Sick/Carer’s Leave. 15.1 This clause only applies to permanent Employees. 15.2 Subject to clause 15.3, a permanent Employee who is unable to attend or remain at his/her place of employment due to personal illness or personal incapacity, shall be entitled to pay at the ordinary hourly rate as follows: 15.2.1 Up to 10 days sick pay for each year of continuous employment, accruing on a pro-rata basis for each completed four week period of continuous employment. The Employee will be paid the amount the Employee would otherwise have been paid had the Employee worked during that period. 15.2.2 Unused sick leave entitlements shall be cumulative. 15.2.3 Permanent Employees are entitled to access a maximum of 10 days per year of their sick leave entitlement to provide care or support to a sick or injured immediate family or household member. 15.2.4 An immediate family member is either a spouse, child, parent, grandparent, grandchild or sibling of the Employee or a child, parent, grandparent, grandchild or sibling of a spouse of the Employee. 15.3 Permanent Employees are expected to comply with the following requirements when taking sick or carer’s leave: 15.3.1 The permanent Employee shall, where practicable, advise the Employer of his/her inability to attend for work at least 3 hours prior to the commencement of their shift and as far as possible the nature of the illness and the estimated period of absence. 15.3.2 The permanent Employee shall produce a medical certificate or other satisfactory evidence to prove the permanent Employee's inability to attend for duty on the days in respect of which sick or carer’s leave is claimed. 15.3.3 A permanent Employee shall not be entitled to paid sick leave for any period in respect of which he or she is entitled to workers compensation.

Sick/Carer’s Leave. 20.1 Employees (excluding casual employees) when absent from work on account of personal illness or an account of injury by accident, other than that covered by Workers’ Compensation, or where the employees is the primary care giver to a sick family member, will be entitled to leave of absence, without deduction of pay subject to the following conditions and limitations: 20.1.1 The employee must within 24 hours of the commencement of such absence inform the Company of their inability to attend for duty, and as far as practicable, state the nature of the injury/illness and the estimated duration of such absence. 20.1.2 The employee must prove to the satisfaction of the Company that he/she was unable (on account of such illness or injury or for carer’s leave purposes) to attend for duty on the day or days for which sick/carer’s leave is claimed. 20.1.3 From the commencement of this Agreement an employee during the first year of employment with the Company will be entitled to sick/carer’s leave entitlement at the rate of 1 day (7.8 hours) after each 5.2 weeks of service. 20.1.4 Provided that an employee who has completed one year of continuous employment with the Company will be credited with a further 10 days (78 hours) sick/carer's leave entitlement at the beginning of the second and each subsequent year. 20.2 Subject to the provision of this clause, an employee will be entitled to claim sick/carer’s pay for two (2) single day absences in any year without substantiating documentation. Thereafter, any single day claimed within the same year will be supported by a statutory declaration. For multiple days claimed at any time, the employee will need to supply to the Company’s satisfaction a certificate from a registered health practitioner supporting the fact that the employee was unable to attend for duty on account of personal illness or injury or for the purposes of carer’s leave. 20.3 Untaken sick/cares leave with the Company will accumulate from year to year. 20.4 If any employee is terminated by the Company and is re-engaged within a period of 6 months, then the employee’s unclaimed balance of sick/carer’s leave will continue from the date of re-engagement. 20.5 Sick leave entitlements are accrued and provided to employees for the purpose of illness or injury as provided in this Clause. Sick/Carer’s Leave. Employees are entitled to 10 days per annum in sick/carers leave, consistent with the Australian Pay and Fair Conditions (AFPC) standard. This will accrue on pro-rata basis each pay period. Assuming no use of Sick/Carers leave, the Employee would have 10 days accrued after 12 completed months of service from commencement/anniversary date. Unused days will accrue, to be available in situations of longer term needs (for example to ‘fund’ the first 90 days before the income protection insurance benefit becomes payable). Additional, unpaid carer’s leave of up to 2 days will be made available where the Employee has utilised their full entitlement of carers leave under the AFPC standard. An Employee unable to attend work on a particular day for sick/xxxxx’s reasons must advise their supervisor as soon as possible – preferably prior to, and no later than 2 hours after, their scheduled commencement of work. Multiple day absences must be supported by a doctor’s certificate (or suitable alternative). For single day absences, Employees may be requested by the Company to provide appropriate documentary support.

Additional unpaid Carer’s leave of up to 2 days per occasion will be made available where the Employee has utilised their full entitlement to paid Sick/Carer’s leave. To be eligible for paid Xxxx/Xxxxx’s leave, an Employee unable to attend work on a particular day for sick/carer’s reasons must advise their supervisor as soon as reasonably practical – preferably prior to and no later than 2 hours after their scheduled commencement of work. The Employee should also advise the supervisor of the nature of the leave and the expected duration. Employees will be allowed 2 single sick/carers leave absences in each year of employment where the need to produce a medical certificate is waived. After the first two single day absences, an Employee must produce a medical certificate or a suitable alternative. The Company reserves the right to require an Employee to produce a medical certificate for any sick/carers above the first two single days absence. For absences greater than 1 day or pre or post a public holiday, or pre or post annual leave, a medical certificate must be produced for the Employee to be paid.
Sick/Carer’s Leave. You will be entitled to paid sick/carer’s leave of 10 days per annum which may be taken as: (a) sick leave for absences due to personal illness or injury; or (b) carer’s leave to provide care or support to a member of your immediate family or household, who requires support because of an illness or injury or because of an unexpected emergency. Sick/carer’s leave accrues on a pro-rata basis with respect to ordinary hours worked, and is cumulative. The Practice may require you to provide a medical certificate from a registered health practitioner evidencing the reason for that personal/carer’s leave. You will be entitled to unpaid carer’s leave of up to 2 days on each occasion when a member of your immediate family or household requires support because of an illness or injury or because of an unexpected emergency. This entitlement only applies if you are not entitled to paid personal/carer’s leave.

Sick/Carer’s Leave. (1) Permanent employees will be entitled to paid leave of absence due to personal illness. The maximum accrued for each year of employment is 80 hours. Any unused leave will be accumulated into the following year. Xxxx leave accumulates pro rata for each four weeks of employment. (2) Any absence from work must be reported to the employee’s direct supervisor or Manager prior to the employees normal start time (or as soon as reasonably practicable). No other form of contact beside a phone call from the employee is acceptable. Only in circumstances where the employee is incapacitated and cannot physically access a phone will contact from a spouse or relative be accepted. Failure to comply with the above may result in the employee not being paid for the period of absence. (3) Any absence extending beyond accumulated sick leave will be considered unpaid leave unless otherwise negotiated with the company (use of annual leave, etc). (4) For any absence of sick leave the company has the discretion to request that the employee supplies a certificate from a registered health practitioner (or a statutory declaration if this is impracticable) to receive payment. (5) For any absence of two consecutive days or more, the employee must supply a certificate from a registered health practitioner (or a statutory declaration if this is impracticable) to receive payment. (6) In any case of extended or recurrent illness, whether work or non-work related, Xxxxxx’x Risk Manager or Rehabilitation Coordinator may provide advice on a return to work program in consultation with the employee and medical practitioner.
