Donation Process. The Pool shall consist of vacation hours donated voluntarily by employees. Bargaining unit members may donate vacation hours to the Pool at any time. They may not donate more vacation hours than they accrue during the calendar year. Donations to the Pool shall be converted to a dollar amount based upon the donor’s salary rate. Once hours are donated, they may not be returned to the donating employee.
Donation Process. DCPS considers any in-kind services provided by Provider under this Agreement valuable to its authorized duties and responsibilities as a District of Columbia government agency. Where DCPS is not paying the services provided under this Agreement pursuant to some other agreement or purchase order as noted in Section 2 of this Agreement, then in accordance with D.C. Official Code § 1-329.01 and the District of Columbia Mayor’s Memorandum #2015-001 (July 21, 2015), all such services must be donated to DCPS through the donation review process administered by Serve DC – The District of Columbia Mayor’s Office on Volunteerism and Partnerships.
Donation Process. 13.21.7.1 A written Donation of Sick Leave form must be submitted and signed by the donor. (See appendix J) 13.21.7.2 Donation forms should be submitted to the Payroll Services Office.

Donations to the 14 recipient employee’s sick leave bank shall be at his/her hourly rate. Donated sick leave 15 may not be cashed out.
Donation Process. 16 If the transfer of leave credits is approved, any part-time faculty may donate eligible leave credits at a 17 minimum of one hour increments thereafter, to a maximum of 16 hours per catastrophic illness. Requests 18 for sick leave credits will be addressed on an individual basis. Human Resources will notify part-time faculty 19 by District email of an eligible candidates request for donation. 20 • A donating part-time employee must retain no fewer than 9 hours of sick leave on record to be 21 eligible to donate sick leave hours. 22 • All transfers of eligible leave credits are irrevocable. Part-time faculty who are donating sick leave 23 must complete the Part-Time Faculty Catastrophic Leave Donation Form” available at Human 24 Resources. 25 • IMPORTANT: Sick leave accrual is a component of the CalSTRS/PERS retirement pension calculation.

Donation Process. 22.4.1 Donations will be solicited by the County Office making an announcement on behalf of the employee who meets the requirements for this benefit.
22.4.2 Those employees donating eligible leave credits must submit a signed donation form to the Human Resource Department.
22.4.3 A list of eligible donors, who request to contribute eligible leave credits to the employee, will be established by the Human Resources Department.
22.4.4 In order to contribute, an employee, after making the donation, must still have a minimum of five accumulated sick days.
22.4.5 The minimum amount an employee may donate is one day of sick leave, which shall be deemed to equate to the legal minimum required by Education Code section 44043.5. One full day of donated sick leave shall provide one full day of catastrophic leave credit to the receiving employee. The maximum contribution by any employee shall be 10 days.
22.4.6 Once a commitment to donate eligible leave credits has been made by an employee, that commitment cannot be revoked.
22.4.7 Catastrophic leave credit shall be used in the order donations are received.
22.4.8 All donations are to remain confidential from other employees and the recipient.
Donation Process. A. The employee requesting donated leave may solicit sick leave from other employees for his/her own personal illness by adhering to the following procedures:
Donation Process. 6.12.1 Any employee may donate eligible leave credits to an Individual Catastrophic Leave request by completing a donation form, indicating the amount of sick leave, vacation leave, or personal floating holiday or any combination thereof totaling no less than four (4) hours. The donation is irrevocable and no portion will be returned. Individual donations will be calculated to a monetary value using the current, hourly rate of pay, of the employee who made the donation. The individual whom the donation was made for will be paid at their current hourly rate. Only employees covered under this Agreement are eligible to donate to the Bank directly.
6.12.2 The employee shall forward the donation form to Human Resources to verify the employee has the minimum accrued leave as defined in Article 6.11.
6.12.3 If the employee meets the eligibility requirement, Human Resources shall transfer the amount shown on the donation form to the Bank with a notification to the CCCUE President. A copy of the form will be placed in the employee’s personnel file. Each hour donated will be transferred to a monetary calculation. The monetary calculation will use the mid-range on the Classified Salary Schedule, current Range 30, Step C of the Classified Salary Schedule. In the event that the Classified Salary Schedule should change, the mid-range will be Step C of the middle range. Employees will be paid at their actual monthly pay rate.
6.12.4 If the employee does not meet the eligibility requirement, Human Resources shall return the form to the employee within ten (10) working days, providing the specific reason why the unit member did not qualify to make the donation.
6.12.5 The donating employee must have a minimum balance of 40 hours in either sick leave, vacation leave, or any combination thereof and donate a minimum of four (4) hours of sick or vacation, or a combination thereof each year. If donating floating holiday hours, then the donation must be the equivalent of a full work day. This applies the same to full-time as well as part-time permanent employees.
6.12.6 Eligible employees that have not donated to the Bank will still have the right to ask for and utilize individual leave credit donated per the process outlined in 6.11 of this Agreement. However, upon exhaustion of these “personal” credits, the employee will not be eligible to draw upon the Bank.
