DHS Responsibilities a) The DMS or its designated representative shall manage the Pharmacy Services Lock-In Program and communicate directly with the HMOs regarding their members.
b) The DMS or its designated representative will monitor prescription drug usage for members enrolled in the Pharmacy Services Lock-In Program.
c) The DMS or its designated representative will accept select review requests from the HMO for potential Pharmacy Services Lock-In Program members. Not all select reviews may result in intervention letters or lock-in for the member.
d) The DMS or its designated representative will accept referrals from the HMO for the Pharmacy Services Lock-In Program. DMS or its designated representative will proceed with Pharmacy Services lock-in for referred members.
e) The DMS or its designated representative may request additional information from the HMO for referrals. The HMO must provide requested information to DMS or its designated representative.
f) The DMS or its designated representative will identify the lock-in pharmacy and the HMO will identify the lock-in primary prescriber for each member. In addition, the HMO will identify any alternate prescribers for restricted medications, as appropriate.
g) The DMS or its designated representative will send letters of notification to the lock-in member and HMO for the lock-in pharmacy.
h) The DMS or its designated representative will provide an electronic monthly report to the HMO that identifies any members in the Pharmacy Services Lock-In Program for the specific HMO.
i) The DMS or its designated representative will coordinate with the HMO for the Pharmacy Services Lock-In Program policies and procedures.
DHS Responsibilities. DHS will use reasonable efforts, within the law, to undertake the following:
a. Facilitate production by enabling reasonable access, as permitted by law and consistent with DHS policy and operational requirements, to:
(1) DHS facilities and/or facilities maintained, controlled, or secured by DHS; (2) records, personnel, and DHS Visual Identities necessary to assist Company in completion of the Production.
b. Participate in media events related to the Production only as requested by Company and/or the Production’s distributors. Producers are responsible for arranging with the network to provide promotional digital assets (trailers, images) to DHS at least one week prior to premier and/or launch date. In conjunction with agreement, DHS will not post such assets in advance of the network’s scheduled promotion. Further, DHS will not disclose any DHS proposed Promotional Materials relating to the Production without Company’s prior written approval in eachinstance.
c. Designate a DHS Representative to be made available to the Company to facilitate any necessary assistance and serve as a point of contact for all technical and other research for this Production. This includes scheduling interviews, tours, and film footage necessary for the research on this Production.
d. Provide reasonable assistance in finding appropriate DHS materials (including audio, video, transcripts, and/or photos) (“DHS Materials”) to be incorporated into the Production. If the DHS Materials include an identifiable person, and using the DHS Materials for commercial purposes may infringe upon that person's right of privacy, to the extent the person is a current or former DHS personnel or a third party under DHS control, Company will work with the DHS Representative in identifying such person(s) and DHS will provide reasonable assistance to Company in obtaining permission from such person(s) for usage of the DHS Materials by the Company. Company will be solely responsible for obtaining permission from such person(s) for usage of the DHS Materials by the Company for all others that are not under DHS control. Company recognizes that only each identifiable person, and not DHS, has the right to waive their own privacy rights, and DHS will not attempt to unduly influence any identifiable person to waive their rights if they do not wish to do so. DHS Responsibilities. DHS will: • Amend existing OARs to allow service recipients to live-in family member’s homes without being on the lease or mortgage of their family member’s home. • Notify the service recipient of the upcoming programmatic change. • Notify RAFH of the programmatic change within ten (10) working days of execution of this LOA. The Union will be provided RAFH communications in advance to review and provide feedback. The Union agrees to provide such feedback in a timely manner. • Assist the service recipient in service planning to determine if the RAFH provider will transition to a HCW, providing direct care, or if other HCW services will need to be located. • Transfer currently licensed RAFH providers, with an approved license who will be providing direct care to the service recipient to the HCW program without requiring a criminal background check. o After the initial transfer, the HCW will be required to have their criminal history rechecked at a minimum of every two (2) years.
DHS Responsibilities. 1. DHS shall train Child Protective Services (CPS) caseworkers to provide a BWEIP brochure to every family with a child under the age of 34 months who is investigated for child abuse or neglect.
2. DHS shall refer any child under the age of 34 months for whom abuse or neglect has been substantiated as defined by CAPTA to BWEIP, using a weekly electronic data transfer from the DHS SAFE system to BWEIP Baby Toddler Online Tracking System (BTOTS),
DHS Responsibilities. 1. DHS is the principal state agency responsible for Mass Care, Housing and Human Services, including shelter services under the ESF-6, as designated by Executive Order of the Governor, and as referenced in the Georgia Emergency Operations Plan (GEOP), developed under the authority of the Georgia Emergency Management Act of 1981 (O.C.G.A. §38-3-1, et seq).
2. Coordinate with other agencies as appropriate during mass evacuations or no notice events to ensure CTN shelters are open in appropriate locations to meet situational needs.
3. Coordinate with ARC to provide support staff at shelters.
4. Provide mass care to disaster victims including fixed site and mobile feeding.
5. Supply emergency first aid services at all mass care facilities and designated sites within the disaster area to supplement emergency health and medical services available to disaster victims and workers.
6. Coordinate and implement the Disaster Supplemental Nutrition Assistance Program (D-SNAP) when appropriate.
DHS Responsibilities. 1. The Under Secretary for Science and Technology will establish a DHS-led Senior Management Council (SMC). The Senior Management Council will establish the mission priorities for the IMAAC. DHS will designate the chairperson of this body.
2. DHS will establish the Senior Scientific Advisory Council (SSAC) to provide scientific guidance and advice to the IMAAC Director consistent with mission priorities established by the Senior Management Council. The SSAC may occasionally consult with subject matter experts and other stakeholders outside the Federal Government.
3. The IMAAC will be led by a Director who is a Federal employee. The IMAAC Director will report to the Assistant Secretary for Programs, Plans, and Budget within the Science and Technology Directorate, or his designee.
4. DHS will assign the necessary resources to support the IMAAC within approved appropriations. Staffing will be coordinated with the IMAAC and provided through individual agency agreements.
DHS Responsibilities. 10.4.1 If DHS learns that the DMO has a prohibited relationship, as defined in Section 10.1.1, or if the DMO has a relationship with an individual who is an affiliate of such an individual, DHS may continue the Agreement if the DMO terminates the prohibited relationship within thirty (30) calendar days, unless the Secretary directs otherwise.
10.4.2 If DHS learns that the DMO has a prohibited relationship with an individual or entity that is excluded from participation in any Federal health care program under section 1128 or 1128A of the Social Security Act, DHS may continue the Agreement if the DMO terminates the prohibited relationship within thirty (30) calendar days, unless the Secretary directs otherwise.
10.4.3 If DHS learns that the DMO has a prohibited relationship with an individual or entity that is excluded from participation in any Federal health care program under section 1128 or 1128A of the Social Security Act, DHS may not renew or extend the Agreement, unless the Secretary provides to DHS and to Congress a written statement describing compelling reasons that exist for renewing or extending the Agreement despite the prohibited affiliation.
DHS Responsibilities. 6.3.1 DHS will acknowledge receipt of the CRB’s Title IV-E quarterly report within three (3) days of receiving the report.
6.3.2 DHS will include CRB’s Title IV-E eligible costs in DHS’ cost allocation process. DHS’ cost allocation process culminates in the filing of the quarterly Title IV-E federal claim.
6.3.3 DHS will notify CRB when they have filed the quarterly Title IV-E federal claim.
6.3.4 DHS reserves the right to withhold the CRB related costs from the federal claim if, and only if, the CRB Title IV-E claim will result in a loss of the Title IV-B transfer authority. DHS shall notify CRB within five (5) days of DHS’ receipt of information which could reasonably indicate that DHS would withhold CRB Title IV-E eligible costs.
DHS Responsibilities. A. Payment DHS shall make payment to the Grantee within forty-five (45) days after receipt by DHS of the Grantee’s Statement of Expenditure Report detailing program related expenditures as set forth in the budget attached to any NFAs that are associated with this Agreement and the Weatherization Assistance Program Monthly Programmatic Report (DHS-1071). DHS reserves the right to withhold payment(s) until all required reports are received.
B. Maximum Amount of Agreement DHS shall notify the Grantee of funding availability associated with this Agreement through an NFA. DHS has no commitment to reimburse the provider for any costs not included in the budget approved by DHS. No reimbursement shall be made by DHS for any costs incurred by the provider until and unless a budget is approved by DHS. The NFA shall be signed by DHS and incorporated by reference into this Agreement unless the Grantee sends written notice of disagreement with the terms specified in the NFA within 14 days of its receipt. DHS hereby agrees to pay the Grantee an amount not to exceed the amount identified in the NFA for activities performed under this Agreement exclusively for the period indicated in the NFA.
C. Cash Advance Payment DHS shall make a one-time cash advance payment as specified in the initial Notice of Funds Available (NFA) to the Grantee within approximately 30 days after full execution of the NFA. This one-time cash advance payment shall not exceed 10% of the Agreement total specified in the initial NFA. Recovery of the one-time cash advance payment shall be made through deductions from each payment to the Grantee during the Agreement period in which the one- time cash advance payment is made, to prevent total payments from exceeding the Agreement amount for that Agreement period. The formula for the deduction shall be: (current xxxx amount/total current Agreement period amount) multiplied by the one-time cash advance amount. DHS reserves the right to accelerate the rate of recovery when, in the sole opinion of DHS, the amount of previous and/or future xxxxxxxx is anticipated to be less than that needed to assure full recovery of the one-time cash advance payment from the current year’s award. In such a case, payments may be adjusted to recover up to 100% of the outstanding one-time cash advance payment from a single billing. DHS Responsibilities. DHS will share resources and information, as appropriate and as available staff and resources allocations allow, with Trinity as follows:
1. DHS will share information with Trinity, leveraging the Pathways Programs during the academic year, and will work to expand the visibility into DHS opportunities for students and recent graduates.
2. DHS will share information with Trinity to increase the pipeline of talent through DHS Component-specific internship programs to students and recent graduates that provide incentives such as scholarships (tuition and fees), monthly salary, housing allowances, and other benefits.
3. DHS will annually develop a series of webinars geared towards students and recent graduates at Trinity to provide valuable information on the DHS mission, current job opportunities, and tips for Federal resume writing and application process.
4. DHS will participate in the Trinity recruiting and outreach opportunities, events, and tours including career fairs, on-campus career orientations, and other awareness activities to inform students, faculty, staff, and alumni about DHS employment and student opportunities. This will include exploring opportunities to highlight DHS’s mission and student opportunities at Trinity conferences as a sponsor, advertiser, exhibitor, or contributor to presentations to students.
5. DHS will work with Xxxxxxx, when invited, to participate in and disseminate information across Headquarters and Components about public policy forums, lectures, presentations, conferences, national level exercises, trainings, and other events at Trinity.
6. To strengthen the partnership between DHS and Institutions of Higher Education (IHEs), DHS will research and review existing MOUs with IHEs to enhance our collaborative efforts and align with existing best practices.
7. DHS and Trinity will assess return on investment for the Department on outreach and recruitment activities to ensure accountability and prudent stewardship.
