Employee Appraisal. (a) When a formal appraisal of an employee's performance is carried out, the employee shall be given sufficient opportunity to read, review and ask questions about the appraisal. The employee will be given until the next working day to read and review the appraisal.
(b) Upon request, an employee shall have the right to have her xxxxxxx present during the performance appraisal meeting.
(c) The appraisal form shall provide for the employee's signature in two places; one indicating that the employee has read and accepts the appraisal, and the other indicating that the employee disagrees with the appraisal. No employee may initiate a grievance regarding the contents of an employee appraisal unless the employee has signed in the place indicating disagreement with the appraisal.
(d) An employee shall receive a copy of the employee appraisal at time of signing.
(e) An employee appraisal shall not be changed after an employee has signed it, without the knowledge of the employee, and any such changes shall be subject to the grievance procedure of this agreement.
(f) An employee shall have the right to request a formal appraisal at any time providing that at least three (3) months have elapsed since the last appraisal.
(g) The formal appraisal form shall include a section that will enable the employee to make comments about her appraisal.

The form shall provide for the employee's signature in two (2) places, one indicating that the employee has read and accepts the appraisal, and the other indicating that the employee disagrees with the appraisal. The employee shall sign in one of the places provided. An employee shall, upon request, receive a copy of the employee appraisal at the time of signing. An employee appraisal shall not be changed after an employee has signed it without the knowledge of the employee. An employee may submit rebuttal documentation, to be placed on file, in response to the appraisal.
Employee Appraisal. Employee Appraisals are designed to provide Employees with opportunity for continued professional growth and development. The process should assist with planning professional development and providing information to facilitate decision-making about work assignments and continuing appointments.
Employee Appraisal. The Bartlesville Board of Education believes that the primary purpose of appraisal is to improve performance and that improvement of individual performance is not accidental but is accomplished through deliberate effort. The Board believes that successful job performance is a joint achievement and unsuccessful job accomplishment is a shared responsibility.
Employee Appraisal. Where a formal evaluation of an employee's performance is carried out, the employee shall be provided with a copy to read and review. The form shall provide for the employee's signature in two places, one indicating that the employee has read and accepts the evaluation, and the other indicating that the employee has read and disagrees with the evaluation. The employee shall sign in one of the places provided within 48 hours of the receipt of the appraisal. The employee shall be provided with a copy of the signed appraisal. No employees may initiate a grievance regarding the contents of an evaluation report unless the signature indicates disagreement with the evaluation.
Employee Appraisal. 15:1 Evaluation and judging of an employee's performance shall be for, but not limited to, the following purposes:
(a) To provide teachers with feedback and support for continuous self-improvement.
(b) To provide an enhancement process in order to promote creativity, innovation, and risk-taking.
(c) To motivate members of the staff to participate in formulating and evaluating instructional programs.
(d) To provide an atmosphere of cooperation between administrators and teachers throughout the evaluative process.
(e) To provide information for decisions on in-service training and staff improvement programs.
(f) To provide information for making judgments about personnel promotions, reassignments, tenure, and dismissal.
15:2.1 The Board and Association shall maintain an on-going appraisal committee. See Article 32.
15:2.2 The state approved appraisal system shall be the official system used to appraise employees. All administrators doing teacher appraisals will be trained according to State procedures.
Employee Appraisal. A Performance Evaluation and Review Program (PERP) has been prepared based on the Job Descriptions referred to in subclause (ii) above.
Employee Appraisal. PERFORMANCE PLAN Prior to
Employee Appraisal.
9.1 Appraisal of an employee's performance shall be for the purpose of assessing current performance and improving future performance.
9.2 An employee shall be given a copy of the appraisal report. Each appraisal report shall be signed by the person doing the appraisal.
9.3 An employee shall be given an opportunity to sign each appraisal report. If an employee refuses to sign an evaluation report, that fact shall be noted on the report. An employee's signature only indicates that the report has been read by the employee and does not evidence the employee's agreement with the findings or conclusions of the report.
9.4 There shall be a conference with the evaluator to discuss the report. The conference shall take place within three (3) days of the date the employee receives a copy of the appraisal report. Following the conference the appraisal report shall be placed in the employee's file.
9.5 An employee may attach written comments to the appraisal form. The employee's written comments shall only become part of the file if they are submitted by the employee to the evaluator within ten (10) working days of the conference to discuss the appraisal report.
9.6 All employees shall have an appraisal at least one (1) time a year. Should an employee receive an unsatisfactory appraisal, an additional appraisal will be done within three (3) months. After receiving three (3) unsatisfactory appraisals, the employee may be terminated, provided the employee failed to meet the recommendations of each improvement plan.
9.7 Appraisal reports shall be:
a) Prepared by and issued in the name of the immediate supervisor.
b) Addressed to the employee.
c) In narrative form including strengths and weaknesses of the employee, specific concerns, dates of the occurrence of performance judged unsatisfactory, and with specific recommendations as to measures which the employee should take to improve upon performance in areas of weakness.
9.8 Time limits for Article Nine may be extended by mutual written agreement.
9.9 Final evaluation of an employee upon termination of his/her employment shall be concluded as of the last date of employment, and no further evaluation statement shall be placed in the personnel file of such employee after such date.
Employee Appraisal. 28.01 Process
(i) An evaluation of each continuing employee shall be completed at least once every three (3) years. After two (2) evaluations confirming the faculty member's competence, a simple statement by the Xxxx added to the faculty member's file, attesting to continuing satisfactory performance shall normally meet the requirements of this clause. The methods of evaluation and the criteria to be used shall be developed by the Xxxx or Vice-President as appropriate in consultation with the Chairs in the School. The methods of evaluation and the criteria used shall be fair and reasonable. The Xxxx or Vice-President as appropriate will ensure that all employees are aware of the methods of evaluation and criteria to be used.
(ii) All faculty members on term appointments shall be evaluated according to the procedures established for their school within the first 8 (eight) months of aggregate appointment periods (independent of percentage of full-time employment). A second evaluation shall be completed prior to the completion of four (4) semesters or six (6) quarters of employment, also irrespective of the percentage of full-time employment.
(b) When a formal performance review of an employee is completed, that employee will be given the opportunity to read and review the appraisal. Provision shall be made on the evaluation form for the employee's signature to indicate that he/she has read the report and either accepts or rejects the appraisal. No employee shall initiate a grievance regarding the contents of an evaluation report unless the signature indicates disagreement with the appraisal. The employee shall receive a copy of the report. Changes in the methods of formal evaluation will be discussed with the appropriate Union representative before being implemented. An appraisal shall not be changed after an employee has signed it without the knowledge of the employee, and any such changes shall be subject to the grievance procedures of this Agreement.
(c) The completed appraisal shall be available only to the employee, his/her Chair, his/her Xxxx, the appropriate vice-president, and the President. In the event of a dispute or legal action, the appraisal shall be available to a joint Union/Employer Committee, an arbitrator board and the legal counsels of the Employer and the Union.
