Personal Illness Leave a. An employee may request personal illness leave under the following conditions:
1. The employee has a serious health condition, as defined by the FMLA, but does not meet the eligibility requirements for FMLA.
2. An employee’s family member has a serious health condition as defined by the FMLA, but the employee does not meet the eligibility requirements for FMLA.
3. The employee does not have a serious health condition as defined by FMLA but has an illness or condition that requires him/her to be absent from work for more than 3 days.
4. In cases subject to Workers Compensation Law, personal illness leave may be used to supplement Workers’ Compensation so that the total amount paid an employee will equal, but not exceed his/her regular salary for the period of absence from duty.
b. An employee requesting personal illness leave shall submit a request in writing and shall submit a certification from a licensed health care provider to Human Resources, indicating that the leave is necessary. Failure to provide medical certification within 15 days will result in a denial of the leave unless an extension is requested and granted.
c. During a personal illness leave, an employee must exhaust all sick and personal business leave and accrued compensatory time, before moving to unpaid status. Once an employee moves to an unpaid leave, all benefits will be terminated at the end of that month.
d. Personal illness leave shall not exceed one year (12 months).
e. Prior to return from personal illness leave, the employee shall present a medical release from his/her physician, if applicable, that he/she is able to return to perform the essential functions required by the position. In addition, before the employee returns, the Board may, at its expense, require examination by health care providers of its choice.

iii. The employee does not have a serious health condition as defined by FMLA but has an illness or condition that requires him/her to be absent from work for more than five (5) days (e.g., minor surgery).
b. In cases subject to Workers’ Compensation Law, personal illness leave may be used to supplement Workers’ Compensation benefits so that the total amount paid to an employee will equal, but not exceed, his/her regular salary for the period of absence from duty.
c. An employee requesting personal illness leave shall submit a request in writing and shall submit a certification from a licensed healthcare provider to Human Resources, indicating that the leave is necessary. Failure to provide medical certification within 15 days will result in a denial of the leave unless an extension is requested and granted.
d. During a personal illness leave, an employee must exhaust all sick and personal business leave and accrued compensatory time before moving to unpaid status. Once an employee moves to an unpaid leave, all benefits will be terminated at the end of that month. However, if an employee has been employed less than 12 months with the district, the district will continue to pay its portion of the benefits for the first 12 weeks of the employee’s leave. Failure to return to work at the expiration of the leave will require repayment of the District’s portion of the benefits paid.
e. Personal illness leave shall not exceed one year (12 months.
f. Prior to return from personal illness leave, the employee shall present a medical release from his/her physician, if applicable, that he/she is able to return to perform the essential functions required by the position. In addition, before the employee returns, the Board may, at its expense, require examination by a healthcare provider of its choice.
Personal Illness Leave. For absences caused by illness or physical disability of the teacher, each teacher shall be allowed ten (10) days each year accumulative to two hundred and fifteen (215) days. If a new teacher has unused sick leave in the last corporation in which they taught, they may transfer a maximum of three (3) days per year to the East Xxxxxx School Corporation until their days have been fully transferred. Teachers shall be given an accounting of their accumulated sick leave on each paycheck stub. (As amended January 1, 1995.)
Personal Illness Leave a. A bargaining unit member whose personal illness or other disability extends beyond the period covered by accumulated sick leave and any additional sick leave granted by the Board and upon written request, will be granted a leave of absence for a period of not more than three (3) consecutive years. Leaves will be granted for a maximum of one (1) year at a time. If the bargaining unit member wishes to extend the leave beyond one (1) school year, it will be necessary to reapply. In order to be eligible for such leave, the bargaining unit member must provide written documentation (i.e., medical report) from the bargaining unit member’s physician.
b. A bargaining unit member who exhausts the sick leave days warranted under this Agreement and has obtained personal illness leave shall be entitled to have the Board pay the cost of the premiums for the fringe benefit insurance subject to the following:
(1) A bargaining unit member having five (5) years of continuous service with the Board shall be entitled to thirty (30) days' coverage.
(2) A bargaining unit member having ten (10) years of continuous service with the Board will have sixty (60) days' coverage.
(3) A bargaining unit member having fifteen (15) years or more of continuous service with the Board will have ninety (90) days' coverage.
c. If this leave has been designated as FMLA leave either upon request by the bargaining unit member or by designation by the Board, this leave shall run concurrent with FMLA leave.
Personal Illness Leave. A teacher shall be credited with ten (10) new sick leave days each year. The maximum number of sick leave days a teacher may accumulate is one hundred twenty (120), unless the teacher has voluntarily reduced the teacher’s maximum in accordance with paragraph F below.
Personal Illness Leave. An unpaid leave that can be used for an employee’s personal illness. The leave requires Board approval prior to use, and is available only after sick leave and medical leave have been exhausted. An employee can request personal illness leave for the remainder of the current contract year only. Employee health insurance will be maintained through the month following the beginning of the leave or longer if required under the Family and Medical Leave Act. An employee can continue health insurance following said time period at his or her personal cost during the leave. All other benefits will be suspended during the leave.
Personal Illness Leave a. To qualify for consideration for personal illness leave:
1) An employee must have completed ten (10) years of cumulative service in the Buckeye Local School District.
2) An employee must have no remaining accumulated sick leave and be under current contract with the Board of Education.
3) The request must be accompanied by a detailed doctor’s statement verifying the seriousness of the employee’s personal illness.
4) If the Board is not satisfied as to the seriousness of the illness, the Board may request a second opinion from an impartial doctor, to be determined by mutual agreement.
b. Upon satisfactory verification of the seriousness of the illness, the Board will grant a personal illness leave of not more than one (1) year for the purpose of the employee’s personal illness.
c. Upon the granting of leave as detailed above:
1) The employee must pay to the Treasurer of the Board of Education, $100.00 per month for a single plan and $225 per month for a family plan, thirty (30) days in advance of when the premium is due. Failure to make advance payment shall result in the teacher being withdrawn from the fringe benefit group plans and all Board responsibility will cease at that time of withdrawal.
(a) The employee will then be covered on the payroll records and shall qualify for all fringe benefits paid by the Board of Education for regular employees for a period of one (1) year only.
d. A teacher returning within the one (1) year period shall be granted a position for which he or she is certified and the appropriate contract status, upon presentation of the attending doctor’s release.
e. At the end of the one (1) year period, the employee who is unable to return to work may request a one (1) year regular unpaid leave of absence with no Board paid fringe benefits.
f. Nothing in this section denies the employee from exercising their legal rights pursuant to O.R.C. 3319.131.
g. The following situations are exempted from this personal illness leave policy:
1) Those certified employees who, at the time of the request for personal illness leave, are receiving disability benefits from S.T.R.
Personal Illness Leave. 25.1 Upon reporting for one full workday, each school year a teacher shall be entitled to be absent from work on account of illness, physical disability, surgery, or quarantine for a total of ten (10) days without loss of compensation. A teacher who teaches less than a full school year shall receive a prorated number of sick leave days.
25.2 If, in any one (1) school year, the teacher shall be absent for illness, physical disability, or surgery less than the number of days to which h/she is entitled by virtue of Section 26.1, the unused days shall be accumulated up to a maximum of ninety (90) days ( See Article 26 Attendance Stipend).
25.3 In the event a teacher shall have accumulated one (1) or more days of sick leave in another school corporation and shall thereupon become employed by the Baugo Community Schools, there shall be added for the second year, and each succeeding year of such employment up to three (3) days of sick leave until the number of accumulated days to which said teacher was entitled in the last place of employment shall be exhausted. This provision is subject to written verification from the most recent employer.
25.4 In the event that the employer has reason to believe that abuse of the personal illness leave by the teacher exists; said employer may require the teacher to verify such illness, disability, etc., affirming that the teacher is unable to fulfill his/her normal assigned duties because of such illness or disablement.
25.5 Teachers may donate up to two (2) sick leave days in a given school year. These days may be donated to any teacher who, in the case of extended illness, has exhausted his/her sick leave days and personal leave days. A request for such donations will be made through the president of the Association. With the consent of the teacher making the request, the Association will convey the request to teachers and will circulate the appropriate form among teachers. Completed forms will be submitted to the Superintendent by the president of the Association. A doctor’s certificate will confirm the illness. The maximum number of days that may be donated to a given teacher in a given school year is the number of days it would take to enable that teacher to be eligible for long term disability insurance up to a maximum of sixty (60) days in a given school year.
25.6 Summer School teachers (other than teachers of driver’s education) shall be eligible to use two (2) sick leave days credited but not used during the previous...
Personal Illness Leave. Each full-time teacher employed under regular or temporary contract shall be entitled to be absent from work on account of personal illness or quarantine for a total of thirteen (13) days the first year of employment and ten (10) days in each succeeding year without loss of compensation. If, in any one (1) school year, the teacher shall be absent for such illness or quarantine less than the prescribed number of days, the remaining days shall be accumulated to a total of 180 days. Teachers shall be given an accounting of their accumulated sick leave under this provision with each paycheck.
