Responsibilities of XXXXX. Xxxxx agrees to provide Laboratory Director and Laboratory Technical Consultant services according to 42CFR493.1413 – Standard; Technical consultant responsibilities. a) Xxxxx confirms that she meets the qualification requirements in 42 CFR 493.1405 – Standard; Laboratory director qualifications. b) Xxxxx acknowledges she is responsible for the overall operation and administration of the laboratory. c) Xxxxx acknowledges she must be accessible to the lab to provide onsite, telephone or electronic consultation as needed. d) Xxxxx acknowledges her responsibilities as laboratory director include: 1. Ensure that testing systems developed and used for each of the tests performed in the laboratory provide quality laboratory services for all aspects of test performance, which includes the preanalytic, analytic, and postanalytic phases of testing. 2. Ensure that the physical plant and environmental conditions of the laboratory are appropriate for the testing performed and provide a safe environment in which employees are protected from physical, chemical, and biological hazards. 3. Ensure that the test methodologies selected have the capability of providing the quality of results required for patient care; verify procedures used are adequate to determine the accuracy, precision, and other pertinent performance characteristics of the method. Ensure laboratory personnel are performing the test methods as required for accurate and reliable results. 4. Ensure that the laboratory is enrolled in an HHS approved proficiency testing program for the testing performed, that the samples are tested as required and the results are returned within the timeframes established. Proficiency testing reports must be reviewed by the director to evaluate the lab's performance and identify any problems that require corrective action. A corrective action investigation will be initiated when any proficiency test result is found to be unacceptable or unsatisfactory. 5. Ensure that quality control and quality assurance programs are established and maintained to assure the quality of lab services and identify failures in quality as they occur. 6. Ensure the establishment and maintenance of acceptable levels of analytical performance for each test system. Responsibilities of XXXXX a. All contracts and agreements with students and/or their legal guardians for services rendered by Xxxxx in coaching Junior Squash shall be under the name of Xxxxx or a related DBA. Xxxxx assumes responsibility for all liabilities relating to or arising from (his) squash coaching activities. Xxxxx absolves the RSRA, its Board and its members from any liability arising from his squash coaching or related activities. x. Xxxxx shall obtain and maintain, at his expense, all required licenses, insurances and certifications to safely and effectively run a Junior Squash Program. x. Xxxxx must provide the RSRA signed agreements showing the financial commitment by parents/guardians of the students to the Program. These must include a summary of the services to be provided by Xxxxx and the payments to be made by the parents/ guardians to Xxxxx or his Assignee. Xxxxx shall provide relevant agreements to the RSRA commissions paid to any and all Assignees. x. Xxxxx will clearly indicate in all announcements that he or a dba- related-entity is solely responsible for the Program. x. Xxxxx will be responsible for providing all advertising and at least weekly, social media updates about the progress of the program, minimally on the RSRA web site and Facebook page. Photos and mention of Juniors on social media and in the press shall only be used with parental/guardian permission. x. Xxxxx will complete a daily report of activities and attendance, in a format similiar to that shown in Exhibit A. This will be delivered to RSRA on a monthly basis.
Responsibilities of XXXXX. Xxxxx shall provide such consulting services relating to the portfolio management of agency backed securities business of Hatteras Financial as Xxxxx and Hatteras Financial agree, at such times as reasonably requested by Hatteras Financial and agreed by Xxxxx. In no event shall Xxxxx be requires to provide more than 8 hours of such consulting services per week. Xxxxx shall not be required to travel in the provision of the consulting services unless mutually agreed between Hatteras Financial and Xxxxx provided, all expenses related to such travel as so mutually agreed shall be borne by Hatteras Financial.

To appoint and engage such dealers, distributors, and re-seller parties as it deems necessary and appropriate to maximize sales of the Commercial Systems; B. To lead all dealer, distributor, and re-seller training programs, including providing qualified operational and safety instructors for such purposes; provided, however, that UAS may also conduct training demonstrations to such dealers, distributors and resellers, and at such times, as the parties shall mutually agree.

XXXXX is responsible for supplying appropriate additional information required by AFFILIATED FACILITY prior to the arrival of the students. x. XXXXX will assign to AFFILIATED FACILITY only those students who have satisfactorily completed the prerequisite didactic portion of the curriculum. x. XXXXX will designate a faculty member to coordinate with a designee of AFFILIATED FACILITY the assignment to be assumed by the student participating in the course of instruction. x. XXXXX and AFFILIATED FACILITY will mutually respect policies and regulations governing students that are participating in the practicum at AFFILIATED FACILITY. f. The parties mutually concur with the Center for Disease Control statement that there is a risk for accidental exposure to blood or bodily fluids for students in health profession education programs; and mechanisms for risk assessment and initiation of prompt treatment situations of high risk exposures are necessary. Therefore, AFFILIATED FACILITY shall have in place all Accidental Exposure to Patient Blood or Bodily fluids Policy. If an accidental exposure to blood or bodily fluids occurs to a faculty member or a student, the faculty or student will be informed of the level of exposure and will be advised to seek medical attention which may include blood testing, and obtaining medication required for emergency treatment of high risk exposures, when appropriate. Testing of the patient/resident known to be the source of the exposure will be coordinated by AFFILIATED FACILITY, and AFFILIATED FACILITY shall cover the costs of such source testing. The faculty member or student is responsible for obtaining their own blood testing, medications, and follow-up care and is liable for their own expenses related to any accidental exposures. A report of any occurrence of an accidental exposure involving faculty or students shall be provided directly to the faculty member or student involved. It is the faculty or students' obligation to provide such report to the appropriate academic unit at DOANE. g. All students and faculty will have their own health insurance and be responsible for all their medical expenses. AFFILIATED FACILITY agrees to make available first aid for minor accidents or incidents occurring on the premises. Responsibilities of XXXXX. 3.2.1 Notwithstanding the fact that the operator of the Equipment is an employee or representative of the Owner, the operator shall operate the Equipment in accordance with instructions from the Hirer, and accordingly, the Hirer shall be liable for all responsibility and costs incurred as a result of the actions of the operator whilst following the Hirer’s instructions. 3.2.2 The Hirer shall provide amenities and first aid services to the Owner’s employees in compliance with all applicable health & safety legislation in operation in the State where the work is undertaken. 3.2.3 Should it be necessary for Equipment to be towed into or out of a worksite, then the Hirer shall be responsible for all damage and/or salvage costs involving the Equipment, and said costs shall be in addition to the Fee and will be either charged to the Hirer’s account plus a margin of ten percent (10%), or payable direct to the salvage company by the Hirer. 3.2.4 The Hirer shall declare the weight of the goods and the Owner will rely on such declared weight when arranging for handling. Therefore, the Hirer shall be responsible for all extra cost and risk incurred by the Owner and for any and all damage sustained by reliance on the declared weight, if the weight declared is found to be wrong. 3.2.5 The Hirer shall provide adequate security for any Equipment left on site overnight or during periods when the worksite is left unattended unless it has been otherwise agreed in writing that the Owner arrange such security on the Hirer’s behalf.

XXXXX will assist students in finding housing for [INSTITUTION] students in Residence Hall and can advise on other accommodation in rented apartments or private homes. Housing applications must be provided at least two months before the commencement of the semester. At the end of the academic year, or semester, XXXXX will ensure that students apply for a transcript in the proper way according to US federal law. XXXXX will inform the [INSTITUTION] in good time of any legal documentation that needs to be processed.
