HEALTH, WELFARE & PENSION. (a) The provisions of Articles 12 (Health Plan), 13 (Pension Plan), 13A (Individual Account Plan), 14 (Retiree Health Plan), and 22 (Contract Services Administration Trust Fund) of the 2015 Producer and Studio Transportation Drivers, Teamsters Local #399 Location Managers Agreement (“LMA”) and any successor agreement shall be applicable to employees hired by the Employer to perform services in the County of Los Angeles, or hired by the Employer in the County of Los Angeles to perform services outside the County of Los Angeles. Eight (8) hours of benefit contributions for travel days and idle days on overnight locations shall be made as provided in the LMA. (b) The Employer will execute any documents required to constitute it an appropriate Employer contributor to the benefit plans specified in the LMA. (c) During the term of this Agreement, the Pension and Health contribution rates will match those now in effect and as may later be modified in the Agreement between the AICP and the IATSE. All changes in the rates of contributions pursuant to this paragraph shall be effective on the Sunday closest to the next anniversary date of this Agreement.

HEALTH, WELFARE & PENSION. ‌ 21.01 As a condition of employment, each regular full-time employee shall enroll, subscribe to and participate in: (a) the Group Insurance Plan, including life insurance equal to the lesser of three times the employee’s annual salary or the maximum set by the Township’s insurer (not more than $140,000.00), weekly indemnity, long term disability (to a maximum of 66 2/3% of monthly earnings or $2,500.00, beginning after 17 weeks of disability and ending at age 65 (or on recovery or death), dental benefits, vision care ($300.00/24 months), major medical and supplementary hospital benefits; (b) the Ontario Municipal Employees Retirement System. Part-time employees who become eligible for OMERS shall be entitled to be enrolled upon request. (c) In the event that any of the foregoing hospital and medical plans for any reason, become unavailable or are discontinued, the Corporation shall continue to contribute towards the premium for any hospital or medical plans substituted therefore, sums of money equivalent to those being paid at the date of such availability or discontinuance. 21.02 The Corporation shall contribute 100% of the premiums for the Group Insurance Plan except for the weekly indemnity plan, the premiums for which shall be paid 100% by the employee. (a) Where an employee is enrolled in the Township’s group insurance plan and, due to sickness or injury, qualifies for sick leave, weekly indemnity, long term disability or Workers’ Compensation, the employee shall continue to be enrolled in the group insurance plan for a period of up to two (2) years from the date the employee ceases work. During this period, the municipality shall continue to pay its share of the employee’s group insurance premiums as specified in the collective agreement. (b) If the employee is still absent from work at the end of two years he/she may elect to remain enrolled in the group insurance plan for one additional year provided that he/she pays all insurance premiums for this third year. The employee shall notify the Corporation, within thirty (30) days of receiving notification from the employer in (c) below, that he/she wishes to continue to remain in the plan for this third year. (c) If the employee’s absence from work continues beyond the periods in (a) or (b), whichever is applicable, the employee’s benefits will be terminated. HEALTH, WELFARE & PENSION. 14.1 The Employer agrees to pay the premiums for the group insurance benefits, short-term disability benefits, vision care in effect as of May 1, 2002. Effective, as per the terms and conditions ratified February 25th, 2007 the following changes will be implemented: • 90% - 10% co-pay on Health Care and Dental CarePrescription Drug cap - $3,500.00 per year • Dental Care cap - $2,500.00 per year • Vision Care - The Company will provide up to $200.00 every two years per family members, as described by the Benefit Carrier, for prescription eyewear (this does not affect current safety prescription glasses).

HEALTH, WELFARE & PENSION. 19.01 The Employer agrees to contribute as shown below on behalf of those non probationary employees in the active employ of the Employer, into Local 183 Industrial Benefit Fund, jointly administered by an equal number of Employer and Union Trustees, for the purpose of purchasing PLAN “A”, life insurance, major medical or dental insurance for the employees covered by this Collective Agreement, represented by Local 000, Xxxxxxxxx' Xxxxxxxxxxxxx Xxxxx xx Xxxxx Xxxxxxx. Effective January 1, 2022, - Two hundred and sixty dollars ($260.00) per month, per employee, plus all applicable taxes. Effective January 1, 2023. Two hundred and seventy ($270.00) per month, per employee, plus all applicable taxes. Effective January 1, 2024. Two hundred and eighty dollars ($280.00) per month, per employee, plus all applicable taxes. Employees will contribute into the Industrial Benefit Plan as follows: Co-payment January 1, 2022 - $20.00 Co-payment January 1, 2023 - $15.00 Co-payment January 1, 2024 - $10.00 19.02 The Employer shall contribute to the Labourers' Pension Fund of Central and Eastern Canada, the following amounts for each hour worked by each employee per month: 19.03 New employees hired after the date of ratification will have to complete their probation in order to qualify for benefits and pension. 19.04 The Employer shall contribute for each employee into the Local 183 Retiree Benefit Trust Fund for the purpose of purchasing benefits as contemplated by the Agreement and Trust establishing the said Retiree Benefit Fund as follows: $ 0.05/hr These contributions shall be remitted on the Employer's Report Form, by the fifteenth (15th) day of the month following the month to the said Retiree Benefit Fund.
HEALTH, WELFARE & PENSION. 23.1 The Company agrees to provide Health & Welfare coverage, for the duration of this Agreement, to qualify for Health & Welfare benefits an employee must work a minimum of twelve (12) days per calendar month. 23.2 One hundred percent (100%) coverage for Health & Welfare and no short term disability for all employees after probationary period is completed. All new hires will have Health & Welfare coverage 80-20%, with no short term disability, one hundred eighty (180) days waiting period, there will be nine (9) months waiting period for dental coverage. 23.3 Three percent (3%) of gross, men to match Company contribution.

HEALTH, WELFARE & PENSION. Except as changed through local negotiations, the Company shall continue to participate in all Health, Welfare and Pension (and/or other Retirement) Plans in accordance with the rules and regulations of each Fund. The Company shall pay annual contribution increases above the rates in effect at the expiration of the prior agreement in the following amounts: $1.25 per hour for year one, $1.00 year per hour for year two, $1.00 per hour for year three to be allocated between the applicable Health, Welfare and Pension (and/or other Retirement) Funds. Contributions to such plans for eligible employees shall be based on monthly, weekly, daily and/or hourly contribution formulas in accordance with plan documents and past practice. In making such allocation, the Union and the Company agree that a sufficient portion of the increase shall be allocated to fully cover additional contributions required by reason of the Pension Protection Act. If for any reason the increase per hour is insufficient to cover additional contributions required for a given pension plan in a given year , then the Union and the Employer shall promptly meet to negotiate changes in this Agreement to generate sufficient savings to cover the cost of such increased contributions. Agreement shall not be unreasonably withheld. Such negotiation shall be treated as an Emergency Reopening under Article 27. If such negotiations result in a revised schedule of benefits, the applicable Fund shall be obligated to accept the schedule as if it was the beginning of the term of a new labor agreement.

The Employer will execute the Standard Trust Agreement provided to all other Employers in the Plan. The Union will supply a benefit booklet to each employee. September 1, 2011 - $175.00 per member (employee) per month. September 1, 2012 - $197.75 per member (employee) per month. September 1, 2013 - $223.46 per member (employee) per month.
HEALTH, WELFARE & PENSION. 15.1 O.H.I.P. - Should the Government reinstate O.H.I.P. or a similar user-fee plan, the Company will pay the full premium costs, plus applicable taxes.
