Hospitalization-Medical Insurance a. The City shall comply with P.A. 152 as set forth by City Council resolution and the City Health Care Policy on file in the Personnel Department. Employees shall pay anything above the annual maximum amounts contributed by the employer that are established annually by the Act (2022 employer contributions $7,304.51* for single coverage, $15,276.01* for two-person coverage and $19,921.45* for family (3 or more) coverage). In the event P.A. 152 is repealed, the cost sharing in place at the time the Act is repealed shall remain in effect. *Subject to change based on yearly employer contributions as outlined in P.A. 152.

Hospitalization-Medical Insurance. The Employer contribution will be equal to the contribution provided for the certified employees. Each employee enrolled in this program shall contribute through payroll deduction, an excess of the monthly premium over the maximum Employer contribution toward the type of coverage for which such employee is enrolled. The Union agrees that it will not support any grievances relating to alleged inequities filed by an employee without dependents who claims discrimination in relation to an employee dependent coverage, nor will the Union encourage any lawsuits in relation to the same. An employee who chooses not to participate in the District insurance plan shall forfeit any right to monetary gain in lieu of insurance premium.
Hospitalization-Medical Insurance. The school employer will pay toward the cost of major medical insurance for each full-time teacher employed under regular contract and enrolled in the school corporation’s group insurance plan. Hospitalization-Medical Insurance. The Borough shall provide full family protection to each full-time employee performing thirty-five (35) hours work a week on a yearly basis average at no cost to said employee. Said plan shall contain major provisions at least as beneficial as the current Borough of Hillsdale Select Plan, including a maximum $200 individual and a $300 family deductible, 100% coverage above deductible as described for using Network Providers, a maximum 70-30 co-pay provision for services outside the Plan, with a maximum co-payment expense of $600 per year. These benefits are described in greater detail in the Plan Booklet which will be provided to each participant. The Borough agrees to deposit $300 per year for each full-time employee (35 hrs. per week) and $150 for each part-time (regularly scheduled for at least 12 hours per week) employee in a Medical Flexible spending account annually. Employees may withdraw monies from this account as prescribed by IRS regulations. The Borough will continue to provide health benefits to a covered full- time employee who retires on an accidental disability pension. The Borough shall make available to any part-time employee the option to acquire the above hospitalization and medical insurance at the then applicable rate to be paid by the employee.

If two eligible employees who are legal spouses choose to be covered by one family or single plus one plan, the married employees will receive a monthly Employer contribution equal to two single Employer contributions to the enrollee’s premium. This election must be made during (1) the open enrollment period or (2) upon the occurrence of a qualifying event, as defined by the hospitalization-medical insurance carrier. If an employee selects a plan for which the monthly premium is less than the Employer contribution toward the premium, the Employer will deposit the excess contribution into the employee’s health reimbursement account. An employee enrolled in the Employer's hospitalization-medical insurance program will contribute through payroll deduction any excess of the monthly premium over the maximum Employer contribution toward the type of coverage for which the employee is enrolled. An employee receiving wage replacement benefits from the Employer's workers’ compensation carrier or the long-term disability carrier remains eligible for the Employer contribution towards a plan.
