Voluntary Part-time Benefits. (a) The Employer agrees to provide regular part-time nurses who regularly work a minimum of fifteen (15) hours per pay period, the opportunity of voluntary participation in group health and welfare benefit programs set out in Article 17, limited to any and all of the following: extended health, semi private hospitalization, dental and voluntary life insurance. Life Insurance is restricted to an individual voluntary flat life insurance plan of ten thousand dollars ($10,000). It is understood and agreed that the regular part-time nurses who participate will assume one hundred percent (100%) of the monthly premiums.
(b) Any regular part time nurse who participates will provide payment of the premiums through the payroll deduction process. If the regular part-time nurse has no earnings, or insufficient earnings, in the pay period in which the premiums are normally deducted, the amount owing will be deducted from the subsequent pay. In the event that there are still insufficient earnings in the subsequent pay to cover 100% of the premiums, the nurse will provide the Hospital with a cheque for the required amount. Failure to provide full payment within three (3) pay periods in accordance with this provision may result in discontinuation of benefits.
(c) If a regular part time nurse’s coverage under a spouse’s or partners health and dental coverage terminates, the regular part time nurse is eligible for coverage under CHEO’s group benefit plan provided that he or she applies within thirty-one (31) days of the date that the coverage ended. The nurse shall provide evidence to CHEO of the termination of coverage with her application to enrol.
(d) If a regular part-time nurse wishes to apply for coverage subsequent to the thirty one (31) day period in (c), she/he will be required to provide satisfactory evidence of insurability and to be qualified in accordance with that evidence.

The Employer agrees to provide part-time nurses with the option of voluntary participation in any and all of the group health and welfare benefit programs set out in Article 17. It is understood and agreed that the part-time nurses who participate will assume the full monthly premiums. Any part-time nurse who wishes to participate will provide payment of the benefits either through post-dated cheques provided on a yearly basis or through a preauthorized withdrawal process. Part-time nurses will be eligible for enrolment in benefit plans within thirty (30) days of hiring, status change and circumstance changes as per our benefit carrier agreements. Enrolment for current part-time staff will occur within thirty (30) days of ratification of this agreement. It is understood that any transaction would be dated the first of each and every month.

(a) The Health Centre agrees to provide regular permanent part-time nurses with the option of voluntary participation in any and all of the group health and welfare benefit programs set out in Article 17 (Semi private, extended health, dental). It is understood and agreed that the part-time nurses who participate will assume the full cost of monthly premiums.
(b) Any regular permanent part-time nurse who wishes to participate will provide payment of the benefit premiums through payroll deduction.
(c) Such nurses may elect to opt in or out of the Extended Health Care and Dental plans on an annual basis or at time of hire.
(d) The Health Centre will notify the Union each time the benefit costs are renegotiated by the Health Centre.

(a) The Hospital agrees to provide regular part-time nurses with the option of participating in the group health and welfare benefit programs set out in Article 17 of the central agreement, with the exception of Group Life Insurance, Group AD&D and Long Term Disability, subject to the terms and conditions of the plan and any enrolment requirements. It is understood and agreed that participating regular part-time nurses will pay the full amount (100%) of the monthly premiums, plus applicable taxes.
(b) Part-time nurses must regularly work a minimum of twenty-two and one half (22 1/2) hours per pay period to be eligible to enrol in the health and welfare benefits.
(c) Current regular part-time nurses may enrol, without evidence of insurability, by no later than June 30, 2006. If a current regular part-time nurse elects to enrol subsequent to that date, she/he will be subject to the terms and conditions of the benefit plans for late enrolees.
(d) Newly hired part time nurses, subsequent to the date referenced in (c), will be eligible to enrol subject to normal enrolment provisions.

It is understood and agreed that participating nurses will pay the full amount of monthly premiums.
(b) Part-Time nurses who retire and have not yet reached age 65 may elect to continue their participation in group health and welfare benefit programs.
(c) Actively employed nurses will pay their monthly premiums through the payroll deduction process, unless on an authorized leave of absence, in which case payment is to be made through post-dated cheques. Post-dated cheques are to be dated the first day of the month, and received by the Hospital prior to the commencement of the leave.
(d) The Hospital will notify the Union of the benefits costs to part-time nurses in January of each year and each time the benefits costs are renegotiated by the Hospital.
Voluntary Part-time Benefits. If the local parties agree, the Hospital will provide part-time employees with the option of voluntary participation in any and all of the group health and welfare benefit programs set out in Article 18.01. It is understood and agreed that the part-time employees would pay the Employer the full amount of the monthly premiums, in advance. NOTE: Part-time voluntary benefits are not arbitrable in local negotiations. LETTER OF UNDERSTANDING CUPE and the Participating Hospitals agree to establish a provincial working group consisting of up to three representatives each, to discuss the issue of RPN rates across the province, and the feasibility of moving towards a provincial or common wage rate. The working group will have access to expertise and resources as appropriate. The working group will commence meeting sixty (60) days following ratification of the collective agreement. CUPE members will be granted such time off as is required to attend joint meetings of the working group. The time spent by the CUPE members to attend joint meetings of the working group will be deemed time worked and CUPE members will be compensated at their regular straight time hourly rate. The working group will arrange its activities in order to endeavour to arrive at joint recommendations for the central parties four (4) months prior to the expiry of the collective agreement.
