Upon written authorization by a teacher, the Board of U.S.D. #350 will withhold the amount designated by the teacher from the monthly salary checks in accordance with applicable state laws. Within a reasonable time thereafter, the Board will write a check for the amount of the dues deducted and forward the check to the Treasurer of the St. Xxxx-Xxxxxx Teachers' Association.

Payroll Deductions for Association Dues. Payroll deductions for the payment of Association dues (WEA, Central OEA/NEA, OEA, and NEA) and UTP affiliated professional organization(s) dues shall be provided by the Board in keeping with the following: 57.1 A member who wishes to authorize payroll deduction shall submit a written authorization for payroll deductions on a form provided by WEA to the Board's Treasurer on or before November 1, of any year the member begins payroll deductions under this agreement. Unless revoked or changed in accordance with procedures contained herein, an authorization will continue from year to year. 57.2 By November 1 of each year, the Association will notify the Board's Treasurer as to the total amount of dues to be deducted per member. Such notification shall be in the form of a letter signed by the Association President or Treasurer. 57.3 Within fourteen (14) calendar days following completion of each deduction, the Board's Treasurer shall remit the amount which was deducted to the WEA Treasurer in check form made payable to "The Worthington Education Association." 57.4 Authorized payroll deductions will be made in twenty (20) equal installments beginning with the November 15 pay and continuing each month through the following August. For those members who elect to receive the balance of their pay July 15 the remaining deductions shall be taken at that time. 57.5 If a member ends his/her employment for reasons other than death of the member or takes a leave of absence before all twenty (20) installments have been deducted, the unpaid balance will be deducted from the final payroll check if the amount of the check is sufficient to cover the unpaid balance and remitted to the WEA Treasurer as provided below. 57.6 A member who wishes to add or delete membership dues deduction for any UTP affiliate organization(s) and where a deletion does not amount to revocation of the entire amount being deducted from dues at the time of the deletion, shall do so only during September of any given year. The Association shall inform members of these options and to process any changes on revised authorization cards. The Association shall submit all revised authorization cards to the Board's Treasurer by November 1 of each year. 57.7 A member who wishes to revoke his/her payroll deduction authorization may do so normally during September of any given year. Payroll Deductions for Association Dues. The College authorizes the deduction of Association membership dues, as established by the Association, from the salary of any employee covered by this Agreement who shall authorize such deduction in writing and upon the following conditions: a) The Association shall submit to the College’s payroll department a certified list and a copy of the written payroll deduction authorization of all members who request payroll deduction of dues no later than October 15 of each year. b) Said authorized deduction shall be made from the pay of each member beginning in November and ending in May of each year. c) On or before the twentieth day of the month following the month for which deductions are made, the aggregate deductions shall be remitted to the treasurer of the Association. d) Each member's written authorization shall be valid until revoked in writing by said member. Written revocations will not be effective until the entire amount of annual dues of the Association (and its affiliated entities) have been paid in full. e) The Association shall be solely responsible in the event of any claims by employees that the deductions and/or remissions were improper.

The deductions will commence with the first paycheck in October and will be evenly deducted from twenty-two (22) paychecks (October through August). Payroll Deductions for Association Dues. The City shall deduct monthly dues required of the employees in the Association who voluntarily execute a wage assignment authorization form. The City shall deposit such dues with the Association. Upon issuance and transmission of such fees the City’s responsibility shall cease with respect to such deductions. The Association and each employee authorizing the assignment of wages for payment of fees hereby undertake to indemnify and hold the City harmless from all claims, demands, suits and other forms of liability that may arise against the City for or on account of any deduction made from the wages of such employee.

Payroll Deductions for Association Dues. Any bargaining unit member who has applied for membership in the Association may sign and deliver to the Board an authorization for continual dues deduction, the amount of which shall be annually certified by the Association. The appropriate authorization forms will be provided by the Association. A continual authorization shall remain in effect unless the employee revokes said authorization in writing between September 1 and September 15 of any year. Should an employee leave the District or drop his/her membership, the Board shall deduct the prorated balance of the year's dues from the next available paycheck. Should the final paycheck not contain sufficient funds, the Board shall deduct only the amount available as allowed under state and/or federal law. With the dues deduction authorization, the Board shall deduct such dues from the regular paycheck of the employee on a biweekly basis over the number of checks each employee is paid during the school year. The Board shall remit deducted dues to the Association within ten days following the pay period. Members may also elect to pay a cash payment to the Association.

The Board will provide a payroll deduction for Local, District, State and National Association dues, provided the employee has signed the appropriate payroll deduction authorization form. The Association agrees to indemnify and hold the Board harmless for any claims, actions, demands, suits, damages, awards, fines and court costs by any employee(s) recognized by the Association, arising from deductions made by the Employer pursuant to this Article. Any missed deductions will be made up on the next scheduled deduction providing said deduction is made up within thirty-one (31) days. All deductions made from members of the bargaining unit will be forwarded to the Local Association Treasurer each pay period by check by the Local School District Treasurer. 2. Dues deduction authorization for the employees covered by this Agreement shall not be afforded any other labor organization as long as the Association remains the collective bargaining representative.
Payroll Deductions for Association Dues. 5.1 The Employer agrees to deduct from the salaries of technical employees, dues for the Michigan Nurses Association, and MAST dues if applicable, when authorized in writing by each technical employee. 5.2 Dues shall be deducted together in twelve monthly installments. Appropriate notice shall be given for a technical employee who is on leave or layoff or for whom, for other reasons, no deduction is made in a given payroll period. 5.3 The Association will notify the Hospital in writing of the proper dues to be deducted. Written notification shall be made not less than thirty (30) days prior to the implementation of any change in the dues withholding rate. Such notice shall not be made more than four (4) times in any calendar year. 5.4 All dues so deducted shall be sent to the Michigan Nurses Association at its office at 0000 Xxxxx Xxx Xxxx, Xxxxxx, Xxxxxxxx, or to such address as the Association shall direct, promptly under procedures to be established by the Employer. The Association shall be responsible for disbursements of dues received by it to the treasurers of the various associations. 5.5 The Association shall refund to the technical employee dues erroneously deducted by the Hospital and paid to the Association. The Association agrees to indemnify and save the Hospital harmless against any and all claims, suits and other forms of liability that may arise out of or by reason of action taken in reliance upon individual authorization cards or by reason of the Hospital's agreement to participate in a payroll deduction of Association dues. 5.6 The Hospital shall honor and utilize lawful payroll deduction forms submitted by the Association. A copy of the current form is attached as Appendix (A).
