Extended Sick Leave Bank. There shall be established for all members of the bargaining unit an extended sick leave bank which shall be administered by the Office of Human Resources, established and utilized according to the following procedures:
Extended Sick Leave Bank. Each employee’s Sick Leave Bank shall accrue to a maximum of seven hundred twenty (720) hours. Only Sick Time shall accrue to the Extended Leave Bank. Employees can donate a portion of their Extended Sick Leave Bank to a fellow employee who has exhausted all of their current year Sick Leave and their Extended Sick Leave Bank, with the approval of the Fire Chief or designee. When an employee leaves the department, any remaining balance shall not be compensated.
Extended Sick Leave Bank. AHP's who are members of the LAAHP may elect to participate in the LEA Sick Bank. Prior to September 15th of each school year in which the agreement is in effect, all members of the negotiating unit may elect to donate one sick leave day from her annual fifteen-day sick leave entitlement. The bank of extended sick leave days may be used by members who so donate under the following conditions: 1. Once established, the bank shall not be increased during the school year. Exhaustion of the bank shall be a bar to the provision of further benefits during that year. 2. Unused sick leave days in the bank at the close of the school year shall carry over into subsequent years up to a unit of two times the number of staff, but will not revert to the membership of the bank. The Association will provide the Superintendent with a report by August 15th. 3. A member of the bank shall be eligible for benefits from the bank only after her/his accumulated entitlement, as specified in Paragraph "B" of this article is exhausted. 4. A committee of members of the bank, to include the Director of Human Resources (with no veto vote) shall be established by the members of the bank to receive requests for benefits and to grant the same within the limitations of this paragraph. Whenever such benefits are granted, the committee shall immediately inform the superintendent’s office so that a proper accounting may be made. 5. A member who is eligible for sick leave benefits may apply for such after an incapacitating illness or disability of twenty (20) working days. Such application shall be accompanied by written evidence of disability from a licensed physician and a copy of which will be made available to the Board. 6. Upon similar application, such member may be granted further benefits from the pool of available days in the bank as may be determined by the committee. 7. The committee may establish guidelines for the allocation of funds from the bank provided such guidelines are consistent with the provisions of this section and the Family Medical Leave Act (FMLA). 8. Notwithstanding any other provisions of this Agreement to the contrary, controversies arising out of the application of this bank shall not be subject to the grievance procedure in Article 5.
Extended Sick Leave Bank. Unused Sick Leave balances accrued by employees hired before 1/1/04 who choose to convert to the Paid Leave benefit plan will be placed in their Extended Sick Leave Bank. Hours in the Extended Sick Leave Bank may be used by an employee for the following: • Personal illness or disability • Medical, optical, or dental appointments with prior approval of Department Head • Prenatal and postnatal care • Treatment for mental illness, alcoholism, or drug addiction • Isolation to minimize threat or spread of contagious disease • For such reasonable periods as the employee’s attendance with his/her child is necessary on the same terms that the employee him/herself is able to use sick leave benefits • For the illness of spouse or parents as is necessary, at the rate of three (3) days per occurrence To access their Extended Sick Leave Bank, employees shall be required to file with their supervisor competent written evidence of the medical need for their absence. Any unused Extended Sick Leave Bank hours remaining when an employee separates from employment shall be forfeited.
Extended Sick Leave Bank. Non probationary full and part-time employees shall accrue up to a maximum of twenty-eight (28) hours of extended sick leave per year based on an accrual rate of .01346 per hour worked by such full and part-time employees. Extended sick leave is accrued on the first eighty (80) hours of work time per pay period (excluding extended sick leave, education leave, funeral/bereavement leave, standby, and short and long term disability). a. The amount of accrued extended sick leave will be listed on the employee's paycheck. b. The extended sick leave benefit described above may be used for periods of illness which occur during an employee's regular work schedule that lasts more than twenty-four (24) scheduled hours of work. PTO may be used during the initial three (3) working day, five (5) calendar days or twenty-four (24) period of illness if available in an employee's PTO bank. PTO may be used when extended sick leave benefits are exhausted if available in an employee's PTO bank. Extended sick leave benefits shall only be provided upon satisfactory proof of illness, not limited to, a doctor's excuse. Notwithstanding the above, employees may use their extended sick leave benefits for any hospitalization stay beginning with the first day of overnight hospitalization or any outpatient surgical procedure as defined as qualifying by Hospital policy. Extended sick leave benefits shall have no maximum cap. Extended sick leave benefits shall not be convertible to cash under any circumstances, nor shall extended sick leave benefits be paid to any employee upon separation of employment at Bay Area Hospital. Employees who work as temporary employees, relief or per diem employees, shall be ineligible to receive any accrual of extended sick leave benefits.

Extended Sick Leave Bank. An extended sick leave bank shall be established for each employee to be used in the event of a continuous illness or an extended disability, which would require the use of more sick leave than is accumulated. A request in writing, supported by an M.D. certificate indicating the nature of the illness and/or disability and the probable date of return to duty, shall be presented upon request. A medical examination performed by the school physician may be required if the Superintendent of Schools deems it necessary. Extended sick leave for this bank shall be accumulated as follows: 1. One half day of sick leave shall be placed in the bank for each day of unused sick leave to a maximum of 50 such days. Such accumulation shall be from the initial date of employment. 2. Such days shall be used only when there are no sick days remaining which were accumulated as part of the 200 days described in (A) above. 3. The calculation for this bank shall be determined on June 30 of each school year only and employees shall be notified of their status relative to their total accumulation.
Extended Sick Leave Bank. Care Of Relatives Accrued sick leave may be used when an employee cannot perform work duties due to but not limited to the following: personal illness or injury; necessity for medical or dental treatment or examination, where such treatment cannot be scheduled outside of working hours; emergency, illness or injury of the employee’s immediate family member which requires the employee’s attendance and care; quarantine directed by a medical physician; disability; pre and postnatal care. For the purpose of this paragraph, immediate family is defined as; spouse, child, stepchild, adult child, parent, stepparent, mother-in-law, father-in-law, or grandchild. To the extent that state statute (§181.9413) or regulations change, this policy shall be construed as consistent with those changes. Sick leave may be used because of illness of the employee’s sibling or grandparent as well. For siblings and grandparents, use is limited to 160 hours all combined per calendar year. An employee must notify the employee’s supervisor of sick leave usage prior to the employee's starting time, unless an emergency prevents the employee from doing so. Failure to give such notice may be cause for disciplinary action. The County reserves the right to require written medical certification from an employee. In the event of three (3) consecutive days of absence or in cases of the repeated and systematic absence of an employee the Department Head or Supervisor may require a medical statement from an appropriate medical authority before granting sick leave, as well as verification that an employee is able to perform the duties of employment before the employee is allowed to return to work.
Extended Sick Leave Bank. The ESLB shall be available and shall be funded by sick leave converted from the sick leave banks of those employees that were in existence prior to the creation of PTO. For employees hired prior to July 1, 2014, absence due to illness, disability or injury may be taken from the ESLB or from the PTO Plan, at the employee’s choice. However, the supervisor may require acceptable medical verification before approving use of ESLB time. Employees shall be paid 50% of their ESLB not to exceed 400 hours upon termination of employment.
