Health Welfare and Retirement Benefits. The Company shall provide group health, dental, hospitalization, life and disability insurance benefits to you and your eligible dependents that are in effect as of the Effective Date in accordance with the terms of such plans as may be modified from time to time. You shall also be eligible to participate in such other welfare and retirement benefit plans or programs that are offered to senior executives of the Company generally to the extent you are eligible under the general provisions thereof as in effect from time to time.

Health Welfare and Retirement Benefits. 26.1 Effective July 1, 2016, the “cap” on District contribution for medical, dental, and vision benefit plans shall be $620 per month for eligible full-time (active) employee and family coverage. The “cap” for eligible part-time employees will be as described in Section 26.3 of Article 26. 26.2 Qualifying employees for full District health and welfare benefit payments shall be employees who regularly work eight hours per day, ten or more months a year. Employees shall be enrolled in insurance programs on the first of the month following fulfillment of the eligibility requirement. 26.3 For employees who regularly work four or more hours per day, ten or more months per year, the District will contribute, for employee and family coverage, that portion of the monthly premium set forth in Section 26.1 as the number of hours regularly worked per day by the part-time employee bears to eight, providing the employee agrees in writing to have the remaining amount deducted from his/her salary. 26.4 Ten and eleven month employees who terminate their employment with the District at the end of the summer recess shall reimburse the District for the full cost of insurance premiums for the non-work period. 26.5 Any employee who is currently employed in the District at the time of retirement through XXXX, who retires under XXXX, and who has served with the District for a minimum of five years, may remain part of the group medical program, the group dental program, and the group vision program, if permitted by the carrier, in addition to benefit rights afforded under COBRA. The retired employee who elects to remain part of the group programs must prepay the cost of such programs to the District quarterly for such coverage at least thirty days prior to the beginning of the quarter. (Also see 26.6, 26.7 and/or 26.8.) 26.6 The District will pay up to $328 towards the combined cost of the health, dental and/or vision insurance for retireant and, if retireant so elects, spousal coverage (total for both persons), if permitted by carrier. Health Welfare and Retirement Benefits. Section 1. All employees covered by this agreement, will be eligible to all benefits provided in this Article. Newly hired employees are eligible for benefits at the start of the month following their hire date. Current employees can enroll during the applicable open enrollment period, unless a qualifying life event occurs that allows them to enroll at that moment.

Health Welfare and Retirement Benefits. You will be eligible to participate in the employee benefit programs applicable to our salaried employees generally, including the Company’s health and welfare plans, as well as the defined contribution plan. In addition, you will be eligible to participate in the Deferred Compensation Plan for Officers and Key Employees of Xxxxxxxxx Technology Corporation. Your annual vacation entitlement will be 5 weeks. Except as herein provided, or as may be hereafter approved by the Board or its Human Resources Committee, you will not be entitled to further compensation or benefits. · Executive Severance Plan: Your employment by the Company is “at will” and may be terminated by the Company or by you at any time. However, if your employment terminates due to a termination by the Company without “cause” or a resignation by you with “good reason” (each, as defined in the attached Plan document), you will be entitled to receive the severance benefits included in the Severance Pay Plan for Executives of Xxxxxxxxx Technology Corporation attached hereto as Exhibit D. · Change in Control Severance: You will be entitled to severance benefits in the event of a change in control, as described in the Change in Control Severance Plan attached hereto as Exhibit E. The Hospital will continue to include the 20% hometown discount as part of the healthcare plan for 2018, 2019 and 2020 calendar years. Section 2 Increases to Health Insurance Any increases during the term of this Agreement to the monthly health insurance premiums will be shared equally by the employee and Employer. Section 3 Labor/Management Benefit Committee VH and the Union recognize the importance of undertaking joint efforts to ensure that employees have access to cost effective, quality health care and other insurance coverage. Both the Employer and the Union share a mutual interest in researching best practices in cost containment features and the benefits that ensure quality but also address increasing costs. To address these issues, the parties will establish a Labor/Management Benefits Committee with representatives from the bargaining units represented by the Union. The Union will appoint up to a total of five (5) representatives from the bargaining units it represents at VH to include one (1) organizer to the committee. The Employer will appoint up to five (5) management representatives. The committee shall be advisory and shall meet at least quarterly and more often as mutually agreed. In guiding the committee’s work, utilization data and cost information, among other data, shall be reviewed.
