HEALTH & LIFE INSURANCE. The State shall pay the basic group life insurance premiums for those employees who are members of this bargaining unit. This provision shall not diminish the right of employees to carry additional insurance on themselves or their dependents under pre- sent statutes. The State shall provide health plan coverage for employees pursuant to Title 5 §285. The State shall pay sixty percent (60%) of the cost of dependent premium for each eligible employee who selects dependent coverage. Part time employees hired into permanent full-time positions will be allowed to apply for health insurance within sixty (60) days of the permanent appointment with no evidence of insurability. The State agrees to provide laid off employees with group health insurance at the employee’s expense for one (1) year provided that the employee is unemployed. Premiums are to be paid directly to the insurance carrier. Failure to make payments would result in cancellation of insurance with no conversion privileges.
HEALTH & LIFE INSURANCE. A. Members shall be entitled to participate in the City's health insurance program as recommended by the Health Care Committee and described in the documents on file in the Finance Department.

HEALTH & LIFE INSURANCE. The Town agrees to provide the same medical and life insurance plans as provided to other eligible employees of the Town. The Town shall pay 75% of the premiums for Preferred Provider Organization (PPO) plans and 80% of the premiums for Health Maintenance Organization (HMO) plans in effect under the Town health insurance plans for all members of the bargaining unit. The employee premium deduction will normally be made on a biweekly basis. The Association may appoint one of its members to serve as a voting member of the Employees Insurance Advisory Committee. It is agreed that notice of any change in the insurance program to the Insurance Advisory Committee is notice to the Union. It is agreed that the members of this bargaining unit will receive any and all benefits agreed upon between the Town and the Employees Insurance Advisory Committee at whatever time they are agreed to become effective. The Town agrees to provide the same Basic Term Life Insurance policy as is provided to other eligible employees of the Town and shall pay 75% of the premiums for such policy.
HEALTH & LIFE INSURANCE. Section 34.1. The Employer agrees to provide single or family medical insurance coverage, as appropriate, and life insurance coverage to all bargaining unit employees. The Employer recognizes the need for adequate insurance coverage and the IAFF/Local #2550 and employees recognize the ever increasing cost of such coverage. Therefore the parties agree to work cooperatively in seeking an insurance provider which can provide adequate coverage at a reasonable cost.
HEALTH & LIFE INSURANCE. During Leave of Absence Teachers shall be granted the opportunity to continue their hospitalization-medical insurance and other current insurances to the extent the insurance companies will allow, while on authorized leaves of absence, for a period of twelve (12) months, provided that they make arrangements to pay the required premiums to the District in advance of the due dates for group premium payment. If the employee is removed from the insurance group for failure to remit payments as required and agreed to, the Board shall not be responsible nor liable for expenses incurred by the employee because of the termination of the employee's insurance coverage. If the employee allows the insurance to lapse, the employee may not re-enroll until such time as reinstated from the authorized leave of absence.
HEALTH & LIFE INSURANCE. The County agrees to make a Health and Welfare monthly premium payment for each employee for medical, dental, and vision benefits (G/W/L, D-5, V-4 subject to availability from OTET). The parties agree that any contribution required by an employee will be deducted from the employee’s paycheck. Effective July 1, 2017, the County’s contribution towards medical, dental, and vision benefits shall not exceed an average monthly composite rate of one thousand two hundred dollars ($1,200.00) per month per eligible employee until June 30, 2018. Effective July 1, 2018, the County’s contribution towards medical, dental, and vision benefits shall not exceed an average monthly composite rate of one thousand three hundred dollars ($1,300.00) per month per eligible employee until June 30, 2019. Effective July 1, 2019, the County’s contribution towards medical, dental, and vision benefits shall not exceed an average monthly composite rate of one thousand four hundred dollars ($1,400.00) per month per eligible employee until June 30, 2020.

HEALTH & LIFE INSURANCE. A. For Plan Year (Calendar Year) 2018, the City will contribute $880.00 per month toward health care premiums for the Employee + One dependent or Family coverage. For calendar year 2018, the City shall contribute $605.00 per month toward health care premiums for employees with Employee Only coverage. For Plan Year (Calendar Year) 2019, the City will contribute $955.00 per month toward health care premiums for Employee + One or Family coverage and $680.00 for Employee Only coverage. For Plan Year (Calendar Year) 2020, the City will contribute $1,030.00 per month toward health care premiums for Employee + One or Family coverage and $755.00 for Employee Only coverage. The amount of the City’s contribution is based on the selection of the “medical” enrollment category. If an employee elects “employee only” medical coverage, then the “employee only” allowance is provided to the employee. Any contribution not utilized by the employee shall revert to the City.
