This also pertains to the technology devices such as: X-box, Wii, etc. • Provide a time to discuss the reading and math activities my child brings home each day/week. • Read/Listen to my child read 20 minutes a day. • Volunteer at school/home to help my child’s classroom/school. • Contact my child’s teacher to learn ways I can help my child. • Guide my child in the understanding on how the information they are learning at school pertains to their daily life. • Promote positive use of my child’s extracurricular time • Staying informed about my child’s education and communicating with the school by promptly reading all notices from the school or the school district either received by my child or mail and responding, as appropriate. • Serving, to the extent possible, on policy advisory groups, such as the Title I Part A parent representative on the school’s School Improvement Team, the Title I Policy Advisory Committee, the District Wide Policy Advisory Council, the State’s Committee of Practitioners, the School Support Team or other school advisory or policy groups. School Responsibilities. School shall provide facilities, services and personnel to support the Program. The Program will take place during school hours. The children or teens (collectively “Students”) will attend the Program rather than their regularly scheduled class or activity. Every attempt will be made for the group meeting schedule to be coordinated in such a manner as to minimize the impact on Students’ classes and activity schedules.
School Responsibilities. The SCHOOL shall be responsible for the following with respect to the Program: the delineation of the academic curriculum; the provision of instructors; awarding any certification or degree in the Program; maintaining appropriate accreditation; and compliance with the applicable laws and rules with respect to operation of the Program. The SCHOOL shall be responsible for the selection, placement, and/or removal, and final grading of Students placed with UCLA for their clinical experience. These decisions shall be made in consultation with UCLA in accordance with the respective responsibilities of each party to this Agreement, and in accordance with mutually agreed upon criteria. The SCHOOL shall be responsible for ensuring that the Students are qualified to participate in the clinical rotation at UCLA. Prior to placing Students at UCLA, the SCHOOL shall transmit to UCLA the name(s) of the Students and any other requested information required by UCLA. The SCHOOL shall instruct Students that they are required to provide proof to UCLA that they meet the requirements for UCLA’s basic infectious disease review, which shall include health status reports for each Student assigned to clinical training, records of proof of immunization against common communicable diseases, a tuberculosis screening, and any other requirements identified by UCLA. Any exception to this provision must be approved by UCLA before placement for the Student. UCLA retains the right to remove any Student who does not meet the standards of the basic infectious disease review. The SCHOOL shall provide orientation as appropriate for the Students prior to assignment at UCLA, if applicable. The SCHOOL shall maintain all records of Student performance in the clinical program as evaluated and transmitted by UCLA. The SCHOOL shall submit in writing to UCLA the name and credentials of the person(s) designated as the SCHOOL’s Clinical Coordinator, or designee, whose responsibilities shall be to act as liaison between the SCHOOL and UCLA in the development and execution of the clinical program, the evaluation of Student performance, if applicable, and to engage in such other activities as are of mutual concern in the provision of student training.
