Medical and Group Insurance. At the expense of the Employer, Employer agrees to include Employee in the group medical and hospital plan of Employer, when such plan is established, and will provide group life insurance for Employee in the amount of not less than Three Hundred Fifty Thousand Dollars ($350,000) during the term of employment.

An employee who is incapacitated for duty through illness shall be paid for the time off from work for such illness or accident for up to six (6)months. Should such illness or accident exceed this six (6) month period the employee must apply before the expiry period for Long Term Disability Sick leave with pay may not apply to an employee during the first three (3) months of employment. Where the Company discharges or suspends for at least one (1) month an employee who is entitled to coverage and who has completed the probationary period and has filed a grievance disputing such discharge or suspension, benefits set out in the welfare provisions of the Agreement shall cease for the employee. The Company shall, within one week of the discharge or suspension, send by registered mail to the address of the employee on record with the Company, a copy of this provision of the Collective Agreement, together with the amount of premium requiredand the dates when such are due. The employee may, at option, reply in writing, within two (2) weeks from the filing of grievance on the discharge or suspension, to the Company for continued coverage under this Article, and pay to the Company the amount of any premiums to provide with the benefits set out in this Article during the period subsequent to discharge or suspension, whichever is applicable. In the event the employee’s discharge is upheld the employee will no longer be eligible to be part of the Company benefit plan. The Arbitrator, if the grievance is successful, shall be limited to reimbursement to the employee for welfare to the amounts paid by the employee for coverage maintenance during such aforementioned periods. Pension Plan The Pension Plan in existence at the signing of this Agreement shall apply during the term of this Agreement, subject to the terms and/or conditions of Provincial Federal legislation. Employee Benefit Plan (Policy #97472) as follows: The following outlines the premium cost sharing formula for each benefit under the Group Plan: Benefit Premium Cost Sharing Employer Employee Basic Life 80% 20% Basic AD&D 80% 20% Dependent Life 80% 20% LTD/Critical Illness 100% Extended Health 80% 20% Vision 80% 20% Dental 80% 20% Sick Leave (STD) 100% Employee Assistance Program 100% Business Travel Accident 100% The Health Benefit Plan covers employees if single coverage is selected and employee, spouse and children if family coverage is selected. If an employee has coverage through a spouse, dental, vision and health care benefits may be waived upon proof of coverage. The Company shall have the benefit provider issue Drug Cards to all employees which enables the employee to fill prescriptions under the benefit plan with payment of 10% for generics and 25% for name brand drugs. Drugs that do not have a generic product will be reimbursed at the full 90%. Medical and Group Insurance. Effective January 2017 employees will transition to the Employer’s Omniflex Benefits Plan. The benefit plan shall provide each employee with a choice of options available for single, couple and family coverage for medical, dental and insurance coverage as well as STD and LTD Disability Plans. Details of the various benefits shall be as discussed and presented to the Guild and its members in November 2016. The Employer also agrees that in the event there are changes to the benefit plan, the Employer will meet with the Guild to review and discuss such changes.

The Employer shall pay one hundred per cent (100%) of the total premium cost of the following during the term of this Agreement: Extended Health Care Group Life Insurance Accidental Death & Dismemberment Occupational Accidental Death & Dismemberment In the event that, in the future, the Employer is required to re-enter the Workers’ Compensation Board program, the Employer has the option of terminating the provisions relating to the said supplemental Occupational Accident Program. For the duration of this Agreement the Employer will continue to make the Long Term Disability Plan available to the employees, the cost of which shall be borne by the employee. Insurance or benefit plans in effect on or before January 1, 1989 will not be altered without prior approval of the Guild if the changes would result in any less favourable benefit. It is understood that any adjustment in shared premiums would continue to be shared by the employee and the Employer on a 50-50 basis. For purposes of this Agreement, Workers Compensation is not to be considered part of the group insurance or benefit plans. The Employer shall pay fifty per cent (50%) of the total cost of the premium required to include Eye Care in the Extended Health Care. This benefit shall contain a limitation with respect to reimbursement for contact lenses and further that eyeglass frames will be subject to a maximum reimbursement of $100.00 for each 24-month period. The Employer shall pay fifty per cent (50%) of the total premium cost of the Dental Plan. The parties agree that no premium increase will be implemented without informing the Guild and the Plan will remain in force during the term of this Agreement.
