Extended Health and Dental Benefits. Eligible Sessional Academic Staff Members who meet the eligibility requirements, their spouses, and their eligible dependents shall be covered by a family dental and extended health plan in accordance with 24.2.1 –
Extended Health and Dental Benefits. Providing Provincial Legislation permits, the Corporation agrees to pay one hundred percent (100%) of the cost of applicable premiums of the Liberty Health Extended Health Benefit Plan for all regular employees who have completed their probation period and while such employees are in receipt of pay from the Corporation. The plan includes the following, all as per the Corporation's Group Policy No. Life Insurance – The Corporation agree to continue Optional Life insurance (all coverages).
Extended Health and Dental Benefits. Eligible Sessional Academic Staff Members who meet the eligibility requirements, their spouses, and their eligible dependents shall be covered by a family dental and extended health plan in accordance with Appendix K – Memorandum of Agreement on Benefit Provisions. Eligible Sessional Academic Staff Members shall be enrolled in the plans. The University and the members shall share equally the cost of premiums for these plans. The University shall provide the Faculty Association with information concerning premiums and claims as reported to the University by the insurance provider. There shall be an annual meeting with representatives of the insurance provider, the University, and the Faculty Association to discuss issues of common concern. Members of the Pension and Benefits Committee shall be invited to this meeting.
Extended Health and Dental Benefits. Employees choosing to opt out of the above benefits may reapply as late applicants, but will be required to complete a medical questionnaire (Statement of Health). Satisfactory evidence of insurability is required and coverage will only become effective with the approval of the insurer.
Extended Health and Dental Benefits a) The employees of the Livestock shall be members of the SGEU Health and Welfare Trust. The Employer shall provide an Employer paid dental plan to employees. The provisions of the plan shall be as per Appendix C.
Extended Health and Dental Benefits. (i) Effective January 1, 2023, the Employer shall pay eighty-five percent (85%) (increased from seventy-five percent (75%)) and the employees' fifteen percent (15%) (reduced from twenty-five percent (25%)) of the cost of the premiums for the Provincial Extended Health Benefits plan.
(ii) Effective January 1, 2023, the Employer shall pay eighty-five percent (85%) (increased from seventy-five percent (75%)) and the regular employee fifteen percent (15%) (reduced from twenty-five percent (25%)) of the costs of a mutually acceptable Dental Plan. The plan coverage shall include: Plan A — one hundred percent (100%) Plan B — sixty percent (60%) Plan C — fifty percent (50%) and a $3,000 lifetime limit per person
Extended Health and Dental Benefits. The Employer and the Union agree to:
(a) effective the first of the month following the date of ratification, increase the coverage for orthotics to the amount of $300 every five (5) years;
(b) effective the first of the month following the date of ratification, increase the coverage for the following paramedicals as follows: chiropractic and naturopath to a combined maximum of $300.00 per calendar year; acupuncture to a maximum of $200.00 per calendar year; and podiatrist to a maximum of $350.00 per calendar year.
(c) effective 2004 January 01, add coverage for eye exams in the amount of $75 every two (2) years;
(d) effective 2004 January 01, add coverage for oral contraceptives;
(e) effective 2004 January 01, increase the EHB deductible to $50; effective 2005 January 01, increase the EHB deductible to $100.
(f) effective 2006 January 01, increase vision care coverage to $300.00 every two (2) calendar years;
(g) effective 2006 January 01, increase the coverage for chiropractic and naturopath to a combined maximum of $500.00 per calendar year;
(h) effective 2006 January 01, increase the EHB deductible to $150 and increase the lifetime maximum on Dental Plan “C” to $4500. Subject to mutual agreement on the wording, the Employer and the Union agree to reflect the coverage listed in (a), (c), (d), (e), (f) and (h) above in Articles 11.2 and 11.3.
Extended Health and Dental Benefits. When you or your Dependent ceases to be insured under the Group Contract, you may apply to convert to an individual plan. Application for conversion to an individual plan must be made within days of leaving the group. For details regarding conversion of Life Benefits, see Basic Group Life Insurance section. PAYMENT OF DEATH BENEFIT The amount of life insurance coverage in effect on the date of your death (as described in the Summary of Benefits) will be paid to your designated beneficiary when Liberty Health receives proof that you died while covered for this benefit.
Extended Health and Dental Benefits a) The Dental benefits currently in effect shall continue for the term of the collective agreement.
b) The Employer shall pay 100% of the premium costs for these benefits.
c) A prescription pay direct drug card will be provided to cover the appropriate percentage of all eligible expenses for prescriptions purchased from a licensed pharmacy. Reimbursement of eligible drugs and medicines are subject to the tiered formulary found in “My Drug Plan” with Sunlife Financial, which reimburses 80% for drugs in tier 1, 50% for drugs in tier 2 and 20% for drugs in tier 3. The plan shall have a twenty-five dollar ($25) deductible.
d) Regular employees as at October 27, 2016 will be encouraged to switch to a tier one drug (prescription) where appropriate, but where the employee for whatever reason does not, the current prescriptions of those regular employees shall be grandfathered indefinitely, provided they submit the special authority documentation. Changes in Article 35.01 (c) above shall be effective March 1, 2017.
Extended Health and Dental Benefits a. The Extended Health and Dental benefits currently in effect shall continue for the term of the Collective Agreement.
b. The Employer shall pay 50% of the premium costs for these benefits. Effective date of ratification (September 20, 2022), the Employer will pay seventy percent (70%) of the monthly premium for the Extended Health and Dental plan. Effective August 1, 2023, the Employer will pay eighty-five percent (85%) of the monthly premium for the Extended Health and Dental plan. Effective August 1, 2024, the Employer will pay one-hundred percent (100%) of the monthly premium for the Extended Health and Dental plan.
c. Employees enrolled for family coverage under these benefits may change their status to single or couple coverage only if their dependents have alternative benefit coverage.
