SCOPE OF WORK/DELIVERABLES. The work to be performed for this RFP is described in the Scope of Services incorporated as Attachment 2.
SCOPE OF WORK/DELIVERABLES. The work to be performed for this RFQ is described in the Background and Scope of Services incorporated as Attachment 2.
SCOPE OF WORK/DELIVERABLES. The Contractor shall:
1. Connect the telemonitoring (TM) records of Visiting Nurse Association of Xxxxxxxxxx and Xxxxx Isle Coun ties (VNA), Central Vermont Home Health & Hospice (CVHHH) and Franklin County Home Health Agency (Franklin) to their own respective electronic medical records EMRs.
2. Connect the telemonitoring (TM) records of Visiting Nurse Association of Xxxxxxxxxx and Xxxxx Isle Counties (VNA), Central Vermont Home Health & Hospice (CVHHH) and Franklin County Home Health Agency (Franklin) to the Vermont Health Information Exchange (VHIE).
3. Create a new repository for telemonitoring results data in the VHIE.
4. Facilitate development of message processing to consume, transform, store, and view home health monitoring data within the VHIE. Vermont Information Technologies Leaders (VITL) will ensure that the repository and subsequent processing is scalable should the pilot be approved for expansion state-wide.
5. Establish an inbound and outbound interface with the VHIE to support interoperable clinical health care data transfer. The system shall be able to provide clinical, lab, and immunization data to the VHIE where applicable.
6. Enable providers who were not previously able to access VHIE provider portal to gain training and credentials to access VHIE provider portal.
7. Use the current VHIE provider portal, VITLAccess, to view data. Furthermore, all users with current permissions to access the provider portal will have access to these data with patient consent.
8. Participate in monthly telephone and/or on-site meetings to give programmatic progress reports with the State.
9. Compile qualitative and quantitative project results on data connections, data storage, and data use for a final evaluation report due within 45 days of the end of the contract.
10. Create a presentation on the use and function of the data created in the project and present to the Vermont Health Care Innovation Project Health Data Infrastructure Work Group, Steering Committee, and other VHCIP groups as requested.
SCOPE OF WORK/DELIVERABLES. The Construction Cost Estimator shall provide all deliverables as requested in Section 2.0 “Scope of Services” section of the RFP and as enumerated below and in Paragraph 4.2.5 above and in Paragraph 3.20, Deliverables, of this Contract.
(a) Original, logs, reports, tables, figures, photographs, exhibits and certifications prepared by the Construction Cost Estimator shall be provided to the School District.
SCOPE OF WORK/DELIVERABLES. TIMELINES
1) Details of work (SoW) for Laboratory Items/articles
a) The supplier shall be responsible to provide required items/item/articles mentioned at Annexure – 1: Bill of Material (BoM) and Annexure-2: Technical Specifications of this RFP document.
b) Supplier shall deliver the laboratory items at departmental testing laboratories across the state as per the list of locations at Annexure 16.
c) All rates will be firm and fix. The rates per chemical/glassware/filter paper/gas will be required to be quoted. The rates quoted must be for delivery at consignee stores across Rajasthan, including packing, loading, transportation, insurance, unloading, stacking, and all incidental charges, octroi and taxes except VAT/ CST.
d) Supplier shall get the items/item/articles inspected and verified by the designated officer at the respective site.
2) Project Deliverables, Milestones & Time Schedule for Laboratory Items
SCOPE OF WORK/DELIVERABLES. A. Subrecipient Shall:
1. Provide leadership for CHAC’s activities regarding budget, legal requirements, quality improvement, data repository and reporting services, in collaboration with CHAC’s senior management staff.
2. Develop and implement a care management model. Such work shall include:
a. Identification, selection, and acquisition of care management tools;
i. Implementation of telemonitoring intervention: Contractor shall engage (1) vendor Pharos Innovations, LLC and (2) a care coordination vendor organization to implement a telemonitoring clinical intervention focusing on Medicare beneficiaries with complex conditions with the goal of reducing hospitalizations. This intervention will provide approximately 20 phone calls/month/enrolled patient, and include technology to flag patients at risk for hospitalization who require clinical follow up.
ii. Implementation of analytics solution for CHAC’s claims data before or during ACO PY2017.
iii. Implementation of and event notification solution.
3. Prepare ACO quality reporting submissions for the 2014, 2015 and 2016 Program Years for the:
a. Medicare Shared Savings Program
b. Medicaid Shared Savings Program
c. Commercial Shared Savings Program
4. Resource permitting, support systems development for ongoing ACO quality reporting for all programs.
5. For the period of July 1, 2016 through June 30, 2017
a. Hire and maintain appropriate staffing for project, including (but not limited to) a CHAC Director and Project Coordinator (or Project Manager).
b. Execute and monitor activities, including a quality compliance program, to ensure compliance with CHAC’s Medicare, Medicaid, and Commercial Shared Savings Program and regulatory Agreements and requirements.
c. Recruit providers who will participate and collaborate with CHAC.
d. Complete reporting for CHAC’s Medicare, Medicaid, and Commercial Shared Savings Program Agreements according to schedule.
e. Plan, staff and support CHAC’s Board of Directors Meetings.
f. Plan, staff and support CHAC’s Clinical, Financial, Consumer Advisory Panel, and Operations Committees in collaboration with the respective Chairs.
g. Maintain CHAC’s website to meet compliance requirements, and provide general information for beneficiaries and the public.
h. Represent CHAC at State meetings.
i. Actively collaborate with OneCare Vermont, the VT Blueprint for Health, and other stakeholders in the development and implementation of the Vermont Care Organization.

XxXxxx to provide at least three (3) customer engagement and speaking activities each year during the consulting term pursuant to the parameters outlined by the Company in writing (each an “Engagement Assignment”), plus Xx. XxXxxx to provide such additional sales and marketing support as may be requested by the Company and upon mutual agreement, including, but not limited to, phone conversations with customers or other industry participants, speaking engagements, and social media marketing. For purposes hereof, an Engagement Assignment shall include any of (a) a half day strategic session with management on M&A and the partnership landscape for the Company; (b) any field marketing event speaking engagement, and (c) a one-on-one meeting with a C-level executive of a Tier 1-target. In addition, Night Dragon to make Xx. Xxxxxxxx or other Night Dragon associates available to provide business development, competitive analysis and other in-depth market analysis services to the Company to supplement the in-house Business Development team at Forescout, pursuant to the parameters outlined by the Company in writing. Such analysis (“Business Development Analysis”) to be provided once per year (2019 & 2020) in the form of a written report and requires full complete downstream analysis of entities that Forescout should consider partnering with or acquiring.
