Organizational Responsibilities. A. DOC and DSS-CD agree the following will be shared responsibilities of both agencies: 1. DOC and DSS-CD agree to define “common client” as a DOC offender who has also had contact with DSS-CD, as verified by name, date of birth, and social security number. 2. DOC and DSS-CD will develop a process to identify common clients between the two agencies. 3. DOC and DSS-CD will provide an overview of their agency’s review and referral process to the other agency, which will include relevant offender information agreed to by both agencies. 4. DOC and DSS-CD will work together towards utilizing each agency’s existing databases in order that information may be shared between the two agencies. DOC and DSS-CD will not add confidential information into this database, but will share relevant confidential information as otherwise provided in this agreement. DOC and DSS-CD agree that they will comply with all state and federal statutes and regulations and policies governing the confidentiality of the information shared between DOC and DSS-CD pursuant to this agreement. DOC and DSS- CD agree that neither will redisclose confidential information received from the other unless otherwise authorized or required by state or federal statutes or regulations. B. DOC and DSS-CD agree the following will be the responsibilities of the DSS-CD: 1. DSS-CD will notify and invite designated Probation and Parole staff to Family Support Team (FST) meetings when agreed to by the family or where necessary for the care and treatment of the child(ren), not withstanding the family’s failure to agree. 2. DSS-CD will notify and invite designated Probation and Parole staff to Continuous Quality Improvement (CQI) meetings. 3. DSS-CD will notify and share information with designated Probation and Parole staff on any hotline report of abuse or neglect concerns involving DOC common clients who are currently on Probation or Parole. 4. DSS-CD will disseminate a local DSS-CD staff directory to Probation and Parole that includes employee contact information and will update that information as required for performance under this MOU. 5. DSS-CD will provide designated Probation and Parole staff with a signed copy of a common client’s written service agreement. 6. DSS-CD will provide designated Probation and Parole staff with a record of a common client’s history when the client is an alleged perpetrator of child abuse and neglect (Child Abuse and Neglect, CA/N-5) on all prior Preponderance of Evidence findi...

Organizational Responsibilities. A. The Department and the Contractor agree that the following activities are the responsibility of the Department: 1. The Department shall work with the Contractor to schedule Employees for TST/evaluations to be administered at the Columbia-Xxxxx County Department of Public Health and Human Services. 2. The Department agrees to pay the Contractor a firm, fixed price for each TST/ evaluation as indicated herein:  TST administration and reading $20.00 per test B. The Department and the Contractor agree that the following activities are the responsibility of the Contractor: 1. The Contractor agrees to require Employees to present documentation of the Department’s approval to receive specified services at the Contractor’s site at the Department’s expense prior to rendering services and billing the Department for any services provided pursuant to this Agreement. 2. The Contractor agrees that services rendered to Employees at the local public health department without documentation of approval are at the expense of the Employees. 3. The Contractor shall provide written reports (stated in millimeters of induration) within three (3) days of administering the TST utilizing the Department’s approval form. 4. The Contractor agrees to follow section 199.350 RSMo; 19 CSR 20-20.100, Tuberculosis Testing for Residents and Workers in Long-Term Care and Correctional Centers; MO Department of Health Rules and Regulations; Centers for Disease Control and Prevention (CDC) guidelines and Department procedures for testing, vaccination and documentation. 5. The Contractor agrees that the two-step TST shall be given as a four (4) appointment process: 1) give first TST at first appointment; 2) the second appointment must occur within 48 to 72 hours of the first TST; at this appointment, assess the results of TST #1; if first TST is read as positive per the CDC Core Curriculum, then further evaluation per the CDC Core Curriculum is necessary; 3) if TST #1 is determined to be negative per the CDC Core Curriculum, give TST #2 one to three weeks from the result date of TST #1; and

OHS All employee have a responsibility to ensure requirements of the Occupational Health and Safety Act 2004 are followed. Code of Conduct All employees are required to familiarise themselves with and abide by the CBCHS Code of Conduct and ensure that their actions are in accordance with the CBCHS values and organisation policies and procedures. Community Participation All employees should assist CBCHS to promote awareness to the community that consumer, carer, and community participation at all levels of the organisation is encouraged. This includes encouraging community participation in surveys, focus groups and committees as required. Additionally, Primary Health employees must contribute as relevant to CBCHS health promotion plan by working collaboratively with other health professionals.
