HEALTH & LIFE INSURANCE. The State shall pay the basic group life insurance premiums for those employees who are members of this bargaining unit. This provision shall not diminish the right of employees to carry additional insurance on themselves or their dependents under pre- sent statutes. The State shall provide health plan coverage for employees pursuant to Title 5 §285. The State shall pay sixty percent (60%) of the cost of dependent premium for each eligible employee who selects dependent coverage. Part time employees hired into permanent full-time positions will be allowed to apply for health insurance within sixty (60) days of the permanent appointment with no evidence of insurability. The State agrees to provide laid off employees with group health insurance at the employee’s expense for one (1) year provided that the employee is unemployed. Premiums are to be paid directly to the insurance carrier. Failure to make payments would result in cancellation of insurance with no conversion privileges.

HEALTH & LIFE INSURANCE. A. Members shall be entitled to participate in the City's health insurance program as recommended by the Health Care Committee and described in the documents on file in the Finance Department. (1) The City agrees to maintain a City Health Care Committee for the purpose of regularly reviewing employee health care needs, and implementing a health care program for its employees. The Committee shall act in accordance with the Final Report of the Health Care Task Force Report dated July 10, 2006, and further amendments by the Committee. If the Committee is not maintained by the City, the City will provide the health care benefits in place at the time of the dissolution of the Committee for the remainder of the Agreement. (2) The Union agrees to participate in the City Health Care Committee and to adhere to the recommendations of the Committee regarding all aspects of health issues, including, but not limited to, the selection of carrier, determination of coverage and determination of co-payments, deductibles, and employee contributions. The City agrees to adhere to recommendations of the Committee as such recommendations apply to the members. The Union shall have one (1) designee that serves on the Committee and may exercise voting rights on behalf of the Union. Any member appointed as the Union’s designee shall be paid for attending the Committee meetings. Any overtime incurred by a member while attending meetings of the committee shall be paid at one and one-half times their regular rate of pay. (3) All coverage shall be subject to the insurance company’s requirements and eligibility. (4) The City shall not be required to contribute any premium amount toward health insurance for any employee who is absent or on leave without pay for ten
HEALTH & LIFE INSURANCE. ‌ 1. The County and the Association agree that, effective the first of the month following ninety (90) days of employment, the County will pay the entire employee-only premium cost of group health, dental, vision, and 25k term life insurance for each individual employee covered by the provisions of this Agreement. The medical benefit levels will be equivalent to those delineated in and provided by the plans in effect on April 1, 2003. 2. The County shall pay the following amounts toward the cost of dependent health insurance purchased by the County through the County’s group insurance plan by current employee: For coverage of a spouse only $143.75/month For coverage of a child or children only $108.75/month For coverage for a full family $152.50/month 3. The parties acknowledge that the insurance companies may not offer one or both existing plans at the time of renewal. In the event that the current plan becomes unavailable at renewal time, the parties agree to meet to select a replacement plan. The replacement plan shall provide as close to current benefits as is practical and available. 4. The County and the Association agree that this Article shall be subject to re- opener, upon call of either party, if the premium rate, at any strata, suffers a premium increase of twenty percent (20%) in any single insurance year. 5. Because of the extensive travel often times associated with access to a doctor’s care, employees may utilize up to three (3) days (twenty-four (24) hours for eight (8)-hour shift personnel and thirty (30) hours for ten (10)-hour shift personnel) per insurance year of sick leave for attendance at a specialist, vision, or dental doctor’s office/laboratory outside of the community where the employee lives, when such service is not available in the community within a reasonable period of time, without such days being considered “use” for purposes of Article 16, paragraph 6 herein. 6. The County will purchase and maintain a $50,000 term life insurance policy for each employee in the bargaining unit with at least two (2) years County service.

(C) The City agrees to leave this “as is” but subject to changes with new Health Care Reform laws.

HEALTH & LIFE INSURANCE. Section 34.1. The Employer agrees to provide single or family medical insurance coverage, as appropriate, and life insurance coverage to all bargaining unit employees. The Employer recognizes the need for adequate insurance coverage and the IAFF/Local #2550 and employees recognize the ever increasing cost of such coverage. Therefore the parties agree to work cooperatively in seeking an insurance provider which can provide adequate coverage at a reasonable cost. Section 34.2. The Employer shall contribute up to the following amounts each month toward the premium cost for each bargaining unit employee’s medical, dental and prescription drug insurance coverage. Single coverage — $390.00 Family coverage — $975.00 Any increase or decrease in insurance premiums outside the limits specified above shall be shared fifty percent (50%) by the Employer and fifty percent (50%) by the employee. Section 34.3. Employees shall pay all co-payments and/or deductibles as provided in the group health insurance plan. Section 34.4. The Employer agrees to establish a Group Insurance Committee with representatives from each department of the Township to review and make recommendations to the Trustees regarding various types of group health insurance coverage and plans, alternate services and cost containment programs. The Union shall have one (1) voting representative on the Group Insurance Committee. Section 34.5. Nothing herein shall be construed as preventing the Employer from offering bargaining unit employees, on a voluntary individual basis, alternative health insurance plan(s) as determined by the Board of Trustees, in addition to the one that is currently in effect. Section 34.6. The Employer shall pay the full cost for a $15,000 group term life insurance policy covering all bargaining unit employees.

HEALTH & LIFE INSURANCE. The Town agrees to provide the same medical and life insurance plans as provided to other eligible employees of the Town. The Town shall pay 75% of the premiums for Preferred Provider Organization (PPO) plans and 80% of the premiums for Health Maintenance Organization (HMO) plans in effect under the Town health insurance plans for all members of the bargaining unit. The employee premium deduction will normally be made on a biweekly basis. The Association may appoint one of its members to serve as a voting member of the Employees Insurance Advisory Committee. It is agreed that notice of any change in the insurance program to the Insurance Advisory Committee is notice to the Union. It is agreed that the members of this bargaining unit will receive any and all benefits agreed upon between the Town and the Employees Insurance Advisory Committee at whatever time they are agreed to become effective. The Town agrees to provide the same Basic Term Life Insurance policy as is provided to other eligible employees of the Town and shall pay 75% of the premiums for such policy.
