Billing and Payments. Transmission Provider shall bill the Interconnection Customer for the Costs associated with the facilities contemplated by this ISA, estimates of which are set forth in the Specifications to this ISA, and the Interconnection Customer shall pay such Costs, in accordance with Section 11 of Appendix 2 to this ISA and the applicable Interconnection Construction Service Agreement. Upon receipt of each of Interconnection Customer’s payments of such bills, Transmission Provider shall reimburse the applicable Interconnected Transmission Owner. Pursuant to Section 212.4 of the Tariff, Interconnection Customer requests that Transmission Provider provide a quarterly cost reconciliation:
Billing and Payments. You shall pay all fees or charges to your account in accordance with the fees, charges, and billing terms in effect at the time a fee or charge is due and payable. Where Services are offered on a free trial basis, payment may be required after the free trial period ends, and not when you enter your billing details (which may be required prior to the commencement of the free trial period). We reserve the right to change products and product pricing at any time. We also reserve the right to refuse any order you place with us. We may, in our sole discretion, limit or cancel quantities purchased per person, per household or per order. Billing and Payments. 8.1 Pursuant to this Agreement, Windstream shall xxxx Spectrotel those charges, which Spectrotel incurs as a result of Spectrotel purchasing Resale Services from Windstream.
8.2 Windstream shall provide Spectrotel a monthly xxxx including all charges incurred by and credits and/or adjustments due to Spectrotel for the Resale Services ordered, established, utilized, discontinued or performed pursuant to this Agreement. Each xxxx provided by Windstream to Spectrotel will include:
8.2.1 all non-usage sensitive charges incurred for the period beginning with the day after the current xxxx date and extending to, and including, the next xxxx date;
8.2.2 any known unbilled non-usage sensitive charges for prior periods;
8.2.3 unbilled usage sensitive charges for the period beginning with the last xxxx date and extending up to, but not including, the current xxxx date; and
8.2.4 any known unbilled usage sensitive charges for prior periods. Windstream will also xxxx all charges, including but not limited to 911 and E911 charges, telecommunications relay charges, and franchise fees, to Spectrotel.
8.3 Any switched access charges associated with interexchange carrier access to the resold local exchange lines will be billed by, and due Windstream from the Interexchange Carrier.
8.4 All End User common line (EUCL) charges, subscriber line charges (SLC), or other similar charges will continue to apply for each local exchange line resold under this Agreement. All applicable federal and state rules, tariffs and regulations associated with such charges shall be applicable, as may be amended from time to time.
8.5 Each Party will provide the other Party at no charge a contact person for the handling of any Resale Billing questions or problems.
8.6 Windstream will render bills each month on established xxxx days for each of Spectrotel's accounts.
8.7 If Spectrotel requests an additional copy(ies) of a xxxx, Spectrotel will pay Windstream a reasonable fee per additional xxxx copy, unless such copy(ies) was requested due to errors, omissions, or corrections, or the failure of the original transmission to comply with the specifications set forth in this Agreement.
8.8 Payment of all charges will be the responsibility of Spectrotel. Spectrotel shall make payment to Windstream for all Resale Services billed. If you have a health insurance policy, it will usually provide some coverage for mental health treatment. If I am an out-of-network provider, your plan may include deductible and higher co-pay than if I am an in-network provider. I will bill your insurance company, unless we agree otherwise, and provide you with an invoice of our sessions, however you, not your insurance company, are responsible for full payment of my fees. You may pay by cash, check, or credit card. However, if your check bounces, you will be responsible for paying the fee that my bank charges me as well as the full amount. Thereafter I will no longer accept checks from you and you must pay in cash or by credit card. I will contact your insurance company or EAP provider to verify your mental health benefits. Since mental health benefits may differ from medical benefits, it is very important that you find out exactly what mental health services your insurance policy covers. Billing and Payments. The Accounts have various billing and payment structures, including varying periodic finance charges and fees. The following is information on the current billing and payment characteristics of the Accounts. With limited exceptions (for example, where there is no outstanding balance on a cardholder’s credit card account), monthly billing statements are sent by the Seller to cardholders at the end of the billing period. Each month, the cardholder is required to make at least the minimum payment indicated on the monthly billing statement. The minimum payment amount is typically calculated by one of two methods: (1) where the cardholder (or if held jointly, all cardholders) on the credit card account reside(s) outside the province of Quebec or is a small business credit card account, by adding the amount of any outstanding periodic finance charges plus fees outstanding (excluding any installment plan interest or monthly fees) plus $10, plus any monthly installment plan payments due that statement; or (2) for accounts held individually or jointly by any person resident in the province of Quebec, by adding any monthly installment plan payments due that statement, plus a percentage of the account’s other total outstanding balance (which percentage will be gradually increasing from 2.5% to 5% between August 1, 2019 and August 1, 2025 for Accounts opened prior to August 1, 2019, and which is 5% for Accounts opened on or after August 1, 2019). In all cases, a balance of $10 or less on a monthly billing statement must be paid in full. The full amount of any unpaid minimum payment required in respect of preceding monthly billing statements, will also be added to the minimum payment for a billing period. Payments by cardholders to the Servicer on the Accounts are processed and applied first to the minimum payment, then to any remaining balance. If the different amounts that make up the remaining balance are subject to different interest rates, the payments are generally allocated on a pro rata basis. You are agreeing here to be responsible for the fee we establish. If you have any questions about your insurance coverage you should ask your plan administrator. If you choose to request reimbursement from your insurance company for your treatment expenses, you should be aware that your contract with your health insurance company likely requires that I provide it with information about my services to you. I am usually required to provide a clinical diagnosis. Sometimes I am required to provide additional clinical information such as treatment plans or summaries, or copies of your entire Clinical Record. In such situations, I will make every effort to release only the minimum information about you that is necessary for the purpose requested. This information will become part of the insurance company files and will probably be stored in a computer. Though all insurance companies claim to keep such information confidential, I have no control over what they do with it once it is in their hands. In some cases, they may share the information with a national medical information databank. I will provide you with a copy of any report I submit, if you request it. By signing this Agreement, you agree that I can provide requested information to your carrier if you choose to request reimbursement for fees paid to me by you personally or through your insurance plan.
