SERVICE BENEFITS. Benefits provided by this Agreement ( but only to the extent they are not hereafter excluded) are for the necessary treatment of any Sickness or Accidental Injury as follows: A. BASIC BENEFITS 1. Blue Shield will pay the following: a) Coverage of Part A Medicare Eligible Expenses for hospitalization to the extent not covered by Medicare from the 61st day through the 90th day in any Medicare Benefit Period; b) Coverage of Part A Medicare Eligible Expenses incurred for hospitalization to the extent not covered by Medicare for each Medicare lifetime inpatient reserve day used. Each Medicare beneficiary is given sixty (60) lifetime reserve days which begin from the 91st day and after; c) Upon exhaustion of the Medicare hospital inpatient coverage including the sixty (60) lifetime reserve days, coverage for the Medicare Part A Eligible Expenses for hospitalization will be paid at the appropriate standard of payment which has been approved by Medicare, subject to a lifetime maximum benefit of an additional 365 days (except that psychiatric care in a psychiatric hospital participating in the Medicare program is limited to 190 days during the Subscriber's lifetime); d) Room and board charges shall be no more than the charge for a semi-private accommodation in the Hospital of confinement, unless confinement in a subacute skilled nursing facility or private room is certified as medically necessary by an attending Physician. 2. Blue Shield will pay the following: a) Coverage for Medicare Parts A and B for the reasonable cost of the first three (3) pints of blood (or equivalent quantities of packed red blood cells, as defined under federal regulations) unless replaced in accordance with federal regulations; b) Blue Shield will provide coverage for the coinsurance amount or, in the case of hospital outpatient Services, the copayment amount of Medicare Eligible Expenses under Part B regardless of hospital confinement, subject to the Medicare Part B Deductible provided the Subscriber is receiving concurrent benefits from Medicare for the same Services.‌ c) Blue Shield will provide coverage for hospice care which includes cost sharing for all Part A Medicare eligible hospice care and respite care expenses. 3. Benefits for the coverage listed above shall be paid when the Subscriber is not entitled to payment for such Services under Medicare by reason of exhaustion of Medicare Benefits or reductions for coinsurance and Deductibles required under Medicare. SERVICE BENEFITS. Length of service is recognized through increasing benefit entitlements for certain benefit programs, discretionary reduction of probationary period for transferred or promoted employees, and in the review and scrutiny given to applications for promotion or transfer.

SERVICE BENEFITS. It will build upon the current joint equipment service to provide a seamless service across Health and Social Care and will set a benchmark for what can be achieved through integration of resources and finance. • It will provide the potential for a seven day service. • It will have a Joint Management Board to oversee the Operational Services. • It will have a single operational manager who will be accountable on a day to day basis for the running of the Service. • By integrating budgets, costs for ordering, storing, maintenance and delivery can be reduced with further cost savings being achieved by creating a robust recycling facility within the Service. • It will give Service Users a single point of contact and a facility to see for themselves the range of Community Equipment available. • Creating a 'core stock' of Community Equipment will ensure quick access to equipment needed at short notice, thereby facilitating more efficient hospital discharge. • Having a stock of Community Equipment for 'Continuing Care' will ensure quick access to items for those Service Users with more complex health needs. • The Service will have an up-to-date state of the art cleaning and sterilising unit to ensure all Community Equipment issued and re-issued meet current health and safety standards and decontamination requirements.

SERVICE BENEFITS. An employee with six months or more of continuous service with the Company having entered a branch of the Armed Forces of Canada since June will be granted military service benefits in the amount and under the conditions specified below: The Local Management must receive proof, on the Company form provided, of the employee’s entrance into a branch of the Armed Forces of Canada. The employee must be on active duty when the form is filed and when payment of bonuses is made. Bonuses will not be paid to members of reserve components when on annual active duty. An Air Forces reservist who is required to spend 5 to 7 days taking physical and other examinations prior to recall for active duty will be entitled to pay allowances as follows: The Company will pay for each day so spent the difference between his straight time pay for the hours he would have worked and his military pay per day (including longevity pay and special increments such as flight pay, but excluding such allowances as rent, subsistence, uniform and travel). This will not apply to any day on which the employee would not have otherwise worked. Payment will be made when the employee returns to work upon presentation of satisfactory proof of the amount of military pay received. A written statement on the Company form provided signed by the Finance or Disbursing Officer of the field to which the employee was sent is acceptable as proof. This statement should specify separately the amount of base pay the employee received and the amounts of various allowances, if any. If any other employee is required by the government to report for examination during working hours in order that his physical fitness for military service may be determined, he will be entitled to time off with pay for the time required at his regular straight time hourly rate.
