Required Personnel. A. The Employer will identify the services required during suspended operations and notify employees required to work in accordance with the Employer's suspended operations procedures. Personnel required to work during suspended operations will be identified by the employer dependent upon the situation and will be notified as far in advance as is reasonable and practical. Upon request, the Human Resource Services will make the suspended operations written procedures available to an employee.
B. Employees required to work during suspended operations will have no loss in pay for the first day of suspended operations, and will receive penalty pay of one-half (1/2) times their regular pay and additional paid leave (referred to as “suspended operations leave”) on an hour for hour basis for work performed during the first day of suspended operations. Suspended operations leave earned under this section:
1. May be used alone or conjunction with other leave;
2. Has no cash value;
3. Must be requested and scheduled in accordance with Article 11, Vacation Leave;
4. May not be donated as shared leave; and
5. Must be used prior to using compensatory time and vacation leave, unless this would result in the loss of the employee’s vacation leave.
C. After the first day of suspended operations, employees required to work during suspended operations will receive one and one-half (1-1/2) times their regular pay for work performed during the remaining period of suspended operations.
D. Employees not receiving callback, who are required to work during suspended operations will receive a minimum of two (2) hours of pay for each day worked.
E. Any overtime worked during suspended operations will be compensated according to Article 8, Overtime.
Required Personnel. Each proposal/grant is required to have, at a minimum, an authorizing official, a project director and a fiscal officer. See page 7 for a description of each. This information must be kept current.

Non-required personnel are expected to work remotely or at a facility/location 1838 within a reasonable commuting distance from the non-operational location during 1839 suspended operation events. Non- required personnel will not report to the worksite 1840 when operations have been suspended. Supervisors are responsible to ensure that 1841 remote work is available to non-required personnel. 1842 B. If non-required personnel are unavailable to work during suspended operations, the 1843 following options will be made available :
1844 1. Vacation leave; 1845
1846 2. Personal holiday; 1847

Requirements for personnel for the Project are set forth in Exhibit B, including the following: (1) required titles of personnel, (2) minimum requirements per title, (3) estimated hours per title, and (4) Maximum Allowable Direct Salary Rate per hour per title. If the commencement date (i.e., the date set forth in the Notice to Proceed) is more than twelve (12) months after the date on which the Engineer submitted its Fee Proposal for the Contract, the Commissioner may, by written directive, increase the Maximum Allowable Direct Salary Rates per hour set forth in Exhibit B.

Required Personnel. Licensee must have a designated representative, with the Authority to act on behalf of the Licensee, to be present at all times indicated on the Event Schedule Form. B. Phone: Licensee acknowledges and agrees that (i) it shall be required to provide, at its own cost and expense, all necessary ticket staff, ushers, stage manager, and crew, and (ii) Licensor will provide, at Licensee’s cost and expense, an on-site supervisor to be present at all performances.
Required Personnel. Each claiming unit must designate a claiming unit employee as the claiming unit’s coordinator or MAA program contact. This single individual is designated within the claiming unit to provide oversight for the implementation of the time study and to ensure that policy decisions are implemented appropriately. The claiming unit coordinator cannot be affiliated with or employed by a consultant/consulting firm or vendor.
Required Personnel. The Contractor shall provide necessary personnel and equipment to fulfill the requirements and specifications of the services described in this contract. Contractor shall include the cost for providing a supervisor and customer service representatives to address all problems as they arise, interface directly with customers, and have direct communication with City staff, the Contractor customer service staff, and route drivers. The supervisor shall meet or communicate with City staff as necessary.
Required Personnel. A. In addition to any other requirements contained herein, no service may utilize a driver to operate a unit unless:
1. He/she is familiar with the geographic area of the organization’s service area;
2. He/she has adequate knowledge of all the hospitals with the organization’s service area;
3. He/she can operate a two-way radio; and
4. He/she can drive a unit under emergency conditions.
B. Each service or agency must provide written notice to the Local EMS Agency within ten (10) days of any change in personnel or units unless an alternate plan has been approved by the Local EMS Agency.
C. Services may utilize only those drivers and attendants who are appropriately licensed, certified and accredited.
D. Every advanced life support transport unit when in operation must be staffed, at a minimum, by two personnel, one of whom must be locally accredited as an EMT-P and one of whom must be certified as an EMT-IA, unless exempt.
E. Every limited advanced life support transport unit when in operation must be staffed, at a minimum, by two personnel, one of whom must be locally certified at the limited advanced life support level and one of whom must be certified as an EMT-IA, unless exempt.
F. Every Basic Life Support transport unit, when in operation must be staffed, at a minimum, by two personnel, both of whom must be certified as an EMT-IA, unless granted a waiver from the California Highway Patrol (Title 13, California Code of Regulations, Section 1108 and California Vehicle Code Section 2512).
G. An agency vehicle endorsed at the LALS level must have at least one individual locally certified as an EMT-II, or if endorsed at the ALS level, one locally accredited EMT-P.
H. All providers are responsible to implement reasonable employee shift schedules to avoid problems of sleep deprivation, fatigue, and incompetent performance.
I. Services must insure that their attendants are free of physical defects or diseases which may impair their ability to attend to patients. No person, service or agency shall be out of compliance with Government Code, Section 8355, in matters relating to providing a drug-free work place.
J. Drivers and attendants utilized on medical transport vehicles shall be at least eighteen (18) years of age, shall hold a current certificate in level C basic cardiac life support from the American Heart Association/American Red Cross, and shall demonstrate compliance with all applicable State and local laws and regulations.
