DUTIES OF THE RESIDENT. 5.1 The Resident shall upon admission and for the duration of this Agreement comply with the obligations set out in this clause 5. The Resident shall: a. present all medications to any person appointed by the Proprietor upon admission to the Nursing Home; b. take all reasonable steps to ensure that visitors and relatives do not bring into the Nursing Home medication or food for consumption or use by the Resident without the prior consent of the Proprietor; c. ensure that his/her clothing and belongings are clearly marked and/or labelled with the Resident’s name; d. comply with all reasonable requests and suggestions made by the Proprietor; e. ensure the punctual payment within 21 days of a demand by the Proprietor of all fees or sums due or owing to the Proprietor under this Agreement; f. if it appears that the Resident’s funds necessary to pay the fees due under this Agreement will become depleted within a period of twelve weeks, immediately advise the Proprietor in writing; g. if State Support is withdrawn for whatever reason in respect of the Resident (retrospectively or otherwise), immediately inform the Proprietor in writing and indemnify and keep indemnified the Proprietor against all costs, losses or liabilities arising from such withdrawal or cessation of State Support; h. nominate a next-of-kin and a financial next-of-kin prior to arrival at the Nursing Home (as per Schedule 2 attached), which person shall be contacted by the Proprietor where considered necessary under this Agreement and as appropriate. Any changes to such nominee during the term of this Agreement shall be notified by the Resident to the Proprietor in writing; i. observe all rules laid down by the Proprietor for the orderly operation of the Nursing Home including any policies and arrangements with regard to smoking, use of electronic cigarettes and alcohol consumption by the Resident in the Nursing Home; j. comply with all Safety Regulations including fire drills and safety inspections; k. notify the Proprietor promptly of the name of the Resident’s appointed medical practitioner and pharmacist; l. ensure that the details required for the Admission Form, set out in Schedule 2 to this Agreement, are duly completed and up to date prior to or on admission; and m. as long as the Resident is in receipt of the Long-Term Residential Care Services, the Resident may use the accommodation facility allocated to the Resident at the Nursing Home (or such alternative accommodation as may be des...

DUTIES OF THE RESIDENT a. The Resident agrees to work full-time at CMMC and at such other offices or facilities in the Central Maine Healthcare (“CMHC”) service areas as are related to the operation of the CMMC. The Resident agrees to devote his/her best efforts and attention to the diligent performance of his/her duties herein. The Resident agrees to act in accordance with the current CMHC Citizenship Standards. The Resident shall comply with CMHC’s handbook, rules, regulations, policies and any/all applicable state and federal statutes, rules, or regulations, relating to teaching, which may be revised from time to time. The Resident shall work under the administrative supervision of the Director of the Family Medicine Residency Program (the “Program Director”) or designee. b. The Resident’s work schedule shall be set by the Program Director or designee and will be communicated to the Resident reasonably in advance. c. The Resident’s duties will also include but not be limited to: (1) Develop a personal program of self study and professional growth with guidance from the teaching staff; (2) Participate in safe, effective and compassionate patient care under supervision, commensurate with his/her level of advancement and responsibility; (3) Demonstrate an ability to assume graded responsibility; (4) Participate fully in the educational activities of this program and, as required, assume responsibility commensurate with his/her level of advancement for teaching and supervising other residents and students; (5) Participate in CMMC programs and activities involving the medical staff; (6) Participate in CMMC committees and councils, especially those that relate to patient care review activities; (7) Apply cost containment measures in the provision of patient care; and (8) read, understand and comply with the requirements of (a) the certification by the American Board of Family Medicine, the ACGME accreditation requirements, (b) the Program’s Resident Manual and (c) CMMC’s Medical Staff By-laws.
DUTIES OF THE RESIDENT. 6.1. By moving into the Place of Residence, the Resident shall make sure that it is clean and order- ly, that the University’s individual usage inventory and equipment which is granted for the Resident for the period of residence is undamaged, is functioning and is in a condition for safe operation. In case of any observed compliance failure the Resident shall notify the University (Dormitory) ad- ministration. All of the complaints on the cleanliness of the Place of Residence, damage or malfunc- tioning will not be accepted later than 24 hours after accommodation procedures. 6.2. During the period covered by the Agreement the Resident shall use the Residence only for its intended purposes and take care of it as well as of the University equipment, inventory and any oth- er property in the residential and common areas, shall act in compliance with fire-safety regulations and other legal acts and rules related to both the exploitation of the Residence and the common are- as in such a way that the condition of the place of Residence would be of the same condition as it was before assigning it to the Resident as much as possible; 6.3. The Resident shall pay the charge for the accommodation in the Residence in time and in ac- cordance with the conditions defined in the Agreement; 6.4. The Resident shall obey the legal requirements of the University (Dormitory) executives; 6.5. The Resident shall personally familiarise with and present the Rules to his/her guests and take responsibility for breaking the rules either personally or by his/her guests in the Place of Residence and common areas; 6.6. In case the Resident breaches the obligations indicated in this Agreement or neglects the duties, responsibilities and prohibitions provided in Rules, he/she shall be responsible for his/her actions in accordance with the Agreement regulations and, on demand of the University, shall pay the penalty which equals forfeit and the amount of which is defined by the Xxxxxx’x order. The Resident shall compensate the damage experienced by the University through the Resident’s or his/her guests’ fault, non-compliance or inappropriate compliance with the duties indicated in this Agreement or Rules. The Resident shall also compensate the damage to the third party if it occurred because of the Resident’s or his/her guest’s fault or negligence. 6.7. DUTIES OF THE RESIDENT. During the term of this Agreement, the Resident shall perform the duties described below. Resident acknowledges that failure to fulfill any or all of these duties shall be considered a material breach of this Agreement and may lead to corrective action, including but not limited to probation, suspension, or non-renewal of this Agreement and termination. A. Fulfill the educational and clinical requirements of the graduate medical education and graduate clinical training programs as specified by the Accreditation Council for Graduate Medical Education and the UIC College of Medicine including mandatory attendance at New Resident Orientation. B. Use his/her best effort, judgment, and diligence in a professional manner in performing all duties, tasks, and responsibilities assigned to the Resident for the duration of the program. C. Provide safe, effective, and compassionate patient care whenever assigned or assumed. D. Participate in the educational activities of the training program and, as appropriate; teach and supervise other Residents and students. E. Participate in institutional committees and councils to which the Resident is appointed or invited. F. Maintain a valid State of Illinois Medical license throughout the training period. Notify the Office of Graduate Medical Education and Program Director in writing immediately if his/her medical license is revoked or otherwise restricted or if his/her application for a temporary license is denied. Any such revocation or denial shall serve automatically to terminate this Agreement. G. Obtain, if requested by the University during the term of this Agreement, any medical examination, including physical, psychiatric, and/or laboratory testing, in accordance to policy. H. Complete and keep current any and all medical records, progress notes, charts, reports or other necessary documentation in a timely manner. Complete a discharge summary for each patient assigned to the Resident according to the documentation timeline of the hospital or clinical site in which the Resident is assigned. Failure to complete any medical record including such discharge summaries as required may result in progressive discipline. I. The Resident is obliged to: 4.1.1. carry out exclusively the types of activities specified in a business plan based on the types of activities according to the List; 4.1.2. comply with the terms of this Agreement; 4.1.3. quarterly, until the 15th of the month following the reporting quarter, to provide information about its activities in accordance with the form approved by the Directorate, including copies of statistical and tax reports, through the Directorate's web portal; 4.1.4. monthly, no later than the 20th day of the month following the reporting period, to transfer to the deposit account on demand of the Directorate deductions in the amount of one percent of total income; 4.1.5. annually, until July 1, following the reportingyear, to conduct a mandatory audit of financial and economic activities and compliance audit in accordance with the form approved by the Directorate and send one copy of the audit report and audit opinion to the Directorate via the web portal. When conducting an audit of a special issue, issues of compliance with the actual activities of a legal entity, the types of activities listed in the List, as well as the types of activities carried out during the financial year and their share in total income should be considered. The audit report indicates the compliance of the income received and the actual activities of the legal entity with the types of activities specifi ed in the List; 4.1.6. annually, until January 15, following the reporting year, to provide the Directorate with information about the ongoing training courses in information technology, if any, as well as to coordinate the training program within these courses; 4.1.7. carry out new types of activities not specified in the business plan submitted by a resident of the IT Park, after approval by the Directorate of a new (additional) business plan; 4.1.8. within ten working days from the date of reorganization in the form of transformation, including changes in the organizational and legal form, state registration of changes and (or) additions made to the constituent documents of the resident, change of the name of the resident's organization, as well as its legal and/or actual add ress, bank details, notify the Directorate with the submission of copies supporting documents by sending a written notification by registered mail or other available means of communication that allows you to record the delivery of the notification to the addressee, to make changes to t...

DUTIES OF THE RESIDENT. The Resident undertakes to: 7.1. use the Room only for its intended purpose, to maintain it, to take care of the security of the University property therein, to strictly comply with fire safety requirements and other legal requirements and regulations related to the maintenance of the Room or premises containing the Room. 7.2. use the Room and the shared premises and facilities in a manner that does not violate the rights and legitimate interests of other residents living in the Room or the Student House. 7.3. maintain the Room and any equipment, furniture and other items therein during the term of this Agreement. Properly maintain the elements of the Student House’s shared premises and facilities (doors, windows, floors, walls, ceilings, etc.) in such a manner that the Room and the shared premises and facilities are in a state as close as possible to that upon the day of signature of the Agreement. 7.4. immediately inform the Student House Manager of any accident or incident that may have occurred and of the measures taken to minimize the damage to the Room and its inventory. The damage committed and other consequences of the incident shall be communicated to the Student House Manager in writing, and such consequences shall be removed only with the consent of the Student House Manager, unless immediate action to eliminate the damage is required to prevent further damage to the Room, shared premises and the residents of the Student House. 7.5. fulfill the legal requirements of the Student House Manager and/or other responsible employees. 7.6. make good use of the shared premises and facilities: kitchen equipment, hallways, staircase and other premises, as well as water supply, sewers, electricity and other equipment. 7.7. enable the Student House Manager and/or other responsible staff to inspect the condition of the Room occupied. 7.8. observe personal hygiene, sanitary norms, personal safety and other requirements. 7.9. be responsible for the behavior of their guests in the Student House. 7.10. it is forbidden for a Resident to sublet the Room or other property entrusted to him, to accommodate other residents or to allow other persons to use it. 7.11. pay the monthly fee for accommodation in the Student House under the terms and conditions of the Agreement. 7.12. compensate the University for all losses incurred by the University as a result of the Resident’s failure or improper fulfillment of their obligations under this Agreement.
