Hospital and Surgical Insurance. (a) All full time employees shall be eligible to participate in the Blue Cross/Blue Shield Hospital Surgical Insurance Plan, or its equivalent from another carrier, at the Employer's expense. The Employer shall pay for the cost of coverage for the employee's spouse and minor dependents provided, however, that if the employee and the employee's spouse are both eligible to participate as employees in group health plans funded by or through the County, the employee and the employee's spouse shall elect coverage under only one such plan. Coverage of the employee, the employee's spouse, and/or the employee's dependents under two or more County health care plans shall not be permitted unless it is to the financial advantage of the County to permit such. If the employee and the employee's spouse fail to make an effective election within two (2) weeks after being requested to do so, the Employer shall have the right to elect the plan for the employee or employees. The Blue Cross/Blue Shield Plan shall be MVF-l basic coverage with master medical option 1 (including Prescription Drug Program with $5 co-pay), XX Xxxxx, a D45NMRider, OMB,and VST. Applications shall be secured from the Personnel Oftice, signed, and returned indicating whether or not the insurance is desired. Coverage shall become effective with the first (1st) billing cycle following completion of thirty (30) calendar days of continuous employment with the employer. The costs of Family Continuation and Sponsored Dependent riders elected after January 1, 1994, shall be paid by the employee with a payroll deduction through the Flexible Spending Account. Such riders existing before January 1, 1994, may continue in effect at the Employer's expense. (b) All full time employees who elect at their own discretion not to participate in the hospital and surgical insurance program as set forth in this Section shall be eligible to receive an additional forty dollars ($40) per pay period if he or she is not already covered by the insurance of a relative whose premiums are paid by County or Court funds, and provided that the employee supplies proof of a reasonable level of health care coverage from another source. (c) For the duration of this Agreement, employees will be provided with vision care coverage at County expense as is currently available. Hospital and Surgical Insurance. 1. Effective January 1, 2018, the group health, hospitalization and prescription drug coverage plans/providers available to eligible employees (and future retirees) under this agreement may be changed by the employer if the employer ascertains the City can achieve a cost savings as a result. The Union shall consent to the change, provided the employer guarantees that the level of benefits available to employees under the replacement plan/providers shall be equivalent or greater than the benefits enjoyed by the employees prior to the change, and there shall be no diminution in benefits as a result of the change. Providers as referred to in this section shall only apply to the names of companies/groups providing insurance coverage and shall not exclude any medical professionals (Doctors, Hospitals or other individuals providing medical care). i. Prior to any change, the Employer will provide the Union with all the necessary plan documents for its review as far in advance as possible, including plan/drug formulary disruptions, plan comparison and evidence of coverage reports, but no later than THIRTY (30) days prior to the anticipated change. In the event of a dispute as to the equivalency of the level of benefits under the new plan, the Union shall have the right to file a class action grievance under the parties’ contractual grievance arbitration provisions. No change in the health insurance plans/providers shall be enacted unless and until the proposed plan is deemed to be equivalent or better by an Arbitrator, as per this section. ii. Also, any individual that has gone through drug trials under a previous group health, hospitalization and prescription drug coverage plan does not have to endure the same trial period under a new group health, hospitalization and prescription drug coverage plan. WorkCover) accident resulting in death or permanent total disablement. The full and precise conditions of this cover will be in accordance with the terms of the policy, but in general will provide that, in the event of a workplace accident occurring which results in either the death or total permanent disablement of a worker covered by this Agreement, a lump sum payment as specified below will made. The defined payments are: With dependants $250,000 Without dependants $150,000 This benefit has been agreed to by the company on the grounds that premium costs have been set at $7 per week/worker and will not exceed that amount. In the event of insurance costs rising, it is agreed that the table of defined benefits will be reduced so as to maintain the $7 premium figure. The benefit levels of this program shall provide one hundred percent (100%) coverage for preventive care and eighty percent (80%) coverage for general service care. The State agrees to provide payroll deduction for dental insurance, provided such arrangements are agreed to by the insurance carrier. Dependent coverage will be available provided there is sufficient employee participation in the dental insurance program. Dependent coverage will be at the employees' expense.

  • Industrial Insurance Coverage The Contractor shall comply with the provisions of Title 51 RCW, Industrial Insurance. If the Contractor fails to provide industrial insurance coverage or fails to pay premiums or penalties on behalf of its employees, as may be required by law, Agency may collect from the Contractor the full amount payable to the Industrial Insurance accident fund. The Agency may deduct the amount owed by the Contractor to the accident fund from the amount payable to the Contractor by the Agency under this contract, and transmit the deducted amount to the Department of Labor and Industries, (L&I) Division of Insurance Services. This provision does not waive any of L&I’s rights to collect from the Contractor.

  • Travel Insurance The Employer shall provide and pay the full cost for travel insurance to cover all members of the bargaining unit for all modes of travel, in the amount of $200,000.00. The travel insurance policy shall also cover employees while on union business.
