Short Term and Long Term Disability. 13.15.1 Temporary and casual employees who have elected benefit coverage and permanent employees will participate in the Short Term and Long Term Disability Plans.
13.15.2 Employees will pay the premium costs by payroll deductions.
13.15.3 Current Income Tax regulations state that when premiums are entirely employee paid, any disability benefits received by employees are non-taxable.
13.15.4 Short Term Disability coverage is provided according to the terms of the contract with the insuring company on the following general basis:
(a) Employees will receive 65% of their weekly salaries to a maximum of $500 per week;
(b) The maximum length of benefits is 52 weeks.
(c) Benefits start at the end of paid Sick Leave if the absence exceeds 10 consecutive working days.
13.15.5 Long Term Disability coverage is provided according to the terms of the contract with the insuring company on the following general basis:
(a) Employees will receive 65% of their monthly salaries to a maximum of $1720 per month;
(b) Benefits are payable until retirement or as stated in the insurance contract.
(c) Benefits start at the end of Short Term Disability coverage.
13.15.6 If the Union wishes to increase the level of coverage for Short Term or Long Term Disability, it will do so through the College Benefits Committee.
13.15.7 While the premiums are entirely employee paid, the College will not make any changes to the Short Term or Long Term Disability Plans without the approval of the Union.
13.15.8 Any medical information required by the insurance company may be sent by employees directly to the company.
13.15.9 Any amount of Short or Long Term Disability benefits paid according to this Article which are later recovered under a motor vehicle insurance contract will be repaid to the disability insurance company.
Short Term and Long Term Disability. The Library will provide eligible full-time and regular part-time employees with short term disability benefits and long term disability benefits as set forth in the applicable Plan.
Short Term and Long Term Disability. Effective as soon as practicable after January 1, 2015, and subject to a carrier’s minimum enrollment requirements, employees may purchase Short Term Disability (STD) and/or Long Term Disability (LTD) insurance through an external insurance company when not otherwise covered by Workers Compensation. These supplemental benefits are intended to offer additional income protection to employees who experience a personal serious health condition, are unable to perform the essential functions of their position, and have exhausted all accumulated sick leave. Final approval of STD and LTD benefits will be determined by the insurance company and summary plan document. All disability payments approved through these benefit plans are made by the insurance company directly to the employee and are not considered time worked through the City and are not reported as wages earned by the City. If elected by the employee, premiums are fully employee- paid. Supplemental Short Term Disability (STD) provides income protection up to 26 weeks of disability. All accumulated sick leave must be exhausted before an employee is eligible to apply for STD benefit payments. Supplemental Long Term Disability (LTD) provides income protection from 26 weeks of disability up to age 65, Police and Fire Disability and Pension Fund Retirement or Social Security. All accumulated sick leave must be exhausted before an employee is eligible to apply for LTD benefit payments. Employees approved for Long Term Disability insurance will be required to voluntarily resign from their position with the City to receive the LTD benefit payments. In special cases, the Director of Administration and/or Mayor may waive this requirement. Cost of coverage is based upon age and salary. Individual rates will be provided during the enrollment period.
Short Term and Long Term Disability. 22.01 The Board will provide a self insured short term disability benefit plan for all employees on the following basis:
(a) Each regular employee who has successfully completed his/her probationary period, who gives proof satisfactory to the Board of disabling illness or non- compensable accident shall be paid short term disability on the basis of full pay for the first three (3) weeks of absence due to such illness or accident. Employees will be paid for an additional one and one-half (1.5) weeks and effective on December 31, 1993 an additional two (2) weeks for each year of service to a maximum of seventeen (17) weeks at one hundred percent (100%) of their regular pay. Employees absent in excess of the period for which they are entitled to receive one hundred percent (100%) of their pay will receive seventy- five percent (75%) for that period until seventeen (17) weeks are completed, and deductions for C.P.P., E.I. and OMERS will continue during this seventeen (17) weeks “Short Term” disability period.
(b) During the “Short Term” period of disability, periods of disability separated by less than ten (10) full working days of active employment will be considered the same period of disability unless disability is due to unrelated causes. During the “Short Term” disability period, the employees benefit plans will be continued.
(c) Short Term disability will be paid out at the employee’s standard daily wage as of the first day of absence.
(a) A regular employee who has satisfactorily completed his/her probationary period, who gives proof satisfactory to the Board of disabling illness or non-compensable accident, shall be paid in accordance with Section 22.01.
(b) An employee may be required to produce proof of sickness after three (3) consecutive working days of absence in the form of a medical certificate.
22.03 Any employee prevented from performing his/her regular work with the Employer due to an occupational accident which is recognized by the Workplace Safety & Insurance Act as compensable within the meaning of the Act, shall receive his/her regular salary from the employees’ Short Term Disability and in return the Employer shall receive any monies paid by the Workplace Safety & Insurance Board.
22.04 For the purpose of this Article:

Short Term and Long Term Disability. The Hospital will continue to pay 32 the full cost of coverage under the group long-term disability program for eligible full- 33 time nurses. The Hospital will continue to pay the full cost of coverage under the group 34 short-term disability and long-term disability programs for eligible full-time and eligible 35 part-time nurses, subject to the provisions of Section 7.1 above.
Short Term and Long Term Disability. The District shall provide each employee with Short-Term Disability Insurance. The Short-Term Disability Insurance shall provide for sixty percent (60%) of regular weekly salary, to a maximum of $1,667 weekly benefit, after a 29-day waiting period. Benefits continue for a maximum of one year, if totally disabled. Integration of short-term insurance benefits and sick leave is to be automatic; the District may not waive integration. Long Term Disability Insurance shall provide 66 2/3% of regular monthly salary, to a maximum of $6,000 monthly benefit, after 365 days of short term disability coverage. Both Short Term and Long Term Disability Insurance benefits cease the day the termination of employment occurs.
Short Term and Long Term Disability. Victoria University has provisions for both short-term and long-term disability leave. All faculty members and librarians must belong to the long-term disability insurance plan.
Short Term and Long Term Disability. .01 Suppression Staff: After eight (8) calendar days of illness (regardless of whether work was scheduled during that time or not), or on the day of injury, the City’s short term disability plan comes into effect for the firefighter. Upon the expiration of the short term disability (17 weeks), the City’s long term disability comes into effect. The firefighter will continue to pay their portion of all regular and statutory deductions and benefits, including STD and LTD. XXXX and RSP contributions are the responsibility of the firefighter to manage through the respective program providers.
.02 Non-suppression Staff: After seven (7) calendar days of illness (regardless of whether work was scheduled during that time or not), or on the day of injury, the City’s short term disability plan comes into effect for non-suppression staff. Upon the expiration of the short term disability (17 weeks), the City’s long term disability comes into effect. The employee will continue to pay their portion of all regular and statutory deductions and benefits, including STD and LTD. XXXX and RSP contributions are the responsibility of the firefighter to manage through the respective program providers.
.03 Vacation, sick time and stat bank time are not accrued while on STD or LTD leaves.

Effective as of the Distribution Date, Wireline (acting directly or through a member of the Wireline Group) shall establish a short term disability program and a long term disability program with benefits that are the same as those under the corresponding NTELOS programs immediately prior to the Distribution Date. Effective as of the Distribution Date, Wireline (acting directly or through a member of the Wireline Group) shall assume all Liabilities (a) with respect to short term disability benefits for Wireline Participants who (i) were receiving short term disability payments as of the Distribution Date or (ii) incurred an illness or injury before the Distribution Date which would entitle such Wireline Participants to receive short term disability payments beginning on or after the Distribution Date and (b) with respect to long term disability benefits for Wireline Participants who are or who become eligible for long term disability benefits. With respect to each Delayed Transfer Employee, effective as of such Delayed Transfer Employee’s Transfer Date, Wireline (acting directly or through a member of the Wireline Group) shall assume all Liabilities (a) with respect to short term disability benefits under NTELOS’ short term disability program to such Delayed Transfer Employee who (i) was receiving short term disability payments as of the Delayed Transfer Employee’s Transfer Date or (ii) incurred an illness or injury before the Delayed Transfer Employee’s Transfer Date which would entitle such Delayed Transfer Employee to receive such short term disability payments beginning on or after such Delayed Transfer Employee’s Transfer Date, or (b) with respect to long term disability benefits for any Delayed Transfer Employee who is or who becomes eligible for long term disability benefits. With respect to each NTELOS Delayed Transfer Employee, effective as of delay such NTELOS Transfer Employee’s Transfer Date, NTELOS (acting directly or through a member of the NTELOS Group) shall assume all Liabilities (a) with respect to short term disability benefits under Wireline’s short term disability program to such NTELOS Delayed Transfer Employee who (i) was receiving short term disability payments as of the NTELOS Delayed Transfer Employee’s Transfer Date or (ii) incurred an illness or injury before the NTELOS Delayed Transfer Employee’s Transfer Date, which would entitle such NTELOS Delayed Transfer Employee to receive such short term disability payments beginning on ...
