Health & Wellness. A. Employees who actively participate in the Employer-sponsored wellness program shall receive the same benefits/incentives that are offered to any other participants.
B. Employees who attend a gym/workout facility at least four (4) times per calendar month (the Employer may request/require proof of attendance) will be reimbursed up to $35.00 per month for gym/workout facility costs.
C. The bargaining unit has the right to representation on the joint wellness committee equal to that of any other participating bargaining unit or employee group.
Health & Wellness. To promote better health among employees and help control future health care costs, the City shall provide $100 per month for Health & Wellness to employees who participate in annual wellness checks and/or wellness program.
Health & Wellness. The Board is committed to providing a school environment that promotes and protects the health and well-being of its students. The Board encourages all members of the school community to create an environment that supports lifelong habits of health and wellness. The Board will review all health and wellness policies annually and make a good faith effort to continue to maintain the health and wellness of all students and employees.
Health & Wellness. 10.1 If an Employee is hired on with the Employer and they come from a Building Trades of Alberta affiliated Union they may choose to stay in their affiliated Unions Health and Welfare Plan if allowed by said Union or join the Prairie Teamsters Health and Welfare Plan. The Employer agrees to cover all full time Employees under the above stated Health and Welfare Plans at no cost to the employee. All remittances will be paid retroactively upon completion of the probationary period. The Employer agrees to remit the minimum hourly contribution required to ensure coverage and maintain benefits for the Employees. Should an Employee be hired from a workplace that is not a Building Trades of Alberta affiliate that Employee will be added to the Prairie Teamsters Health and Welfare Plan. In addition any employee that is from a Building Trades of Alberta affiliate may elect to participate in the Prairie Teamsters Health and Welfare Plan. They may do so at any time. The Employee must indicate this choice to the Company in writing with a copy sent to the Local Union Office. Once an employee has elected to participate in the Prairie Teamsters Health and Welfare Plan they cannot revert back to the plan they were previously on. Below outlines the Prairie Teamsters Health and Welfare Plan to all employees who have opted into the plan and/or are to receive the plan.
(a) Any regular employee or member of the Union who has opted into the plan and/or is hired by the Company after the effective date of the Health and Welfare Plan, shall join the Plan on the first day of the month immediately following ninety (90) calendar days from the date of employment with the Company.
(b) It will be the responsibility of the Company to ensure that all employees who have opted into the plan are enrolled in the Health and Welfare Plan, and for making premium remittances on their behalf. Failure by the Company to enroll employees, forward completed forms and/or remit premiums on the due date, being the tenth (10th) of each month, to the Trustees will cause the Company to be liable for any claims arising thereof.
(c) It shall be the Union's responsibility to supply all necessary enrollment forms to the Company.
(d) The Company shall remit the premiums to the Administrator, as designated by the Trustees of the Health and Welfare Plan. It shall be the Trustees' responsibility after receipt of the premiums to distribute same to applicable insurance underwriters.

Health & Wellness. Both parties will agree to a Physical Ability Test that would be determine representatives of the Township and representative(s) of Local 3906. If through the Ability Test it’s determined an employee needs medical evaluation the employee shall seek direction from their personal Physician. This would be achieved through the employee’s medical insurance provided by Township. Any overages in cost due to physical examination would be covered by Township. It would be the initiative of both parties to encourage and promote a health and wellness program. This program would encourage the employees to receive physicals and dietary awareness education. As well as weight management and overall health management.
Health & Wellness. 26.01 The Union and Employer agree that health and wellness of Employees is important. It is agreed that Employees are expected to be fit for work and maintain their fitness and wellness for work.
26.02 The IAFF/IAFC Wellness Fitness Initiative (WFI) shall provide guidance on fitness and wellness.
Health & Wellness. Unit employees shall be eligible for reimbursement of up to three hundred dollars ($300) per calendar year for a health club membership, durable fitness equipment, smoking cessation programs, weight loss programs, insurance deductibles, office visit co-payments, prescriptions and any other medical expenses that are recognized as being tax deductible by the IRS; (to include dental, vision, and durable goods) upon proof of payment.
Health & Wellness. Paragraph 1: The Board of Education provides up to $100 per school year for health club memberships with cooperating businesses.
Health & Wellness. 12.1 The Company agrees to obtain from an insurance carrier legally authorized to operate in the Province of Alberta and Alberta Health Care Insurance Commission (herein after referred to as ‘Benefit Carriers’), group insurance protection for each employee covered by this agreement which will provide each employee with “Employee Benefits”
12.2 The benefits provided for in this Article shall be paid for on the following basis: The Employer shall pay one hundred percent (100%) of the premium costs of the benefits for each Employee and their dependents.
12.3 Should there be any indication that the current Benefit Plan will require a benefit adjustment, benefit changes or become endangered within the term of this Agreement the Employer and the Union mutually agree to meet and review said plan. The Employer will ensure an adequate benefit plan is in place.
Health & Wellness. The performance of risk assessment and risk management services via biometric screenings, health risk assessments and onsite wellness coaching for health and care management companies, including wellness companies, disease management organizations, clinical research organizations and health plans (but expressly not including exam services). The information collected from such services is used by such customers, among other things, to measure the populations they manage, to identify risks in those populations, to target interventional programs, to promote positive lifestyle behaviors and to measure the results of their health and care management programs, but not in connection with underwriting insurance. Such health & wellness services include: • scheduling of individual and group screenings; • provision and fulfillment of needed supplies (e.g., assessment kits, blood pressure cuffs, stadiometers, scales, centrifuges, etc.) at screening events; • end-to-end biometric screening management; • non-insurance biometric screenings (e.g., height, weight, body mass index, the taking of a person’s hip, waist and neck measurements, as well as his or her pulse and blood pressure) and blood draws via venipuncture or fingerstick, all performed by our wellness certified health professionals, but not in connection with underwriting insurance; • lab testing of blood specimens utilizing our Heritage Labs laboratory; • data processing and transmission; and • analytics to identify critical values of lab tests and notification services to individuals and customers to better manage risk. Health & Wellness also provides Xxxxxx Xxxxxx OnSitesm, a face-to-face, on-premises health coaching service for work locations. The service is delivered by specially trained "Health Champions", many of whom are physical therapists, personal trainers and nutritionists.
