Employee Personnel File. A copy of any formal discipline report to be entered on an employee's file will be given to the employee. The employee will be required to sign Management's copy. Such signature will indicate receipt of formal reprimand only.
Employee Personnel File. A. The Board shall maintain in the District a personnel file on each employee. An employee shall have the right to inspect this file during regular business hours, provided such inspection shall not interfere with the employee’s regularly assigned duties and shall comply with the guidelines specified in Board Policy. Such inspection shall take place only in the presence of an administrator or administrative designee. Individual copies of any non-confidential material shall be given the employee on request.
B. Without an employee’s permission, no portion(s) of an employee’s file shall be disclosed to third parties except as required by law or as shall be necessary in the performance of the Board’s or Administration’s duties.
C. An employee shall be given a copy of any document placed in his/her personnel file that is disciplinary in nature or may otherwise have a negative impact on the continuing employment of the teacher. The employee’s receipt of a copy of such documents shall be acknowledged by the signature of the employee.
D. Employees shall have the right to attach written responses to documents placed in their personnel file. Such written responses must be filed within fifteen (15) working days of the date the document is placed in the personnel file.
E. All personnel files are kept in the District Office and include the following types of information: • Job application • Official transcripts of all course work completed • Health/physical forms • References • Dates of employment • Contracts • Valid certificate(s) for services performed • Supervisory evaluations • Promotions • Disciplinary actions • Awards received • Letters of resignation or retirement • Discharge Any other information the Administration deems to be relevant to the employee’s job and continued employment in the District Additional information required to be kept for school employees shall also be maintained by the District (e.g., TRS records, payroll information, leaves of absence data).
Employee Personnel File. 7.2.1 The official District personnel file of each employee shall be maintained at the District's central administrative office.
7.2.2 An employee may inspect material in his/her personnel file which may serve as a basis for affecting the status of his/her employment except materials which:
1) were obtained prior to his/her employment,
2) were prepared by identifiable examination committee members, or
3) were obtained in connection with a promotional examination
7.2.3 An employee may inspect such materials in his/her personnel file, with the exception of the items specified in Section 7.2.2, during the normal business hours of the District Office at times other than when the employee is required to render service. Such inspection shall take place under the supervision of a District administrator or designee.
7.2.4 No materials of a derogatory nature, except the items specified in Section 7.2.2, may be placed in an employee's personnel file without allowing the employee an opportunity to review and comment thereon. An employee shall have the right to enter, and have attached to any such derogatory statement his/her own comments thereon. The review and comment upon materials of a derogatory nature shall take place during the normal business hours of the District Office and at a time when the employee can be spared from duty, as determined by the supervisor. The employee shall submit a request in advance to the supervisor to leave the normal place of work during assigned duty times for such review and comment.
7.2.5 All material placed in an employee's personnel file shall be dated and signed by the contributor.
Employee Personnel File. A. The district shall maintain one (1) official, confidential personnel file for each support employee. The employee shall have the right to inspect his/her file during regular business hours of the Human Resources Office upon request by scheduling in advance to do so. A representative of the employee may examine the employee's file with written authorization by the employee.
B. No disciplinary document shall be placed in the employee's official Human Resources personnel file without first providing the employee a copy. The employee shall sign the document indicating his/her knowledge that the document is being placed in the employee's file.
C. The employee shall have the right to attach a written response to any document placed in his/her file. Such response must be provided within ten (10) days from the date the document is placed in the employee's file.
D. Any suspension, demotion, dismissal, or non-reemployment action taken against an employee for disciplinary reasons must be based on documentation contained in the employee's personnel file.
E. If the employee has not received additional disciplinary documentation after seven years, the employee may request, in writing, that the disciplinary documentation in the personnel file be reviewed by the Superintendent for possible exclusion. Documentation regarding employee involvement in safety incidents will be maintained permanently in an employee’s personnel file. Employee Personnel File. All employee personnel records shall be maintained under the following circumstances:
1. A copy of all personnel records relating to any employee shall be kept in an official personnel file which shall be located in the College personnel office.
2. All personnel records shall be in writing. Written material relative to an employee's conduct or service shall be placed in the personnel file after that employee has had an opportunity to read such material. The employee shall acknowledge that he/she has read such material by affixing his/her signature to the actual copy to be filed, but it shall be understood that such signature merely signifies that he/she has read the material in question. Such signature indicates neither agreement nor disagreement with its content. If the employee refuses to sign the copy to be filed, then the administrator shall with a third party as witness state in the file the above refusal.
3. The employee shall have the right to answer in writing any material filed in his/her personnel file, and his/her answer shall be attached to the file copy.
4. Upon request by an employee, he/she shall be given access to the nonconfidential documents in his/her file without delay. Confidential documents are those materials used to evaluate a person's qualifications prior to employment to which the individual waived the right to access (placement credentials, letters of reference, etc.). At his/her written request, each employee shall be furnished, without cost, a copy of any material in his/her file. No items may be removed from the employee's file except for brief inspection or copying.
5. No material in the employee's personnel file will be duplicated without a subpoena, court order, or written authorization from the employee and appropriate vice president.
Employee Personnel File. An employee's Personnel Record File shall be made available and open to the employee for inspection at a reasonable time established by mutual agreement between the employee and his/her immediate supervisor.
Employee Personnel File. In order to facilitate the investigation of a grievance, or to prepare for a pending arbitration, an employee shall, on reasonable advance notice, be entitled to review his/her personnel file, in the presence of a University representative and in the office in which the file is normally kept. The employee may be accompanied by a Union Xxxxxxx or other authorized representative of the Union. On request, the employee will be provided with a photocopy of any item contained within the file pertaining to the issue at hand. The employee shall not remove, deface or alter any materials contained in the file.
Employee Personnel File. Upon written or verbal request of an employee, the School District shall permit the employee to review his or her own personnel file within seven (7) working days after receipt of the request. The employee shall be permitted to attach a statement to any evaluation or discipline with which the employee disagrees. The School District shall provide a copy of documents within the personnel file upon an employee’s request and without charge to the employee.

Written notices of disciplinary action(s) in the employee’s personnel file after six (6) months cannot be used thereafter as part of disciplinary procedure provided such employee has not received any additional disciplinary action for the same, or a similar, infraction during those six (6) months. Subject to giving the Employer advance notice, employees shall have access to their personnel file as soon as practicable within seven (7) calendar days of a request or as soon thereafter as is practicable.
Employee Personnel File. (a) An employee shall, upon twenty four (24) hours notice, be entitled to review their personnel file, in the presence of a University representative and in the office in which the file is normally kept. The employee may be accompanied by a Union Xxxxxxx or other authorized representative of the Union. On request, the employee will be provided with a photocopy of any item contained within the file. The employee shall not remove, deface or alter any materials contained in the file;
(b) Any disagreements about the accuracy of information contained in the employee's personnel file may be subject to challenge through the grievance procedure in this Agreement.
