Submission of Claim Forms. 6.1 Claim Forms mailed to Class Members shall be pre-populated with the Class Member’s name, current address, and date of Covered Loss to the extent feasible and if such information is reasonably available. 6.2 To be considered valid and timely, a Claim Form must be signed by or on behalf of the Class Member, and mailed to the Administrator’s address as specified in the Claim Form, postmarked by the Claim Deadline, which shall be forty-five (45) days after the scheduled date of the Final Approval Hearing. Signed Claim Forms may also be scanned and uploaded on the Settlement Website by the Claim Deadline. Claim Forms may be submitted on behalf of deceased or incapacitated Class Members by Legally Authorized Representatives, with written evidence of authority. 6.3 The Claim Form will reasonably request of Class Members such information as described on the attached Exhibit C. To be eligible for a Claim Settlement Payment, Class Members must, on or with the Claim Form: 6.3.1 Confirm that the pre-populated contact information contained on the Claim Form is correct or, if necessary, correct or add additional information to any pre-populated contact or claim information contained on the Claim Form; and 6.3.2 If the Class Member under the Covered Loss is deceased or incapacitated, include written evidence that the Person submitting the Claim Form is the Legally Authorized Representative of the Class Member. The Claim Form will not require that a Class Member sign under penalty of perjury or be notarized. 6.4 Claimants who timely submit a Claim Form and are deemed eligible for a Claim Settlement Payment shall be paid in accordance with the following provisions: 6.4.1 Class Members who timely submit a materially complete Claim Form shall be paid a Claim Settlement Payment equal to 100% of the Nonmaterial Depreciation that was withheld from the ACV Payment on the Class Member’s Covered Loss and not subsequently paid to the Class Member by Defendant, as determined by Defendant as described herein, plus simple interest on that amount at the rate of 6% per annum from the date of each respective ACV Payment to the Effective Date. 6.4.2 Class Members who timely submit a materially complete Claim Form but for whom all Nonmaterial Depreciation that was withheld from ACV Payments was subsequently paid, shall be paid a Claim Settlement Payment equal to simple interest at the rate of 6% per annum on the estimated Nonmaterial Depreciation that was initially withheld from ACV Pa...

Submission of Claim Forms. 13 Class Members who seek monetary damages must complete a claim form and 14 cause it to be filed with the Claims Administrator by the claim filing deadline set forth in 15 the Preliminary Approval Order. The claim form must be postmarked or submitted 16 online on or before such date in order to be considered timely. All claim forms must be 17 signed under penalty of perjury to be considered. Submission of Claim Forms. Settlement Class Members who wish to receive 12 payment under the Settlement shall complete, sign and return their Claim Forms in accordance with 13 the instructions contained therein. Unless the Court orders otherwise, all Claim Forms must be 14 postmarked no later than fifty (50) days before the Final Approval Hearing. If requested, any 15 Official Documentation required to support a Claim Form must be submitted no later than twenty- 16 one (21) days after the Claim Deadline. Any Settlement Class Member who does not timely and 17 validly submit a Verified Claim Form within the time provided shall be barred from receiving 18 payment under the Settlement, unless otherwise ordered by the Court, but shall nevertheless be 19 bound by any Final Judgment entered by the Court.
Submission of Claim Forms. Proposed Class Members who seek recovery of a monetary award from the Claim Form Discrimination Survey Component of the Class Monetary Awards Settlement Fund must complete a Claim Form and file the completed form with the Claims Administrator by the claim filing deadline described in the Preliminary Approval Order. The Claim Form must be postmarked on or before such date in the Order to be considered timely. All Claim Forms must be signed under penalty of perjury to be considered. Failure to file a timely Claim Form, for any reason whatsoever, shall bar the Proposed Class Member from receiving a monetary award from the Claim Form Discrimination Survey Component.
Submission of Claim Forms. In order for a Claim Form to be approved as a Verified Claim, among other requirements specified in this agreement, Settlement Class Members must electronically or physically sign the Claim Form under penalty of perjury (but they need not be notarized) and Settlement Class Members may submit their Claim Forms electronically on the Settlement Website or by regular mail.

Payment to Class Members in Group 1 who do not timely request exclusion shall be paid the amount listed in the Group 1 Class Members List on or before the Claim Payment Deadline without the need to submit a Claim Form. Class Members in Groups 2, 3, 4, and 5 must timely submit a Claim Form as provided in this Section VII. 59. Any right to payments made under this Agreement shall inure solely to the benefit of Class Members and cannot be transferred or assigned to others, unless the assignment predates the date of this Agreement or is made in the ordinary course, such as a roofing company that takes an assignment from a Class Member in exchange for the repair of the damaged roof and right to make a subsequent replacement cost benefits claim. A valid assignment does not include third-party entities that purport to take class action assignments in exchange for cash 60. The Administrator will send a Claim Form in the format attached hereto at Exhibit A. To the extent reasonably practicable, the Claim Form will identify the named policyholders, the claim number, the date of loss and the location of the insured structure. In the event the Claim Form does not contain this information because it was obtained off the class settlement website or by way of telephone request to the Administrator, the policyholder will be asked to provide the following information to the extent available to the policyholder: named policyholders, claim number, policy number, date of loss and/or location of insured structure.
Submission of Claim Forms. To receive an Individual Settlement Payment from the Net 1 Settlement Amount, a Settlement Class Member must timely submit a Claim Form that satisfies the 2 requirements of this paragraph as detailed below, must not have submitted a request for exclusion, and must be 3 eligible for a payment under the formula established in paragraph 42. A Claim Form is timely if it is postmarked 4 by the Response Date and mailed or submitted as an attachment to an email to the Claims Administrator at the 5 address in the Class Notice, or if it is submitted online to the Claims Administrator, in accordance with the 6 online submission instructions to be provided by the Claims Administrator, by the Response Date. The Claim 7 Form must be signed (electronically, if submitted via online portal) under penalty of perjury. The Settlement Administrator shall maintain and administer a dedicated settlement website (xxx.xxxxxxxxxxxxxxxxxx.xxx) containing claims information and related documents, along with an electronic version of the Claim Form that Settlement Class A Members can view, complete, and submit electronically, as discussed below. The Parties shall agree on all information and documents to be posted on this website.
