Reporting and Monitoring Requirements a. The Subrecipient will comply with HUD’s standards for participation in a local Homeless Management Information System (“HMIS”) and the collection and reporting of client-level information. If Subrecipient is a Domestic Violence provider, Subrecipient must participate in a comparable database in terms of functionality, data collection and reporting capacity that can meet all reporting requirements that the local HMIS system is able to meet. b. The City will perform project monitoring for work quality, programmatic activities, fiscal documentation and site inspections. Notification of monitoring will be provided to the Subrecipient prior to visits. c. Evaluations and disclosure of findings will be provided to the Subrecipient in writing within 45 days of a monitoring visit. d. If the Subrecipient fails to meet the goals and objectives of the Project, the City will attempt to find a mutually agreeable timeline to assist the Subrecipient in meeting program requirements. e. Technical assistance will be provided by the City prior to any issuance of a finding and will be provided upon the request of the Subrecipient. f. Project monitoring will be used to assure compliance with ESG regulations and Project performance. If compliance is not met, the Project will not continue and the grant will be re-distributed. g. The Subrecipient agrees to provide client level data, project activity and any other reporting information as requested by the City.

Reporting and Monitoring Requirements. (a) Within 28 days of each and every anniversary of the commencement of the term the Licensee shall provide to Council, a statutory declaration from an authorised officer of the Licensee, stating the hours and days during which activities were undertaken at the premises during the year then just completed, together with the proposed or intended hours and days for the then current year. (b) The Licensee shall report annually to Council, within 28 days of a request from Council, on the following matters in accordance with Council’s reporting guidelines to enable Council to incorporate these details into its quarterly and annual reports: (i) the contribution made to the community by the Licensee as evidenced by the uses, activities, user and membership numbers and a survey of user and community views; (ii) the enhancement of the local environment by the activities; (iii) the financial performance of the Licensee detailing the profit and loss and balance sheet details with membership and subscription being separated from the actual takings of the Licensee; (iv) how the Licensee has managed its public risk responsibilities by detailing measures undertaken by the Licensee to lessen the risk factors for its use of the premises; (v) its achievements or otherwise in water and energy savings as well as in waste minimisation according to the Waste Management Plan; (vi) other required reporting responsibilities to be specified by Council from time to time. A failure to meet the deadline in relation to the obligations hereunder after two (2) written requests for the same shall lead to the automatic termination of this agreement without the requirement for Council to give any notices to terminate the licence. (c) The Licensee shall report annually to Council, within 28 days of a request from Council, on the following matters in accordance with Council’s reporting guidelines to enable Council to incorporate these details into its reports: (i) Number of patrons; (ii) Fees taken based on category and description; (iii) Details of any competitions or tournaments that have occurred during the month; (iv) other required reporting responsibilities to be specified by Council from time to time. The Recipient shall provide ASHC with the following reporting and monitoring information with respect to the Project: 8.1.1. meetings will be held quarterly between the Recipient and ASHC representatives or at any time at the request of ASHC to discuss progress updates and any issues regarding the Project; 8.1.2. financial year quarterly Construction Progress Reports, in the form prescribed in Schedule “D”, that include, without limitation, information respecting all activities relating to the Project’s scope, schedule, budget, risks, photographs, Initials: and such other information as requested by XXXX, to be submitted no later than the 15th day of the following month and can be combined with quarterly reports on the months described in clause 8.2 below; 8.1.3. financial year quarterly reports, including a financial report prepared in accordance with GAAP for all payments, and expenditures made with the Capital Grant and all revenue received with respect to the Project, in the form prescribed in Schedule “C” and certified by a senior financial officer of the Recipient stating that the information submitted is true and accurate; 8.1.4. financial year quarterly financial reports shall list the expenses against the Capital Grant including any portion of the Provincial Contribution paid to any third party and any accrued interest in the form prescribed in Schedule “C” from the Effective Date to the current date or the time the Project is completed; 8.1.5. all Development Permits, Building Permits, Occupancy Permits, and confirmation of Substantial Performance upon receipt. Following receipt of all Occupancy Permits and confirmation of Substantial Performance, the Recipient shall provide an Audited Financial Statement of Final Capital Costs in the form outlined in Schedule “E”, at the Recipient’s sole expense; 8.1.6. any other information as reasonably requested by XXXX in order to assess the Recipient’s compliance with this Agreement; and 8.1.7. upon the request of XXXX, arrange for ASHC’s representative to have access to the business premises of the Recipient and to the site(s) where the Project activities are being carried out to verify their progress. 8.2. Reporting and Monitoring Requirements. The Authorized Representative must electronically submit performance reports quarterly during the 2024/25 Period of Performance. Quarterly performance reports are due within forty-five (45) days after the completion of the quarter and within sixty (60) days of the last quarter. Performance reports will be used to monitor activities for compliance with work progress to ensure grant activities are performed according to the quality, quantity, objectives, timeframes and manner specified within the agreement. Performance reports must be a summary of the activities, outputs, outcomes, and achievements in these reporting categories with accompanying management analysis. Furthermore, these reports must justify the invoiced expenditures. Data integrity and the verification and validation of performance results are a GO-Biz priority and provide the validation of the quality and impact of the program. The RECIPIENT may be obligated to meet, at minimum, once a quarter with GO-Biz staff to share performance progress and discuss any barriers or opportunities including shared best practices. RECIPIENT will respond to requests for reporting revisions and clarifications within one-week (seven (7) calendar days) of the request being sent from GO-Biz. If no revision or approved extension is received within that time, this could result in delayed payment by GO- Biz for the reporting period until requested revisions have been resolved.
Reporting and Monitoring Requirements. A) Borrower shall submit financial and operational reports to Lender as Lender may reasonably request in addition to the following reporting requirements: 1) The Borrower shall submit or cause the submission of Progress and Financial Reports through the Intelligrants Management System to the Lender on a quarterly basis, starting with the period ending June 30, 2015, until all Grant and Loan Proceeds are expended and all activities funded by the Grant and Loan Proceeds are complete, and the National Objective has been met. B) The Lender shall conduct, and the Borrower shall permit, on-site inspections of the Project upon reasonable request during the rehabilitation activities and thereafter on an annual basis, or more frequently, as the Lender may reasonably request. C) The Borrower shall submit to the Agency a Final Program Report and an Interim Audit Report(s) and/or Final Audit Report covering the Period of Performance under this Agreement. Additionally, one copy of all reports shall be maintained with other program documents available for public review, and at least one copy must remain in the Borrower’s files. D) Certificate of Program Completion shall be issued to the Borrower when the Agency determines that all required work under this Agreement has been satisfactorily completed, including the execution of a Closeout Agreement if applicable and the submission of the Final Program Report, the Interim Audit Report(s), and/or the Final Audit Report. The Agency must determine that all program and financial compliance issues have been addressed and that the findings and/or concerns, if any, of monitoring reports, program reports, and audit reports have been resolved and cleared in writing.
