Sick Leave Procedures. The supervisor shall notify the Appointing Authority at or before his/her normally scheduled starting time of any illness. Supervisors utilizing leave under this Section shall furnish a statement from a medical practitioner upon the request of the Appointing Authority if it has reason to believe that a supervisor has abused or is abusing sick leave. Those supervisors who misuse sick leave may be subject to disciplinary action. The Appointing Authority may also request a statement from a medical practitioner if the Appointing Authority has reason to believe the supervisor is not fit to work or has been exposed to a contagious disease which endangers the health of other supervisors, employees, clients or the public. Supervisors returning from extended sick leave shall notify the Appointing Authority within a reasonable amount of time prior to returning to work.

Sick Leave Procedures. Sick leave may not be taken in excess of that actually accrued. Continuance of pay during absence from duty due to sickness or other approved reasons, shall depend upon compliance with the following procedures: 1. On the first day of absence from duty, the employee, or someone on their behalf, shall notify the City of the reason for such absence. If the duration of the illness lasts longer than one day, the employee must notify the Department Head daily. 2. Within 36 hours after returning to duty such employee shall fill out, and file with the Department Head, a written report and request for approval of the absence as sick leave. 3. An employee may be required to furnish a certificate from a licensed physician or practitioner to support their sick leave claim. 4. Except for industrial accidents as provided in Article 15(c)(1), when an employee receives California State Disability Insurance as a result of an on the job injury or illness, the City will pay the difference between the benefit and full pay, limited to accumulated sick leave. The employee must report the amount of payment and the period which it represents to the City. 5. Any fraction of time over an hour shall be charged to the next quarter (.25) hour. 6. An employee who is admitted to a hospital or confined to bed under medical orders while on vacation leave may have the period of illness charged to their accumulated sick leave instead of vacation leave if, immediately upon return to duty, the employee submits to the Department Head a written request for sick leave and a written statement signed by their physician describing the nature and dates of illness and the period of disablement, and the Department Head recommends and the City Manager approves the granting of such sick leave. 7. Sick leave shall not be applied to absences which occur on a day designated as a City holiday. 8. If an employee works anytime, either on their regular shift or overtime on a day in which they take sick leave, the combination of hours worked, and sick leave shall not exceed 8 hours. 9. In cases of disability certified by an employees' personal physician, the City has the right to obtain a second medical opinion to confirm the employee's ability or inability to work. Such examination will be performed by a licensed physician of the City's choosing and at the City's expense. Sick Leave Procedures. A. Initial Enrollment 1. The enrollee will sign and forward a Sick Leave Bank Program Card to the Union for necessary processing. This card will be the same size as the present Application for Leave Card and will contain authority for the initial deduction of one day sick leave upon enrollment in the program. Additionally, the card will authorize the Union to deduct an additional day of sick leave from the employee, if the Sick Leave Bank is depleted as specified in Section 5. 2. Upon completion of internal processing by the Union, the card and a transmittal listing will be forwarded to the Payroll Department. The Payroll Department will deduct a day of sick leave from the employee s leave balance. 3. For those employees who do not have the required leave balance for the deduction, the authorization card will be returned to the Union with the notation "NO LEAVE BALANCE”. The Sick Leave Bank Committee shall notify employees with an insufficient leave balance that they were unable to enroll in the Sick Leave Bank. B. Subsequent Deduction After Initial Enrollment Subsequent deduction as described in Section 5 shall be accomplished by utilizing the Sick Leave Bank Program Card which will be checked in the box titled “Subsequent Deduction”. This card need not be signed by the employee since the initial card authorized subsequent deductions. This card will be signed and dated by an authorized member of the DCSAA Sick Leave Bank Committee. The cards will then be forwarded to the Payroll Department. Upon receipt of the cards and transmittal listing by the Payroll Department, the individual Payroll Data Cards will be processed per 7(A)(2) above. The DCSAA Sick Leave Bank Committee will notify affected employees when subsequent deductions are made from participants. Insufficient leave balance to permit deduction will be returned to the DCSAA Sick Leave Bank Committee per 7(A)(3) above. The Sick Leave Bank Committee shall, on an individual basis, notify employees with an insufficient leave balance that they are no longer enrolled in the Sick Leave Bank and advise them of subsequent enrollment procedures. C. Granting of Sick Leave When employees are granted sick leave from the DCSAA Sick Leave Bank, the DCSAA Sick Leave Bank Committee will utilize the Sick Leave Bank Program card to notify the Payroll Department. Sick Leave Procedures. If the need for sick leave is foreseeable, employees shall provide reasonable advance notification to their supervisor/designee, as provided below. When the need for sick leave is unforeseeable, employees must provide notice of the need for leave as soon as practical under the circumstances, but if possible shall make reasonable efforts to provide notice in accordance with the requirements below. A. If a substitute is required: The employee shall notify the District’s automated system and their supervisor/designee when requested, of their absence at least two (2) hours (if possible) prior to the starting time of their work day, unless conditions make notification impossible. When an employee cancels an absence request made through the automated system, the attendance clerk’s voice mail must also be notified. B. If no substitute is required: The employee must notify the attendance clerk (or the automated system), and their supervisor/designee prior to the beginning of the workday. C. Taking sick leave while on duty: The employee must notify their supervisor/designee of the reason for taking sick leave during mid-work shift.

A reserve Flight Attendant must maintain daily contact with Crew Scheduling when on sick leave. 2.
