Hospitalization-Medical Insurance a. The City shall comply with P.A. 152 as set forth by City Council resolution and the City Health Care Policy on file in the Personnel Department. Employees shall pay anything above the annual maximum amounts contributed by the employer that are established annually by the Act (2020 employer contributions $6,818.87 for single coverage, $14,260.37 for two-person coverage and $18,596.96 for family (3 or more) coverage). In the event P.A. 152 is repealed, the cost sharing in place at the time the Act is repealed shall remain in effect.

Hospitalization-Medical Insurance. The Employer contribution will be equal to the contribution provided for the certified employees. Each employee enrolled in this program shall contribute through payroll deduction, an excess of the monthly premium over the maximum Employer contribution toward the type of coverage for which such employee is enrolled. The Union agrees that it will not support any grievances relating to alleged inequities filed by an employee without dependents who claims discrimination in relation to an employee dependent coverage, nor will the Union encourage any lawsuits in relation to the same. An employee who chooses not to participate in the District insurance plan shall forfeit any right to monetary gain in lieu of insurance premium.
Hospitalization-Medical Insurance. During the 2018 - 2019 school year, the Board will pay $470.00 per bargaining unit member per month toward the cost of single coverage hospitalization-health insurance. During the 2019 - 2020 school year, the Board will pay $485.00 per bargaining unit member per month toward the cost of single coverage hospitalization-health insurance. Bargaining unit members who do not take district sponsored health insurance will receive $25.00 each month toward a Flexible Medical Benefit Account.

Hospitalization-Medical Insurance. The school employer will pay toward the cost of major medical insurance for each full-time teacher employed under regular contract and enrolled in the school corporation’s group insurance plan. Option IV, F.A.E., VST, R.C.R.P., hospitalization and medical insurance for each teacher and his or her eligible dependents for semi-private room benefits; special riders shall be paid by the employee. The Master Medical Coverage deductible will be one hundred dollars ($100.00) for single member coverage and two hundred dollars ($200.00) for full family coverage. The Board shall pay the cost of CVS Caremark prescription drug coverage. Co-pay for generic, or no generic available, drug prescriptions will be ten dollars ($10.00), and twenty dollars ($20.00) for brand name drug prescriptions.

Hospitalization-Medical Insurance. 1. The Board shall pay the maximum as defined by statute for hospitalization, medical insurance, and prescription drug coverage for each teacher and his or her eligible dependents annually as defined by the plan recommended by the Association and approved by administration. Special riders shall be paid by the employee.
Hospitalization-Medical Insurance. A. The Board agrees to provide and pay for individual hospital and medical coverage for each employee assigned regularly to work on a full-time basis who desires such insurance. The Board agrees, in addition, to pay the additional premium for the provision of full-family hospital and medical insurance coverage to eligible employees who elect same and who were hired to commence service prior to July 1, 2005. Employees hired to commence employment on or after July 1, 2005 shall be enrolled in the POS plan, and the Board’s premium expense will be limited to 100% of the POS plan for the employee and his or her eligible dependents. Upon acquisition of tenure, or in the case of a custodial/maintenance employee after completion by such employee of three (3) years of continuous employment in the District, the employee who is assigned to work regularly on a full-time basis may elect to transfer to the traditional plan and the Board agrees to provide and pay for such coverage in the event of such election. The Board maintains the right to change carriers or to self-insure, either by itself or as a member of a joint insurance fund, as long as the level of benefits remains equal to or better than the benefits provided to the MPEA as of December 31, 1996. Any changes to this level of benefits shall be negotiated with the MPEA regardless of the carrier.
Hospitalization-Medical Insurance. A. The City agrees to provide Blue Choice or equivalent coverage to full-time EMPLOYEES. In addition, the City will make available, through a carrier of its choice, other comparable plans as long as they are available and at no additional cost to the City. EMPLOYEES shall be responsible for 15% of the premium cost for health insurance.
