalf in cash. The administrative assistant may then enter into a salary reduction agreement in order to place the cash into a tax-sheltered annuity as authorized by the Internal Revenue Code, of two hundred ($200) per month.
(1) If the employee voluntarily and in writing opts out of the health benefits coverage available under Plan A; and (2) provides documentation to the Board that the employee has other health coverage that meets the minimum value and coverage requirements of the Affordable Care Act. The employee shall be eligible for cash in lieu or Plan B. The Board shall contribute 80% of the cost of the full monthly premium cost toward Plan B and the employee will contribute 20% of the cost of the premium for Dental and Vision. Administrative assistant payments shall be by payroll deduction with the option of payment through an approved pre-tax Internal Revenue Code (IRC) Section 125 flexible benefits plan.
B. The Association may elect to modify the Plan A coverage provided in Section A above. Any such modification shall not serve to increase the maximum premium obligation of the Board.

1. Medical The City will provide access to medical insurance through the California Public Employees’ Retirement System (CalPERS). If an employee elects to participate in a CalPERS medical plan, the maximum monthly City contribution, including any PERS required minimum, will be made as follows: The City shall pay up to 100% of the plan selected, up to a maximum of: • $722 per month for employee only • $1,444 per month for employee+1 • $1,877 per month for employee+2 Employees may select any available CalPERS benefit plan. Should an employee select a plan with premiums in excess of the City contribution level, the employee will be responsible for payment of such through payroll deductions.
