SICK LEAVE, PENSION AND GROUP MEDICAL AND HOSPITALIZATION PLANS. BENEFIT PLANS 10.1 The benefits provided hereunder shall continue for the life of this Agreement. 10.2 The Employer shall pay the full cost of the premiums of all benefits provided hereunder. 10.3 On completion of the probationary period an Employee shall be entitled to the following benefits: (a) Group Life Insurance, as per the attached Appendix "D". The detailed terms and conditions of your group life benefits are described in the Contract held by the Employer with the Insurance Carrier. (b) Extended Medical Plan, as per the attached Appendix "E". The detailed terms and conditions of your extended health benefits are described in the Contract held by the Employer with the Insurance Carrier. (c) Dental Care Plan under the terms of the current Ontario Dental Association (O.D.A.) fee schedule. The detailed terms and conditions of your extended health benefits are described in the Contract held by the Employer with the Insurance Carrier. (d) Employees will be given access to Short Term Disability Protection as detailed in the attached plan, Appendix "C" after completion of their probationary period. The provisions of the "Cumulative Sick Leave Allowance" Bylaw, as amended shall continue as modified by the Income Protection Plan. (e) For the purposes of this benefit where Employees are required by law to obtain a physical examination these occasions will not be charged against the Employee's Short Term Disability entitlements. It is understood that Employees will arrange for these physicals to be taken at either the beginning or the end of the workday. (f) Long Term Disability protection as per the attached Appendix “C”. (g) Where an Employee is required by the Employer to be immunized, the Employer agrees to provide or reimburse Employees for the cost of immunizations not covered by OHIP. Where a prophylactic alternative to immunization is available it may be taken as a substitute to immunization where appropriate based on medical or religious grounds. It is understood that the Employer cannot force an Employee to be immunized or to take the prophylactic alternative without their consent. It is further understood that where such immunization (or the prophylactic alternative to immunization) is required in order for the Employee to attend work and the Employee refuses the immunization or its substitute, they may be placed on unpaid leave with no loss of seniority. SICK LEAVE, PENSION AND GROUP MEDICAL AND HOSPITALIZATION PLANS. 12.1 The benefits provided hereunder shall continue for the life of this Agreement: (a) Employees are entitled to provisions of the Short Term Income Protection Plan as attached hereto. (b) Employer shall introduce and pay the full cost of the premiums of a Long Term Disability Plan which shall be subject to tender and implementation in accordance with the terms of the Long Term Plan attached hereto. 12.2 The Employer shall pay the full cost of the premiums of a Group Life Insurance plan for all Employees who have completed their probationary period. Benefits under the plan shall equal two (2) times the annual basic wage rate of the Employee to the nearest one thousand dollars. All Employees will receive a copy of the plan. 12.3 The Employer will provide Accidental Death and Dismemberment insurance for Employees actively at work at the benefit rate of twice annual earnings. 12.4 The Employer shall pay the full cost of the premium for dental care, for all Employees on completion of their probationary period, under the terms of the ODA fee guide in place one (1) year prior to the current calendar year. The dental plan provides for 100% reimbursement for dental coverage equivalent to the Blue Cross Plan #9, which includes basic preventative services and including endodontics and periodontics. The Dental Plan shall also include coverage for removable prosthodontics, fixed prosthodontics, and major restorative on the basis of a 50% co-insurance with a maximum amount payable of One Thousand Dollars ($1,000.00) per calendar year. Dental recall (6 months under the age of twelve and every nine months for those 12 years and older) The Dental Plan shall also include coverage for Orthodontics for dependent children (to the age of 18) on the basis of 50% co-insurance with a lifetime maximum payment of One Thousand Five Hundred Dollars ($1,500) per dependent child. 12.5 The Employer shall pay the full cost of the premium for extended health benefits, for all Employees on completion of their probationary period. Eligible expenses, in excess of the deductible, are reimbursed at 100%. Eligible expenses include charges for Drugs (Formulary Two), Private Nursing, Physiotherapy, Diagnostic Services, Accidental Dental, Prosthetic Appliances, Durable Medical Equipment, Hearing Aids, Medical Services and Supplies, Ambulance, Clinical Psychologist, Masseurs, Speech Pathologist, Chiropractors and Out of Province Emergency Treatment, and Homeopathic Care - $200.00 limit per family ...
SICK LEAVE, PENSION AND GROUP MEDICAL AND HOSPITALIZATION PLANS. 24.1 Employees covered by this Agreement other than students hired exclusively for the summer period (May - September) shall be paid bi-weekly, 12% of their earnings, exclusive of vacation pay, in lieu of any other sick leave, pension, group medical and hospitalization, life insurance and any other benefit. 24.2 The age of retirement shall be the first day of the month following the Employee's 65th birthday. 24.3 In the event an Employee is required to participate in the OMERS pension plan under OMERS regulations they shall be enrolled. The Employer shall make the Employer's required contributions and the Employee shall receive 6% in lieu of benefits rather than the 12% noted in article 26.1.

  • Health and Welfare Benefit Plans During the Employment Period, Executive and Executive’s immediate family shall be entitled to participate in such health and welfare benefit plans as the Employer shall maintain from time to time for the benefit of senior executive officers of the Employer and their families, on the terms and subject to the conditions set forth in such plan. Nothing in this Section shall limit the Employer’s right to change or modify or terminate any benefit plan or program as it sees fit from time to time in the normal course of business so long as it does so for all senior executives of the Employer.

  • Welfare, Pension and Incentive Benefit Plans During the Employment Period, Executive (and his eligible spouse and dependents) shall be entitled to participate in all the welfare benefit plans and programs maintained by the Company from time-to-time for the benefit of its senior executives including, without limitation, all medical, hospitalization, dental, disability, accidental death and dismemberment and travel accident insurance plans and programs. In addition, during the Employment Period, Executive shall be eligible to participate in all pension, retirement, savings and other employee benefit plans and programs maintained from time-to-time by the Company for the benefit of its senior executives, other than any annual cash incentive plan.

  • Defined Benefit Pension Plans The Borrower will not adopt, create, assume or become a party to any defined benefit pension plan, unless disclosed to the Lender pursuant to Section 5.10.

  • Health & Welfare Benefits Executive shall be eligible to participate in all health and welfare benefits provided generally to other employees of the Company.

  • Welfare Benefit Plans During the Employment Period, the Executive and/or the Executive's family, as the case may be, shall be eligible for participation in and shall receive all benefits under welfare benefit plans, practices, policies and programs provided by the Company and its affiliated companies (including, without limitation, medical, prescription, dental, disability, employee life, group life, accidental death and travel accident insurance plans and programs) to the extent applicable generally to other peer executives of the Company and its affiliated companies, but in no event shall such plans, practices, policies and programs provide the Executive with benefits which are less favorable, in the aggregate, than the most favorable of such plans, practices, policies and programs in effect for the Executive at any time during the 120-day period immediately preceding the Effective Date or, if more favorable to the Executive, those provided generally at any time after the Effective Date to other peer executives of the Company and its affiliated companies.

  • Pension and Welfare Plans During the twelve-consecutive-month period prior to the Closing Date and prior to the date of any Credit Extension hereunder, no steps have been taken to terminate any Pension Plan, and no contribution failure has occurred with respect to any Pension Plan sufficient to give rise to a Lien under Section 302(f) of ERISA. No condition exists or event or transaction has occurred with respect to any Pension Plan which might reasonably be expected to result in the incurrence by the Borrowers or any member of the Controlled Group of any material liability, fine or penalty. Except as disclosed in Item 6.11 of the Disclosure Schedule, neither any Borrower nor any member of the Controlled Group has any contingent liability with respect to any post-retirement benefit under a Welfare Plan, other than liability for continuation coverage described in Part 6 of Title I of ERISA.

  • Retirement and Welfare Benefits During the Term, the Executive shall be eligible to participate in the Company’s health, life insurance, long-term disability, retirement and welfare benefit plans, and programs available to similarly-situated employees of the Company, pursuant to their respective terms and conditions. Nothing in this Agreement shall preclude the Company or any Affiliate (as defined below) of the Company from terminating or amending any employee benefit plan or program from time to time after the Effective Date.
