Extended Sick Leave Bank. AHP's who are members of the LAAHP may elect to participate in the LEA Sick Bank. Prior to September 15th of each school year in which the agreement is in effect, all members of the negotiating unit may elect to donate one sick leave day from her annual fifteen-day sick leave entitlement. Exhaustion of the bank shall be a bar to the provision of further benefits during that year. 2. The Association will provide the Superintendent with a report by August 15th. 3. A member of the bank shall be eligible for benefits from the bank only after her/his accumulated entitlement, as specified in Paragraph "B" of this article is exhausted. 4. A committee of members of the bank, to include the Director of Human Resources (with no veto vote) shall be established by the members of the bank to receive requests for benefits and to grant the same within the limitations of this paragraph. Whenever such benefits are granted, the committee shall immediately inform the superintendent’s office so that a proper accounting may be made. 5. A member who is eligible for sick leave benefits may apply for such after an incapacitating illness or disability of twenty (20) working days. Such application shall be accompanied by written evidence of disability from a licensed physician and a copy of which will be made available to the Board. 6. Upon similar application, such member may be granted further benefits from the pool of available days in the bank as may be determined by the committee. 7. The committee may establish guidelines for the allocation of funds from the bank provided such guidelines are consistent with the provisions of this section and the Family Medical Leave Act (FMLA). 8. Notwithstanding any other provisions of this Agreement to the contrary, controversies arising out of the application of this bank shall not be subject to the grievance procedure in Article 5.

Extended Sick Leave Bank. Care Of Relatives Accrued sick leave may be used when an employee cannot perform work duties due to but not limited to the following: personal illness or injury; necessity for medical or dental treatment or examination, where such treatment cannot be scheduled outside of working hours; emergency, illness or injury of the employee’s immediate family member which requires the employee’s attendance and care; quarantine directed by a medical physician; disability; pre and postnatal care. For the purpose of this paragraph, immediate family is defined as; spouse, child, stepchild, adult child, parent, stepparent, mother-in-law, father-in-law, or grandchild. To the extent that state statute (§181.9413) or regulations change, this policy shall be construed as consistent with those changes. Sick leave may be used because of illness of the employee’s sibling or grandparent as well. For siblings and grandparents, use is limited to 160 hours all combined per calendar year. An employee must notify the employee’s supervisor of sick leave usage prior to the employee's starting time, unless an emergency prevents the employee from doing so. Failure to give such notice may be cause for disciplinary action. The County reserves the right to require written medical certification from an employee. In the event of three (3) consecutive days of absence or in cases of the repeated and systematic absence of an employee the Department Head or Supervisor may require a medical statement from an appropriate medical authority before granting sick leave, as well as verification that an employee is able to perform the duties of employment before the employee is allowed to return to work.
Extended Sick Leave Bank. An extended sick leave bank shall be established for each employee to be used in the event of a continuous illness or an extended disability, which would require the use of more sick leave than is accumulated. A request in writing, supported by an M.D. certificate indicating the nature of the illness and/or disability and the probable date of return to duty, shall be presented upon request. A medical examination performed by the school physician may be required if the Superintendent of Schools deems it necessary. Extended sick leave for this bank shall be accumulated as follows: 1. One half day of sick leave shall be placed in the bank for each day of unused sick leave to a maximum of 50 such days. Such accumulation shall be from the initial date of employment. 2. Such days shall be used only when there are no sick days remaining which were accumulated as part of the 200 days described in (A) above. 3. The calculation for this bank shall be determined on June 30 of each school year only and employees shall be notified of their status relative to their total accumulation.
Extended Sick Leave Bank. If an employee leaves and has taken more vacation than has accrued, the excess will be deducted from the final paycheck.
Extended Sick Leave Bank. The ESLB shall be available and shall be funded by sick leave converted from the sick leave banks of those employees that were in existence prior to the creation of PTO. For employees hired prior to July 1, 2014, absence due to illness, disability or injury may be taken from the ESLB or from the PTO Plan, at the employee’s choice. However, the supervisor may require acceptable medical verification before approving use of ESLB time. Employees shall be paid 50% of their ESLB not to exceed 400 hours upon termination of employment.
