Performance and ReportingThe period of performance shall be from _____________________ to _____________________ (hereinafter referred to as "Period of Performance"). As used herein, "Project" shall mean the research described in Appendix A hereof, under the direction of (hereinafter referred to as "Principal Investigator"). Penn State shall commence the performance of Project on the first day of Period of Performance and shall use reasonable best efforts to perform Project substantially in accordance with the terms and conditions of this Agreement. The Principal Investigator shall furnish Sponsor with reports regarding Project.
Performance and ReportingProgress Measurement 41. While all Australian governments are committed fully to achieving genuine improvement for Indigenous children and families, a number of challenges to measuring progress are recognised. These include the relatively small size of the Indigenous population meaning that identifying trends may require aggregating several years of data. It is also recognised that the benefits of this program may not begin to be realised until after a period of years, towards the end of the agreement as programs are bedded down. These and other relevant factors will be considered when measuring progress. 42. Following the agreement of baseline data by the Commonwealth, States and Territories, all jurisdictions will use the following performance indicators for the duration of the Agreement to identify and measure progress towards the agreed outcomes: a) increased proportion of Indigenous children attending the Children and Family Centres who have had all age‐appropriate health checks and vaccinations; b) increased proportion of Indigenous three and four year olds participating in quality early childhood education and development and child care services; c) increased proportion of Indigenous children attending the Children and Family Centres who go on to attend school regularly; d) increased proportion of Indigenous children and families accessing a range of services offered at or through Children and Family Centres, including but not limited to childcare, early learning, child and maternal health, and parent and family support services; e) increased proportion of pregnant Indigenous women with an antenatal contact in the first trimester of pregnancy in each year; f) increased proportion of Indigenous teenagers accessing sexual and reproductive health programs and services; g) reduced proportion of Indigenous babies born with low birth weight each year; h) reduced mortality rate of Indigenous infants each year; i) reduced proportion of Indigenous women who use substances (tobacco, alcohol, illicit drugs) during pregnancy each year; and j) reduced proportion of hospital admissions of Indigenous children 0‐4 years 43. Review of the overall operation of the Agreement will be based on progress against the following agreed performance indicators: a) performance measures identified in clause 42 of this Agreement; b) progressive achievement of the outputs, as specified in bilaterally agreed Implementation Plans; c) satisfactory compliance by States and Territories...
Performance and ReportingKey Performance Indicators (KPIs) will be utilised to monitor the performance of the frameworks on a quarterly basis. The Contractor will be required to report against these KPIs in line with the Management Information requirements.
Performance and Reporting. 2.2.5.1. The Company will monitor the Service Providers performance on a regular basis. 2.2.5.2. The Service Provider shall provide the Company with monthly reports detailing, but not limited to, the following information: • Number of call outs; • Response times to call outs; • Number of defect service delivery or installed parts; • Vehicle downtime; • Vehicle availability; • Service activities carried out; • Preventative maintenance activities carried; • Corrective maintenance activities carried out; • Cost of services and maintenance activities carried out; and • Report on the condition of the vehicle and provide a schedule and alert of the preventative maintenance activities to be carried out. 2.2.5.3. The Service Provider is expected to work with the Company to improve performance and/or process capability where needed. 2.2.5.4. In the cases of repeated poor performance or failure to improve, the Agreement shall be terminated.
Performance and Reporting. 4.1. In addition to providing any other reports and other deliverables that may be required hereunder, MTS will provide monthly reports measuring the Service Levels as identified in Attachment D.
Performance and Reporting. The most critical aspect of these Performance Level Standards and Reports is the monitoring and measuring of Vendor’s service level performance. All key services provided to Covered Persons must be carefully measured and monitored by Vendor and the results analyzed and reported to United as set forth in Attachment 1 to this Exhibit. All benchmarks, targets and metrics, as well as reporting timeframes, are described in Attachment 1. Service performance levels must be reviewed monthly by both United and Vendor. Any problems that might arise must be addressed in accordance with the terms set forth in the Performance and Reporting Specifications Attachment to this Exhibit (Attachment 1). Performance and Reporting. A. The RECIPIENT shall direct all notices, reports, insurance policies, and other communications related to or required by this Agreement to the office of the City of Iowa City, Community Development Office, 000 X. Xxxxxxxxxx Street, Iowa City, Iowa 52240. Notice by both RECIPIENT and City shall be given by ordinary mail. B. Until the completion of the Project and expenditure of all HOME Investment Partnership funds disbursed under this Agreement, the RECIPIENT shall submit quarterly reports describing progress of the project activities. This report will be due 10 (ten) days after the end of each quarter, based upon the City's fiscal year (July 1 to June 30). C. Not later than date , the RECIPIENT shall provide the City with a Certified Statement of the Expenditure of Funds disbursed under this Agreement. D. RECIPIENTs that receive HOME Investment Partnership funds for tenant based rental assistance and rental housing shall maintain records of determination of each tenant income eligibility and eligibility as a family at the time the household(s) receive the assistance. The RECIPIENT shall reexamine family income, size and composition at least annually unless waived by mutual consent by the U.S. Department of Housing and Urban Development, the City of Iowa City and the RECIPIENT. E. The RECIPIENT shall submit annual reports (July 1 – June 30) by the first day of August of each contract year through date . The annual report shall, at a minimum, include statistics relating to the number of households being assisted with HOME Investment Partnership funds, household size, racial characteristics, single head of household by gender, household income and a narrative of project highlights. F. For projects with a HOME grant or loan of $300,000 or more (including all funding sources), an audit report which discloses the expenditure of HOME Investment Partnership funds allocated for this Project, shall be submitted by date G. No reporting requirements for City HOME funds shall extend beyond the final annual report that is due on date .
Performance and ReportingReporting arrangements 15. New South Wales will submit bi-annual performance reports during the operation of the Agreement. Each performance report is to contain a description of actual performance in the period to date against the agreed Project Plan. These reports are due on the 10th business day of May and November each year during the operation of this Agreement. All bi-annual reports must be certified by a New South Wales’ senior official as being a true and accurate representation of the progress of the project for the relevant reporting period. 16. A Final Project Performance Report is required on completion of the project. The Final Project Performance Report will be a report that can be used for public information and dissemination purposes and will evaluate the Project from New South Wales perspective and include a description of the conduct, benefits and outcomes of the Project.
Performance and Reporting. Work to lease/sell property according to established goals for marketing activity and sales or lease up based on each assigned Project. Prepare and submit quarterly reports, or as requested, reflecting performance related to the established goals for each assigned Project. Prior to the commencement of any work under this Agreement, Broker shall furnish copies of all required endorsements and completed Certificate(s) of Insurance to City’s TCI/Contract Services Department, which clearly shall be labeled “insert name of project/contract Project” in the Description of Operations block of the Certificate. The Certificate(s) shall be completed and signed by an Agent, accompanied by an affidavit also signed by Xxxxxx, attesting the furnished Certificate(s) represent Broker’s current coverages. City shall not accept a Memorandum of Insurance or Binder as proof of insurance. The certificate(s) must be signed by the Authorized Representative of the carrier and list the agent’s signature and phone number. The certificate(s) shall be mailed, with copies of all applicable endorsements, directly from the insurer’s authorized representative to City. City shall have no duty to pay or perform under this Agreement until such certificate and endorsements have been received and approved by City’s TCI Department. No officer or employee, other than City’s Risk Manager, shall have authority to waive this requirement. City reserves the right to review the insurance requirements of this Exhibit B during the effective period of this Agreement and any extension or renewal hereof and to request the modification of insurance coverage and limits when deemed necessary and prudent by City’s Risk Manager, based upon changes in statutory law, court decisions or circumstances surrounding this Agreement. In no instance shall City allow modification whereby City may incur increased risk. Broker’s financial integrity is of interest to City; therefore, subject to Broker’s obligation to maintain reasonable deductibles in such amounts as are approved by Broker’s insurance companies, Broker shall obtain and maintain in full force and effect for the duration of this Agreement and any extension hereof at Broker’s sole expense, insurance coverage written on an occurrence basis, unless otherwise indicated, by companies authorized to do business in the State of Texas and with an A.M Best’s rating of no less than A- (VII), in the following types and for an amount not less than the amount listed below. ...
Performance and Reporting. Reporting Time FramesThe recipient shall submit Monthly or Quarterly performance achievements and performance questionnaires to the Department, within fifteen (15) days after the end of the reporting period. In addition, if the award period is extended beyond the "original" project period, additional performance achievements and performance questionnaires shall be submitted. Failure to Submit – Performance achievements and performance questionnaires that are not complete, accurate, and timely may result in sanctions, as specified in Section IV, Award Management and Reporting Requirements. Report ContentsPerformance reporting must clearly articulate, where appropriate, performance during the execution of the award has met a standard against which the recipient’s performance can be measured. Responses must also reflect on accomplishments for the period and identify problems with project implementation and address actions being taken to resolve the problems. Additional information may be required if necessary to comply with federal reporting requirements. Requirement for Data on Performance and Effectiveness Under the Award - The recipient must collect and maintain data that measures the performance and effectiveness of work under this award. The data must be provided to OCJG in the manner (including within the timeframes) specified by OCJG. Data collection supports compliance with the Government Performance and Results Act (GPRA) and the GPRA Modernization Act of 2010, and other applicable laws. Financial Consequences for Failure to Perform - In accordance with s. 215.971 F.S., payments for state and federal
