Sick Leave Assistance Program. 1. A Sick Leave Assistance Program shall be established which contains days contributed by a certified teacher of District 41-4 represented by the LEA All certified teachers working more than twenty (20) hours per week and who contribute to the assistance program shall be eligible to request additional sick leave. 2. To be eligible to participate in the program, contributions of one (1) to five (5) days must be given. a. A personnel form will be filled out at the beginning of each year to indicate the number of days you are contributing that year (including 0) and returned to the business manager by September 10. b. New certified teachers in the system represented by the LEA may join within c. If needed the LEA may request more days to be contributed to restore any or all of those used. 3. In the event of a prolonged illness of any certified teacher who exhausts his/her sick leave, the certified teacher may request days from the bank not to exceed forty (40) days in one (1) school year. Unused days granted under the sick leave assistance program in a given year shall revert back to the sick leave assistance program. 4. The Sick Leave Assistance Program shall not be used to care for members of the teacher's family or for maternity leave. 5. The application for sick leave shall be submitted to the business office which will forward copies to the sick leave assistance committee. Application for sick leave assistance shall be reviewed by the LEA Sick Leave Program Committee composed of three (3) members appointed by the LEA. The LEA Committee must have a written statement from the applicant's doctor stating the medical need and the date the certified teacher may return to work. The committee shall meet to consider the requested leave and inform all appropriate parties of its decision in writing. 6. The application should be prepared in quadruple --one copy for the building administrator, one copy for the LEA, one copy for the individual requesting sick leave assistance and one copy to the business office. 7. The maximum number of days to be used from the assistance program by all members shall not exceed a total of one hundred sixty (160) days per year. Distribution of days shall be determined by the timeliness of the application, by who applies first as determined by the LEA Committee and the receipt of the application by the business manager.

All certified teachers working more than twenty (20) hours per week and who contribute to the Program are eligible to participate. 2. To be eligible to participate in the Program, participants must comply with the following additional requirements: a. All new or first year employees (who have never contributed before) that are eligible to use the Sick Leave Assistance Bank must contribute one day in order to be eligible for the bank. All other employees must contribute one sick day per year into the bank as per 2e. b. A participant must have used at least ten (10) days of paid and/or unpaid leave during the school year before receiving leave assistance. c. A sick leave assistance contribution form will be filled out and returned to the business manager by September 10th of each year by each new employee (see 2a) or by all other staff when a contribution is necessary (see 2e below). Employees may voluntarily contribute one or more days to the bank on years that they otherwise would not be required to do so. d. Certified teachers in the system must contribute during the enrollment period which is the first 30 days of teachers employment in each school year in order to be eligible to participate in the program. e. If at the end of the school year the total number of contributed days in the bank falls below five hundred (500) days, the following school year will be a contribution year and any participant who wishes to remain eligible for the sick bank must contribute at least one day. 3. This bank is for the protection of individual participating teachers during a serious health condition. The teacher may use the sick bank for a family member defined as:  Spouse, and parents thereof;  Children, included adopted children, and spouses thereof;  Parents;  Brother and sister thereof;  Any individual related by blood or affinity whose close association with the employee is the equivalent of a family relationship. The eligible teacher may request days from the bank not to exceed forty (40) days in one school year. Sick Leave Assistance Program. 1. An employee who has exhausted all available sick leave and absorbed five (5) continuous dock days, may receive a maximum of ten (10) days from the Sick Leave Assistance Program. 2. Teachers wishing to donate days to the assistance program may donate no more than two (2) days per year. The Board will bear the cost of the donated sick days and the donor will match that cost in order to make two (2) days available for every one (1) day donated. a. At the time of the donation, the donor must match the Board cost of donating that day. 1) Cost will be based on the receiving teacher’s daily rate of pay plus Board paid retirement, Medicare and Xxxxxxx’x Comp. 2) Matching can be accomplished by paying the board directly or signing up for payroll deduction, to be deducted from no more than two pay periods. b. Sick days may be donated only in whole day increments. 3. A joint committee consisting of three (3) Association officers, the Superintendent and the Treasurer will establish the guidelines. 4. To be eligible, an employee must make a $25.00 contribution to the Sick Leave Assistance Fund prior to September 30th. New hires, after September 1st, may join by making their contribution within thirty (30) days after their employment date. Payments may be made directly to the fund or by payroll deduction. EHTA will provide enrollment and payment information. 5. Using the established guidelines, the committee will determine eligibility on an individual case basis. 6. Employees who wish to offset the donor’s cost of donating a day, may choose to have a payroll deduction that is forwarded to the East Xxxxxx Employee Foundation, to be administered by the Association. Donating employees sick days shall be reimbursed by the Foundation, up to 100% of the docked amount, dependent upon Foundation funds available.

(f) The maximum number of days that may be donated to a given teacher in school year is the number of days it would take to enable that teacher to be eligible for long-term disability insurance, up to a maximum of sixty (60) days in a given school year. (g) Personal business days (PHMTA replaced with “sick days) to be assigned to the applicant will be chosen by lottery of donated days conducted by PHMTA. (h) Donors whose days are drawn will be notified in writing that the days will be subtracted from their allotment and indicated on their pay stub. This deduction may not be immediate since the extended illness may occur over several pay periods. (i) Donated days in excess of the days used will be returned to the donors in a timely manner. (j) Donated days must be received in the Business Office before the end of the payroll period in which they will be charged. Days will not be credited retroactively.

  • Dependent Care Assistance Program The County offers the option of enrolling in a Dependent Care Assistance Program (DCAP) designed to qualify for tax savings under Section 129 of the Internal Revenue Code, but such savings are not guaranteed. The program allows employees to set aside up to five thousand dollars ($5,000) of annual salary (before taxes) per calendar year to pay for eligible dependent care (child and elder care) expenses. Any unused balance is forfeited and cannot be recovered by the employee.

  • Sick Leave Plan The benefits of the Company’s Sick Leave Plan shall be considered as part of this Agreement. However, it is recognized that its provisions are not an automatic right of an employee and the administration of this plan and all decisions regarding the appropriateness or degree of its application shall be vested solely in the Company. The Company’s Sick Leave Plan will provide that probationary and regular employees will commence with a credit of eight days at 100 percent (100%) and 15 days at 75 percent (75%) pay, payable from the first day of sickness. This credit will continue to be available until the employee attains his/her first annual accumulation date as a regular employee. At the time of this accumulation date and each subsequent accumulation date he/she will acquire additional credits of eight days at 100 percent (100%) pay and 15 days at 75 percent (75%) pay. The accumulation of credits will be subject to the provisions of the Company’s Sick Leave Plan. Regular part-time employees shall receive a pro-rated number of sick days. When a regular part-time employee is absent due to illness on a scheduled day of work, they shall be paid for the hours of work scheduled for that day provided sick leave credits are available. Normally employees will be expected to arrange routine medical or dental appointments during non-working hours. Where such appointments cannot be arranged during non-working hours and the employee can be released from his/her duties, then the time shall be charged against an employee's sick leave time except in the case of medical appointments of less than half a day where normal earnings will be maintained. Employees who are on sick leave for 30 days or more may be eligible to participate in a vocational rehabilitation program in accordance with the Company’s policy. All major medical absence forms will be completed for any absence of four (4) continuous days/shifts or more or when requested by management. The Company will compensate the employee for the cost associated with completing these forms up to a maximum of $30.00. Additionally, the company will compensate the employee for the full cost of all medical notes, medical forms or medical information required to support LTD or other Wellness programs. This provision applies to Doctor’s notes requested by Line Management as part of the administration of the sick leave plan. Employees will be required to submit all forms required by management through their personal physician. Sick Leave benefits are conditional upon receipt of these forms and it is the responsibility of the employee to ensure that the employer receives these forms within a reasonable period of time. Any discipline related to sick leave that is imposed and grieved by the union will be referred directly to Xxxxxx Xxxxxxxxx for resolution.
