Health Plan Benefits. The Company shall at all times during the Continuation Period cause Employee and Employee's eligible dependents to be covered by and to participate in, to the fullest extent allowable under the terms thereof, all group health insurance plans and programs that may be offered to the executives of the Company so that Employee will receive, at all times during the Continuation Period, the same benefits under such plans and programs that Employee would have been entitled to receive had he remained an executive of the Company; provided, however, (i) Employee must timely pay the "active" employee premium, if any, for such continued coverage; (ii) Employee must timely elect COBRA continuation coverage thereunder upon his termination of employment; and (iii) in the event Employee and his eligible dependents become covered during the Continuation Period by another employer's group health plan or programs which does not contain any effective exclusion or limitation with respect to any pre-existing conditions, then the Company's group health plans shall no longer be liable for any benefits under this paragraph (b).
Health Plan Benefits. 40.01 The Board agrees to make the following Health Plan Benefits available to employees participating in the University’s group Contracts. The Board shall continue to pay monthly premiums for core coverage based on the current cost shared arrangements. (approximately 80% Employer and 20% Employee)
Health Plan Benefits.
34.01 Subject to Article 4, the Employer and Employee shall share the premium cost of the Government Employees' Group MyCHOICE Extended Medical Benefits Plan and the MyCHOICE Prescription Drug Plan for participating Employees as follows:
(a) one-half (1/2) the cost of the family premium for the MyCHOICE Core coverage, where the Employee and dependents are covered under either or both Plans; or
(b) one-half (1/2) the cost of the single premium for the MyCHOICE Core coverage, where only the Employee is covered under either or both Plans; or
(c) if the Employee selects the MyCHOICE Enhanced coverage for either or both Plans, the Employer’s contribution towards the cost of the single or family premium under the MyCHOICE Enhanced coverage shall be the same as the Employer’s contribution towards the cost of the single or family premium under the MyCHOICE Core coverage, with the Employee paying 100% of the additional premium cost between the MyCHOICE Core and Enhanced coverage.
34.02 Employees shall participate in the MyCHOICE group benefit plans for Government of Alberta Employees in the Bargaining Unit in accordance with the terms and conditions contained in the Appendix to the Agreement Establishing the Government of Alberta Employees’ Group Extended Medical Benefits Plan Trust. The terms and conditions shall not be considered as incorporated in this Collective Agreement by reference or necessary intendment. Differences respecting any matters related to the administration and application of the MyCHOICE group Extended Medical Benefits and/or Prescription Drug plans are not subject to the grievance and arbitration provisions of this Collective Agreement. The Union shall be provided with a copy of the benefit plans.
34.03 The MyCHOICE Dental Plan will be totally funded by the Employer for participating Employees who elect the MyCHOICE Core coverage. The Employer’s contribution towards the cost of the single or family premium under the MyCHOICE Enhanced coverage shall be the same as the Employer’s contribution towards the cost of the premium under the MyCHOICE Core coverage, with the Employee paying 100% of the additional premium cost between the MyCHOICE Core and MyCHOICE Enhanced coverage.
34.04 Employees shall participate in the MyCHOICE group dental plan for Government of Alberta Employees in the Bargaining Unit in accordance with the terms and conditions contained in the Appendix to the Agreement Establishing the Government of Alberta Employees’ Group Dent...
Health Plan Benefits. A31.01 All “Full-time” and “Part-time” Employees are eligible to participate in the Health Care Plan Benefits and the Employer shall pay:
(a) one-half (1/2) of the cost of the family premium where the Employee and his family are covered under the Plan; or
(b) one-half (1/2) of the cost of the single premium where only the Employee is covered under the Plan;
(c) one-half (1/2) of the cost of the monthly premium for those Employees who participate in the Hospital Employees’ Group Extended Medical Benefits Plan.
A31.02 An Employee on approved Employer business outside of Canada who becomes ill and requires medical attention and/or hospitalization shall be reimbursed, upon production of receipts, for such charges that are in excess of those allowed by the Alberta Health Care Insurance Plan and the Hospital Employees’ Group Extended Medical Benefits Plan.
Health Plan Benefits. 35.01 The Employer will pay 50% of the premiums of extended medical care coverage for all probationary and permanent Employees on the following basis:
(a) 50% of the family rate for wage earners with dependants;
(b) 50% of the single rate for all other Employees.
35.02 The Dental Plan as described in the Letter of Understanding Dental Plan, will be totally funded by the Employer.
Health Plan Benefits. 15.01 The Employer agrees that all employees covered by this Agreement shall be covered under and protected by the Health Care Plan provided for by the UNITE HERE Union, Local 40. The Employer shall contribute, on behalf of each employee covered by this Agreement, the amount reflected in Appendix "A" for each hour worked.
Health Plan Benefits. The Employer agrees to share the monthly premium costs of eligible employees for the following benefits when available to the Employer through the Alberta School Employees Benefit Plan:
Health Plan Benefits. 23.01 Effective January 1, 1992 the Employer agrees to pay eighty-five percent (85%) of the monthly premium costs of eligible Employees for the following benefits when available to the Employer through the Alberta School Employees Benefit Plan or an equivalent benefit plan that provides the same benefit coverage as stipulated in this Collective Agreement:
a) Life and Accidental Death and Dismemberment (Schedule 2) - eighty-five percent (85%) of premium
b) Long Term Disability (Plan D) - eighty-five percent (85%) of premium
c) Extended Health Care (Plan 1) - eighty-five percent (85%)of premium
d) Dental Care (Plan 3) - eighty-five percent (85%) of premium
23.02 Effective January 1, 1992, the Employer agrees to pay eighty-five percent (85%) of eligible Employees monthly premiums payable for the Division’s Group Alberta Health Care Plan.
23.03 School Year Employees, with the exception of Education Assistants, shall receive shared benefits as outlined in Article 23 on a twelve (12) month (yearly) basis. Effective August 1, 2002, Education Assistants shall receive shared benefits as outlined in Article 23 on an eleven (11) month basis.
23.04 Payments made towards benefit plans by the Employer shall permit the Employer to retain and not pass on to Employees any rebate of premiums otherwise required under Canada Employment and Immigration Commission regulations.
23.05 The Employer shall make available to all eligible Employees brochures outlining the above plans.
23.06 Participation in the benefits plan is subject to that plan’s eligibility requirements, including requirements for a minimum number of hours worked.
Health Plan Benefits. (a) The Board shall pay the total premium costs of the Extended Medical Plan for all full-time continuing Faculty Members, their spouses, children and dependents.
(b) The Board shall pay one-half the total premium costs of the Extended Medical Plan for Continuing Part-Time Faculty Members appointments and a prorated amount for Sessional Full-Time Faculty Members proportionate to the length of employment, if eligible as determined by the terms and conditions of the plan.
(c) Sessional Part-Time Faculty Members may participate in the Extended Medical Plan by paying the entire cost of the premiums, if eligible as determined by the terms and conditions of the plan.
(d) A Faculty Member on College business outside Canada who becomes ill and requires medical attention and/or hospitalization shall be reimbursed, upon production of receipts, for such charges that are in excess of those allowed by this Agreement.
(e) Subject to the administrative guidelines, all Continuing appointments and Sessional Full-Time appointments will be eligible to receive a Health Spending Account, with monthly allocations prorated from the following annual maximums: Continuing Full-Time Family - $800 Single - $400 Continuing Part-Time Family - $480 Single - $240 Sessional Full-Time Family - $600 Single - $300
Health Plan Benefits. 29.01 A staff member on approved Institute business outside of Canada who becomes ill and requires medical attention and/or hospitalization shall be reimbursed, upon production of receipts, for such charges that are in excess of those allowed by the Alberta Health Care Insurance Plan and the NAIT Extended Medical Benefits Plan.
29.02 Subject to Section 3, the Institute shall pay the premium costs of the NAIT Extended Medical Benefits Plan, or a comparable plan, for participating staff members.
