Group Benefit Plan. The Group Benefit Plan Booklet will provide you additional information on the benefit entitlement identified in this article. The Extended Health Benefits, Group Life, Short-Term Disability and Long-Term Disability insurance, as well as the Dental Plan, will all be paid in accordance with the schedule of benefits listed in the carrier's plans, and subject to the limitation and eligibility requirements specified in the Plans. A current copy of the Group Benefit Plan booklet is available on the Xxxxxxx College website.
Group Benefit Plan. Carefor agrees to provide full-time Nurses with the insured benefits as described in the Group Policy Number G0037436 - Plan C. Enrolment and participation by the Nurse will be in accordance with the terms and conditions of the Carefor Group Benefit Plan. Carefor shall provide each Nurse with information booklets outlining the current provisions in the insured benefit plan set out below. Upon request, Carefor will make the plan available to the Association.
Group Benefit Plan. The Employer shall make arrangements with an insurance carrier to provide a group benefit plan for all employees covered by this Agreement. The selection of the insurance carrier is in the sole discretion of the Employer.
Group Benefit Plan. Carefor agrees to provide full-time Nurses with the insured benefits as described below and as described in the Group Policy Number G0037436. Enrolment and participation by the Nurse will be in accordance with the terms and conditions of the Carefor Group Benefit Plan. Carefor shall provide each Nurse with information booklets outlining the current provisions in the insured benefit plan set out below. The Employer reserves the right to change carriers at the sole discretion of the Employer. The Employer will advise the Association of any change in carrier or underwriter at least sixty (60) days prior to implementing a change in carrier.
(a) Extended Health /Dental Care The Employer agrees to contribute one hundred percent (100%) of the premium for Extended Health Care and Dental Care, based on single, couple or family coverage. It is a condition of employment for Nurses to participate in Extended Health/Dental benefits unless they have at least equal coverage under a spousal plan.
(b) Basic Life Insurance Employer agrees to pay one hundred percent (100%) of the billed premium. Coverage is based on two times annual salary. Participation is mandatory.
Group Benefit Plan. 34.01 A group benefit plan (medical, dental and group life insurance) is provided to eligible employees on the terms set out in the plans. Detailed information on the group benefit plan is available upon request from the Employer’s Administration Office. Each employee upon enrollment in the group benefit plan shall receive a copy of the booklet outlining benefit coverage and eligibility. The Employer agrees to consult with the Union prior to making any changes to the group benefit plan.
34.02 Permanent employees are required to enroll in the group benefit plan. The employee can waive medical coverage (not life insurance, which is mandatory) if they are covered under another medical plan.
34.03 Permanent employees will be enrolled in the group benefit plan the month following satisfactory completion of their probationary period. The cost of the group benefit plan is as follows: ● Life insurance, 100% Employer paid; ● Dependent life insurance, 100% Employer paid (as applicable); ● Long term disability insurance, 100% employee paid; ● Extended health care, 40% Employer paid; ● Dental insurance, 40% Employer paid.
34.04 Casual employees who work at least twenty (20) hours per week and who wish to enroll in the group benefit plan after meeting the eligibility requirements of the insurer may voluntarily join the group benefit plan and the cost of the plan is as follows: ● Life insurance, 100% Employer paid; ● Dependent life insurance, 100% Employer paid as applicable; ● Long term disability insurance, 100% employee paid; ● Extended health care, 40% Employer paid; ● Dental insurance, 40% Employer paid.
34.05 An eligible employee on unpaid leave of absence or an employee who does not have sufficient scheduled hours of work to make regular payroll deductions, may remain enrolled in the group benefit plan provided the employee pays one hundred percent (100%) of the monthly cost of the group benefit plan in advance to the Employer’s Administration Office.
34.06 Employees while receiving Worker’s Compensation Benefits may remain enrolled in the group benefit plan for up to one year provided the employee continues to pay their share (40% or 100%, as applicable), of the monthly cost of the group benefit plan monthly in advance to the Employer’s Administration Office.

33.04 Casual employees who worked at least one thousand (1000) regular hours or more in the preceding year and the current year indicates the same, may voluntarily join the group benefit plan and the cost of the plan shall be co-shared between the Employer and the employee on a fifty (50%) fifty (50%) basis, with the employees share being made by bi- weekly payroll deduction. Casual employees who wish to enroll in the group benefit plan after meeting the eligibility requirements must notify the Employer of their wish to do so by June 30 or December 31 in their year of eligibility.
33.05 An eligible employee on unpaid leave of absence or an employee who does not have sufficient scheduled hours of work to make regular payroll deductions, may remain enrolled in the group benefit plan for up to one year provided the employee pays one hundred percent (100%) of the monthly cost of the group benefit plan in advance to the Employer’s Administration Office. The parties agree that, subject to the requirements of the group benefit plan, the one-year limitation on enrollment into the group benefit plan may be extended upon mutual agreement of the Employer and the Union.

Group Benefit Plan. The Employer will pay the full cost of providing a group benefit plan providing Extended Health Care Benefits, Long-term Disability coverage, Dental, and Vision Care for employees after three (3) months of continuous service. Short-term disability benefits are provided for employees following successful completion of the probationary period and shall start on the first (1st) day of absence due to an accident and the fourth (4th) day of absence due to illness. A summary of the Plan is provided upon enrolment. The Employer shall not be responsible for any benefit claims that the insurance carrier has determined to be non-qualifying claims and shall not be subject to the grievance and arbitration procedures of this Agreement.
Group Benefit Plan. (a) The Employer shall maintain a Group Life Insurance and Medical Care Plan. The Employer and eligible employees shall each pay fifty (50%) percent of the cost of the plan.
(b) The Group Benefit Plan shall be part of this collective agreement. There shall be a Labour Management Committee composed of three (3) non-bargaining unit employees, three (3) bargaining unit employees, Union Business Agent and the General Manager. The Committee shall study; review claims experience, alter the plan and choose the carrier. The Committee shall have full access to all pertinent information concerning the Plan and request the attendance of a representative of the insurer when necessary.
Group Benefit Plan. The Employer and the Union agree on the provisions of a Group Benefit Plan, consisting of:
(a) Basic Group Life Insurance (3 x annual salary);
(b) Accidental Death, Disease & Dismemberment (scheduled benefits to a maximum of 3 x annual salary);
(c) Dependants Life Insurance; and
(d) Long Term Disability, as a term and condition of employment for all eligible employees. The Plan is to be cost-shared between the Employer and employees with the Employer contributing 50% and the employees contributing 50% of the total cost. The employee’s contribution shall be used to pay 100% of the cost of the Long Term Disability premium and secondly to contribute to the cost of the Group Life and Accidental Death, Disease and Dismemberment premiums.
