Health, Dental, Vision and Life Insurance. Employees granted a leave of absence without pay shall be entitled to maintain any Health/Dental/Vision or Life Insurance program in effect; provided that the cost of all such insurance shall be borne solely by the employee. Exception: in the case of an employee on non-paid leave of absence caused by a job-related injury or illness, the County will continue to pay the County’s portion of the insurance premiums if, at the time the employee went out on worker’s compensation, s/he had a sick leave balance of 192 hours or more. The employee will be exempt from the accrual requirement if s/he can demonstrate s/he would have had the 192 hours had it not been for a serious injury or illness requiring the use of sick leave in excess of 4 weeks (160 hours) at one time.
Health, Dental, Vision and Life Insurance. 15.1 All Full Time and Regular Part-Time employees are eligible to participate in the District's Group Health Insurance Program.
15.2 Coverage for new employees and eligible dependents shall become available the first of the month following completion of the initial sixty (60) calendar day employment period.
15.3 Health Plan Design and Premiums: The plan design is described in Health Insurance Plan Design (Appendix C). This plan will remain in effect from January 1, 2023 through December 31, 2025 as described below.
15.4 Premiums for participation in health, dental and vision plans are as outlined in Health Insurance Premiums (Appendix D). Subsequently, the District will look at the annual actuarial study projecting claims costs. If the plan costs are projected to exceed 10%, the plan design and premium costs may be changed through the meet and confer process. lf the costs are projected to be 10% or less, then premiums will be set based on the projected annual increase. The percentage increase will be split between the District and the employees; the employee premium cannot increase more than 10% per year.

(Vision coverage is provided under the available Kaiser plan).
3.2 The eligibility for a District fringe benefit contribution is:
3.2.1 Those employees who work less than full time shall receive a prorated district contribution for fringe benefits in the same ratio as their part time employment bears to full time employment.
3.3 An alternate choice of health and dental insurance plans will be provided as described herein. Medical plan choices include: SISC Anthem Blue Cross HMO plans (20 and 30), Anthem Blue Cross PPO plans (G and J), or SISC Kaiser HMO plans (20 and 30); said plans shall be subject to modification and/or replacement by SISC. Dental plan choices include: Delta Dental PPO and HMO plans.
3.4 Unless modified by subsequent negotiations between the parties, the District's sole obligation for the 2013-14 school year & beyond with regard to the maintenance of employee fringe benefits as described herein, shall be to contribute the amount of an FTE employee’s premiums that it is paying as of January 1, 2014.
3.5 A $50,000 level term Life Insurance Policy shall be made available to full time, active unit members, effective November 1, 1990. District shall pay the tenthly cost for the premium for eligible employees only.

Superintendent may enroll in her choice of the health, dental, and vision insurance coverage under the policies carried or sponsored by the District as provided for full-time, 12-month, administrative employees of the District and the District shall pay all the costs associated with this coverage for the Superintendent and her family. In addition, the District shall procure life insurance for the Superintendent with a death benefit equal to one and a half times the Superintendent’s annual base salary.

A. The cost of premiums for health, dental, and vision insurance will be paid as set forth below. Exception: in the case of an employee on non-paid leave of absence caused by a job-related injury or illness, the Court will continue to pay the Court’s portion of the insurance premiums if, at the time the employee went out on worker’s compensation, they had a sick leave balance of 192 hours or more. The City shall provide comprehensive medical, dental, and vision insurance for the City Manager and his dependents, and agrees to pay the full premium for such coverage. The City shall also provide the City Manager with an amount, not to exceed $1,300 annually, to be used for the purpose of providing additional short-term disability, long-term disability, life insurance/ADD or other insurance coverage as of the effective date of this agreement.

The Company shall designate an HMO health plan as the Company Health Plan. The Company may offer additional health plan options. Employees who choose any of the other health plan options shall pay the difference between the Company’s contributions for the Company Health Plan premium cost as set forth above and the alternative health plan option premium cost.
