Site Certification. 21.1 Contractor shall comply with requirements for state certification as a Medi-Cal organizational provider, authorized to provide specialty mental health services on behalf of the County, in accordance with CCR, title 9, §1810.435, prior to the date on which the provider begins to deliver services under the Contract, and once every three years after that date. The on-site review conducted by the County, as part of the state certification and recertification process, shall be made of any site used to deliver covered services to beneficiaries, except that on-site review is not required for public schools, hospital outpatient departments, primary care and psychological clinics, or satellite sites.
21.2 Contractor may begin delivering covered services to beneficiaries at a site subject to on-site review prior to the date of the on-site review, provided the site is operational and has any required fire clearances. The earliest date the provider may begin delivering covered services at a site subject to on-site review is the latest of these three (3) dates:
21.2.1 The date the provider’s request for certification is received by the County in accordance with the County’s certification procedures;
21.2.2 The date the site was operational; or,
21.2.3 The date a required fire clearance was obtained. The County shall complete any required on-site review of a provider’s site within six months of the date the provider begins delivering services to beneficiaries at the site.
21.3 Contractor may continue delivering covered services to beneficiaries at a site subject to on-site review as part of the recertification process prior to the date of the on-site review, provided the site is operational and has any required fire clearances. The County shall complete any required on-site review of a provider’s site within six months of the date the recertification of the provider is due.
21.4 County will verify through an on-site review that Contractor is in compliance with all applicable requirements in the current DHCS Xxxxx-Xxxxx/Medi-Cal Provider Certification and Recertification Protocol and any other County requirements as necessary for site certification/recertification. If the County identifies deficiencies or areas for improvement, the County will issue the Contractor a plan of correction to resolve any outstanding requirements prior to completing certification.

Site Certification. The State of Washington hereby authorizes TUUSSO Energy, LLC, any and all parent companies, and any and all assignees or successors approved by the Council to construct and/or operate the Typha Solar Project as described herein, subject to the terms and conditions set forth in Council Order No. , Council Order Recommending Site Certification (Attachment 2 to this Agreement), and this Agreement. The construction and operation authorized in this Agreement shall be located within the areas designated herein and in the modifications to the Revised Application for Site Certification submitted by TUUSSO on January 26, 2018 (Revised Application). This Agreement authorizes the Certificate Holder to construct the Typha Solar Project such that Substantial Completion is achieved no later than ten (10) years from the date that all final state and federal permits necessary to construct and operate the Project are obtained and associated appeals have been exhausted. If the Certificate Holder does not begin construction of the Project within five (5) years of the execution of the SCA, the Certificate Holder will report to the Council their intention to continue and will certify that the representations in the Revised Application, environmental conditions, pertinent technology, and regulatory conditions have remained current and applicable, or identify any changes and propose appropriate revisions to the Agreement to address changes. Construction may begin only upon prior Council authorization and approval of such certifications. If the Certificate Holder does not begin construction of the Project within ten (10) years of the execution of the SCA as well as upon final conclusion of any and all appeals of all permits and approvals, all rights under this SCA will cease. The Certificate Holder may begin Commercial Operation of the Typha Solar Project prior to completing construction of all of the Project components, provided that all necessary Project elements are in place for safe operation of the Project and its operation will not adversely affect any obligations under this Agreement.
Site Certification. The State of Washington hereby authorizes OS and any and all parent companies, and any and all assignees or successors approved by the Council, to construct and/or operate the Ostrea Solar Project as described herein, subject to the terms and conditions set forth in Council Report to the Governor Recommending Site Certification (Attachment 1 to this Agreement), and this Agreement. The construction and operation authorized in this Agreement shall be located within the areas designated herein and in the Application for Site Certification (ASC) submitted by CCR on April 7, 2022. This Agreement authorizes the Certificate Holder to construct the Ostrea Solar Project such that Substantial Completion is achieved no later than ten (10) years from the effective date of the SCA. If the Certificate Holder does not begin construction of the Project within five (5) years of the effective date of the SCA, then at least ninety days prior to the end of the five year period, the Certificate Holder must report to the Council its intention to continue and will certify that the representations in the SCA, environmental conditions, pertinent technology, and regulatory conditions have remained current and applicable, or identify any changes and propose appropriate revisions to the Agreement to address changes. Construction may begin only upon prior Council authorization and approval of such certifications. If the Certificate Holder does not begin construction of the Project within ten (10) years of the effective date of the SCA all rights under this SCA will cease.
Site Certification.
A. Site Description The Columbia Solar Project comprises five solar sites to be constructed and operated on distinct locations in unincorporated Kittitas County. Certification details applicable to all five sites are discussed in the body of this Site Certification Agreement, while site-specific details relevant only to the Fumaria Solar Site are presented in Attachment 1.
Site Certification. The State of Washington hereby authorizes Innergex and any and all parent companies, and any and all assignees or successors approved by the Council, to construct and/or operate the Wautoma Solar Energy Project as described herein, subject to the terms and conditions set forth in the Council Report to the Governor Recommending Site Certification (Attachment 1 to this Agreement), and this Agreement. The construction and operation authorized in this Agreement shall be located within the areas designated herein and in the Application for Site Certification (ASC) submitted by Innergex on June 9, 2022, as amended on August 23, 2024, and October 9, 2024. This Agreement authorizes the Certificate Holder to construct the Wautoma Solar Energy Project within the terms provided in WAC 000-00-000, which subject to expiration as provided in WAC 000- 00-000. If the Certificate Holder does not begin construction of the Project within five (5) years of the effective date of the SCA and thereupon continue in a reasonably uninterrupted fashion toward project completion, then in accordance with WAC 000-00-000, at least ninety days prior to the end of the five year period, the Certificate Holder must report to the Council its intention to continue and will certify that the representations in the SCA, environmental conditions, pertinent technology, and regulatory conditions have remained current and applicable, or identify any changes and propose appropriate revisions to the Agreement to address changes. Construction may begin only upon prior Council authorization and approval of such certifications. If the Certificate Holder does not begin construction of the Project within ten (10) years of the effective date of the SCA all rights under this SCA will cease.
Site Certification. 1. The nuclear electric generating facility authorized to be sited by this Certification Agreement as presently defined is to include the following elements, hereinafter called the “PROJECT”; a boiling water reactor with a rate output of approximately 3,323 megawatts (thermal), a turbine-generator, a mechanical draft evaporative cooling tower system, a control and re-cycle facility, pump houses, transmission lines, associated service lines and other associated facilities required for the generation and transmission of electric power which are reasonably necessary and economically practicable for achieving electric generation capacity of approximately 1100 megawatts.
2. This Certification Agreement certifies, to the extent authorized by state law, that within and on the above site the Supply System may construct and operate the Project subject to the terms and conditions of this Certification Agreement.
Site Certification. A. This agreement certifies that, to the extent authorized by state law and those warranties and conditions set forth herein, the State approves and authorizes the construction and operation of an underground storage facility for natural gas and related or supporting facilities at the Mist Site, in the manner described in NWN’s site certificate application, this agreement, and the record of the administrative hearings held pursuant to ORS 469.300 through ORS 469.570, including supporting testimony filed by ONG or NWN with EFSC. This approval by the State binds the State and all counties, cities and political subdivisions in the State as to the approval of the site and the construction and operation of the underground storage reservoir and related or supporting facilities, subject only to the conditions of this agreement. However, each agency and county that issues a permit, license or certificate shall continue to exercise enforcement authority over such permit, license or certificate.
B. This certificate requires NWN to comply with applicable state laws or EFSC rules as they exist on the date it is executed by EFSC, and with stricter state laws or EFSC rules adopted subsequent thereto if compliance with such stricter state laws or EFSC rules is necessary to avoid a clear danger to the public health and safety.
C. The Site Certificate has been amended 12 times, as follows:
1. Amendment 1, approved October 24, 1987, amended the site map and amended certain conditions regarding monitoring for safety and vibration.
2. Amendment 2, approved August 2, 1988, amended the site map to allow the addition of a monitoring well.
3. Amendment 3, approved September 21, 1990, amended the site map to replace two poorly functioning injection/withdrawal xxxxx and add two new xxxxx to increase capacity during the “heating season.”
4. Amendment 4, approved July 21, 1997, enlarged the site boundary and authorized NWN to develop related and supporting surface facilities associated with new underground storage reservoirs in the Xxxxxx Creek Storage Area, and upgrade related and supporting surface facilities at NWN’s Xxxxxx Station. The amendment also authorized NWN to develop and operate new pipelines connecting the storage facilities at Xxxxxx Creek to Xxxxxx Station. It authorized the replacement of two reciprocating compressors with one turbine driven compressor with rated horse power of 5,035 BHP at Xxxxxx Station, subject to an operating limitation to 6,650 total horsepower....
Site Certification. The State of Washington hereby authorizes Chehalis Power to construct and operate the Chehalis Generation Facility (CGF) at the Site subject to the terms and conditions of the Council's Final Orders, Findings of Fact and Conclusions of Law, commitments made during the adjudicative hearings, and this Amended Site Certification Agreement. Such construction and operation shall be located within the areas designated for construction that are indicated in the Application and subsequent amendments, and described in Attachment 1. This Site Certification Agreement authorizes construction of either or both units of the CGF to begin within ten (10) years from the date of signing of this Agreement. Construction may begin separately or simultaneously for each unit within that 10-year period. Construction is deemed to begin upon the start of construction of a unit's major components, excluding site preparation, upon a schedule and with the intention of completing construction within twenty-seven months after commencement. If construction of either unit's major components has not commenced within ten (10) years of the signing of this Agreement, rights under this Agreement to construct and operate the combustion turbine unit that has not commenced construction shall cease. Six months before commencement of construction, Chehalis Power (a) during the first five years after execution of this Site Certification Agreement shall identify to the Council any substantial relevant change or certify the lack of substantial change in relevant environmental conditions, regulatory environment, or economically available technology, and (b) during the second five years shall certify that the representations of the Application, environmental conditions, pertinent technology, and regulatory conditions remain current, or identify any changes and propose appropriate resulting changes in the Site Certification Agreement to deal with changes. Construction may begin only upon prior Council authorization, upon the Council's finding that no changes to the Site Certification Agreement are necessary or appropriate, or upon the effect of any necessary or appropriate changes.

The State of Washington hereby authorizes TUUSSO Energy, LLC, any and all parent companies, and any and all assignees or successors approved by the Council to construct and/or operate the Penstemon Solar Site as described herein, subject to the terms and conditions set forth in the August 22, 2018, Report to the Governor, Recommending Site Certification (Attachment 2 to this Agreement), and this Agreement. The construction and operation authorized in this Agreement shall be located within the areas designated herein and in the modifications to the Revised Application for Site Certification submitted by TUUSSO on January 26, 2018 (Revised Application). This Agreement authorizes the Certificate Holder to construct the Penstemon Solar Site such that Substantial Completion is achieved no later than ten (10) years from the date that all final state and federal permits necessary to construct and operate the Site are obtained and associated appeals have been exhausted. If the Certificate Holder does not begin construction of the Site within five (5) years of the execution of the SCA, the Certificate Holder will report to the Council their intention to continue and will certify that the representations in the Revised Application, environmental conditions, pertinent technology, and regulatory conditions have remained current and applicable, or identify any changes and propose appropriate revisions to the Agreement to address changes. Construction may begin only upon prior Council authorization and approval of such certifications. If the Certificate Holder does not begin construction of the Site within ten (10) years of the execution of the SCA as well as upon final conclusion of any and all appeals of all permits and approvals, all rights under this SCA will cease. The Certificate Holder may begin Commercial Operation of the Penstemon Solar Site prior to completing construction of all of the Site components, provided that all necessary Site elements are in place for safe operation of the Site and its operation will not adversely affect any obligations under this Agreement.
Site Certification a. Contractor will comply with all site certification requirements. Contractor shall maintain all applicable certifications through San Mateo County to provide any of the following reimbursable services: Xxxxx-Xxxxx Medi-Cal, MediCal, Medicare, or Drug MediCal.
b. Contractor is required to inform BHRS Quality Management, in advance, of the following major changes:
i. Major leadership or staffing changes.
ii. Major organizational and/or corporate structure changes (example: conversion to non-profit status).
iii. Any changes in the types of services being provided at that location; day treatment or medication support services when medications are administered or dispensed from the provider site.
iv. Significant changes in the physical plant of the provider site (some physical plant changes could require a new fire or zoning clearance).
v. Change of ownership or location.
vi. Complaints regarding the provider.
