DATA TO BE SUPPLIED TO UNION. With each remittance required under Article 21.05, the Company will supply the Union with a list containing the following information: - Employees by classification, status and rate of pay; - Employees on lay-off or Leave of Absence; - Newly hired employees; and - Employees who have resigned.

DATA TO BE SUPPLIED TO UNION. CAMI will supply the Union with the following information electronically on a monthly basis: (a) Seniority lists (b) Team members by rate (c) Team members transferred into or out of the bargaining unit (d) Team members on leave of absence for more than one week and the reason specified for the leave (e) Team members on layoff for more than one week and team members recalled from layoff of more than one week (f) Team members who have lost seniority, including those discharged (g) The names and addresses, including postal codes, and telephone numbers of all team members covered by the Collective Agreement. (It will be the responsibility of the team member to keep XXXX advised of their current address and postal code at all times. The information so provided will be regarded as the team member’s official address for all purposes under this Agreement.) (h) A list of those team members when dues deductions are made, and a list of those team members when dues deductions are not made and the reason therefore (i) Team members on occupational leaves of absence, including team member number, department, date of injury, return to work date, time lost and claim number (j) Reports indicating usage of income security will be provided to the Union
DATA TO BE SUPPLIED TO UNION. 7.01 The Company will supply to the Union Committee the following information at the end of every month and send a copy to the Local Union office: (a) Employees who are in the bargaining unit regardless of whether or not they paid dues in the month; (b) Employee’s number and their hourly rate and classification; (c) Employee’s transferred into or out of the bargaining unit; (d) Employee’s status (i.e. at work, on vacation, weekly sick benefits, LTD, WCB, retired in the month, any other leave of absence) and the date of occurrence; (e) Layoffs and recalls; (f) Employees who have lost seniority; (g) Names, addresses, and postal codes of all retired and active employees; (h) The Employer will notify the Union immediately with the name and department of any new Managers and/or Supervisors.
DATA TO BE SUPPLIED TO UNION. 6.01 The Company will supply to the Union committee, the following information every quarter and send a copy to the local Union office: (1) Employees who acquire seniority; (2) Employees by rate and classification; (3) Employees transferred into or out of the Bargaining Unit; (4) Employees on leave of absence; (5) Employees on Sickness and Accident and Compensation and the date of occurrence; (6) Layoffs and recalls; (7) Employees who have lost seniority; (8) Employees who are discharged; (9) Names, addresses and telephone numbers of all current bargaining unit employees including those on lay-off with recall rights. The Company will also provide a list of names of supervisors. 6.02 In addition to the above, the Company will provide the Union with a copy of all job postings and will include a list of names of the applicants, the name of the successful applicant and the start date.

DATA TO BE SUPPLIED TO UNION. 5.01 The Company will supply to the Union committee the following information quarterly or as defined below and send a copy to the Local Union office: (a) employees who acquire seniority (within a reasonable amount of time); (b) employees by rate and classification; (c) employees transferred into or out of the bargaining unit; (immediate notification) (d) employees on all types of leave of absence (upon request); (e) employees on Sickness and Accident and Compensation and the date of occurrence; (3 days from company notification) (f) lay-offs and recalls; (g) employees who have lost seniority; (h) employees who are discharged; (i) names, addresses, and postal codes of all active employees; (j) list of salaried employees, supervisors, names and shifts; (k) All job postings, applicants and job awards; and (l) The Company will provide a copy of all health and safety incidents and/or accidents to the Health and Safety Co-Chair and Union Chairperson.
DATA TO BE SUPPLIED TO UNION. 7.01 The Employer will supply to the Union Chairperson and the Local 1090 Financial Secretary, at the end of each month, the following information in excel format, by e- mail: 1. Employees who are in the bargaining unit regardless of whether or not they paid dues in the month. 2. Employee's clock number, hourly rate, classification and whether the employee is part-time or full-time. 3. Employees transferred into or out of the bargaining unit. 4. The number of hours worked in the month. 5. Employees status (i.e. at work, on vacation, WSIB, any other leave of absence) and the date of occurrence. 6. Layoffs and recalls. 7. Employees who have lost seniority. 8. Names, addresses, phone numbers, e-mail addresses, and postal codes of all active employees. 9. A list provided to the Union Chairperson of all currant and new hires, on a quarterly basis. It is understood that each individual Employee must provide written authorization to the Employer before their e-mail address will be provided to the Union. Should a member choose not to provide authorization, the Employer will not supply their e-mail address to the Union.
DATA TO BE SUPPLIED TO UNION. 7.01 The Employer will supply to the Union committee the following information at the end of every month and send a copy to the local Union office: (a) The Employer will supply to the Union Chairperson the following information, if any, on a weekly basis with a copy to the Local Union Office. (i) Employees on leave of absence of one (1) week or more. (ii) Layoffs and recalls. (iii) Employees who have lost seniority and reason thereof. (iv) Employees who are absent due to sickness, accident or compensable injury as well as the last day worked. (v) Employees who have been hospitalized for three (3) or more consecutive days. (vi) Employees on bereavement leave. (b) The Employer will supply to the Union Chairperson the following information, if any, on a monthly basis with a copy to the Local Union Office. (i) Employees who acquire seniority including wage rate and classification. (ii) Employees transferred into or out of the Bargaining Unit. (iii) The names, addresses, phone numbers of all current employees. (c) The Employer will supply to the Union Local Office the “Social Insurance Number” of each employee who has successfully completed the probationary period on a one-time basis.
